Book excerpt: The purpose of this study was to determine the effectiveness of the Breastfeeding Peer Counseling Program at the Chippewa County WIC clinic in Wisconsin. The study examined the relationship of sociodemographic variables to breastfeeding rates as well as factors promoting breastfeeding and barriers to breastfeeding in a sample of WIC participants. Effectiveness was determined by comparing program goals to pre-and post-implementation duration rates, and participant's ratings and recommendations of the peer counselor. Pre-and-post implementation data for all WIC mothers and prenatal WIC mothers were obtained using secondary data. In addition, a survey was administered to 68 women as they entered the WIC clinic for their individual WIC appointments, and data were analyzed for sociodemographic variables related to breastfeeding rates. Subjects were 18 years or older with infants born between May 1, 2010 and January 31, 2011. Most subjects were White ranging in age from 18 to 42. Approximately, half were employed and had completed technical school/some college. Findings showed that breastfeeding goals were significantly higher than breastfeeding duration rates. Significant, positive relationships were found for maternal age and duration rates, and maternal age and exclusive rates. Mothers with highest educational attainment had significantly higher breastfeeding duration rates. No other significant findings were observed in the study. Findings related to program effectiveness showed that all pre-to post-implementation duration rates increased, except for the 6 month rate, and percentage increases exceeded the targeted goals of the Peer Counseling Program; all participants who rated the peer counselor's assistance gave the highest possible rating; and all who used the Program would recommend it to other mothers.

Book excerpt: In an effort to improve breastfeeding, the Oregon WIC Program tested whether a relatively low-cost telephone peer counseling initiative to support breastfeeding could increase the initiation and duration of exclusive breastfeeding among its participants. They conducted a large randomized field experiment (RFE) with over 1900 women from four WIC agencies in the state. In this study we use data from the RFE along with administrative data from the rest of the state to assess whether the results from the RFE can be extended to other agencies in the state. We find small or non-existent effects of peer counseling in the non-experimental settings, which suggest that the experimental estimates may reflect Hawthorne effects. We present evidence of selection into RFE in that exclusive breastfeeding among the controls is significantly greater than among women who were offered but declined to participate in the RFE as well as from women in the rest of the state who had no access to peer counseling. We conclude that despite the strong internal validity of the RFE, extending the program to other agencies in the state would have a limited impact at best on exclusive breastfeeding.

The first one studies the product market segmentation and output collusion within substitutes; the second one examines the effects of WIC program and peers on breastfeeding activities. In Chapter I, we extend the differentiated product model, first developed by Bowley (1924), by relaxing the assumption that each firm produces only one differentiated product. By doing so, we are able to analyze the potential for collusive market segmentation in a two stage decision framework, first in product space and second in output. We find that when firms cannot coordinate on output, the required discount factor that supports collusive market segmentation is strictly decreasing in product substitutability and is greater than partial output and full collusion. Overall we find that output collusion alone is easier to sustain than collusive product market segmentation. In Chapter II, we first use duration analysis techniques to estimate the effects of WIC participation on breastfeeding activities using a nationwide data. Income ineligible participants are excluded from the sample. The models with and without peer effects variables are both estimated. We find that the prenatal WIC participation status does not have significant effect on breastfeeding activities. Peer effects have significant positive effects on both partial and exclusive breastfeeding durations but not on breastfeeding initiation. The magnitude of the peer effects on each individual is different and depends on the individual’s propensity to breastfeed. The results on peer effects based on the full sample are consistent with the findings from restricted sample. Knowing more than five peers who breastfed increases the probability of initiating by 3.7% and the likelihood of breastfeeding at months 3 and 6 by more than 15%. It also increases the partial and exclusive breastfeeding durations by 9 and 3 weeks, respectively. The results suggest the presence of a social multiplier in breastfeeding. Any exogenous change in breastfeeding behavior due to policy interventions would result in an even greater change due to the bidirectional influences within peer groups. Peer effects play a more important role for breastfeeding duration than initiation.

Today the program serves more than a quarter of the pregnant women and half of the infants in the United States, at an annual cost of about $6.2 billion. Through its contribution to the nutritional needs of pregnant, breastfeeding, and post-partum women; infants; and children under 5 years of age; this federally supported nutrition assistance program is integral to meeting national nutrition policy goals for a significant portion of the U.S. population. To assure the continued success of the WIC, Congress mandated that the Food and Nutrition Service of the U.S. Department of Agriculture (USDA) reevaluate the program's food packages every 10 years. In 2014, the USDA asked the Institute of Medicine to undertake this reevaluation to ensure continued alignment with the goals of the Dietary Guidelines for Americans. This, the second report of this series, provides a summary of the work of phase I of the study, and serves as the analytical underpinning for phase II in which the committee will report its final conclusions and recommendations.

The magnitude of the problem has increased dramatically during the last three decades and, despite some indications of a plateau in this growth, the numbers remain stubbornly high. Efforts to prevent childhood obesity to date have focused largely on school-aged children, with relatively little attention to children under age 5. However, there is a growing awareness that efforts to prevent childhood obesity must begin before children ever enter the school system. Early Childhood Obesity Prevention Policies reviews factors related to overweight and obese children from birth to age 5, with a focus on nutrition, physical activity, and sedentary behavior, and recommends policies that can alter children's environments to promote the maintenance of healthy weight. Because the first years of life are important to health and well-being throughout the life span, preventing obesity in infants and young children can contribute to reversing the epidemic of obesity in children and adults. The book recommends that health care providers make parents aware of their child's excess weight early. It also suggests that parents and child care providers keep children active throughout the day, provide them with healthy diets, limit screen time, and ensure children get adequate sleep. In addition to providing comprehensive solutions to tackle the problem of obesity in infants and young children, Early Childhood Obesity Prevention Policies identifies potential actions that could be taken to implement those recommendations. The recommendations can inform the decisions of state and local child care regulators, child care providers, health care providers, directors of federal and local child care and nutrition programs, and government officials at all levels.
