Book excerpt: BackgroundControversy has surrounded endoscopic paediatric nasal surgery due to concerns over adverse effects on nasal and facial growth.ObjectivesTo evaluate the impact of endoscopic sinonasal and skull-base surgery on craniofacial growth in children under 2 years.MethodsWe report a case-series of extended endoscopic paediatric sinonasal and skull-base surgery in children under the age of 2 years conducted at a UK tertiary paediatric centre. All cases underwent endoscopic resection of anterior and midface tumours with appropriate reconstruction. Preoperative and subsequent surveillance imaging over the following 2-5 years was performed.ResultsFive cases are presented with a mean age of 10 months (range; 6-17months). Facial regrowth and satisfactory bony remodelling (including lamina-papyracea and cribriform-plate) is seen in all cases to date. No facial asymmetry has been noted.ConclusionsExtended endoscopic sinonasal and skull-base surgery in very young children can be performed without affecting ongoing facial growth.

Management of cleft lip and palate requires a team of professionals with surgical and orthodontic treatments being the largest role players. Craniofacial growth of cleft lip and palate patients has been found to differ from that of normal individuals and this could play a major part in the treatment needs and the treatment outcome. As sugery is required in management of cleft lip and palate patients but surgical scar later has adverse effects on growth. This research work was initiated to find out the effect of surgery on craniofacial growth by comparing operated and unoperated patients cephalometrically.
