Book excerpt: This study tested the hypothesis that an active-passive group music intervention will decrease anxiety among individuals diagnosed with mild to moderate dementia in assisted living facilities (ALFs) as compared to a control group engaged in care as usual. This study also described the feasibility and acceptability of active-passive group music interventions delivered by a Registered Nurse (RN) without a university or college degree in music intervention techniques to individuals with mild to moderate dementia in an ALF. The study used a quasi-experimental cross-over design with a simplified cluster randomization strategy; simplified because there were only two sites. Thirteen participants were invited, 8 from one facility and 5 from the other. The sample comprised of participants ranging from age 74 thru 98 with Global Deterioration Stages ranging from 2-5. The majority of participants were female, White and were college educated. Music was considered moderately important to very important. Music was used to effect anxiety levels. Altogether 12 intervention sessions were offered or 6 for each arm. There was a 4 week wash-out period. Four RAID measures were taken during each arm at baseline. Specific Aim 1: Insignificant decreases to anxiety existed by group, however, a trend showing decreases to anxiety levels existed over time (p=0.002): Intervention Group 1 = 18.00 at point 1 to 14.83 at point 4; Intervention Group 2 = 17.20 at point 1 to 10.80 at point 4. During control periods, RAID were: Group 1= 13.33 at point 1 to 10.83 at point 4; and Group 2 = 13.60 at point 1 to 10.40 at point 4. Specific Aim 2: With music education, the use of RNs to deliver group music interventions and supervise music interventions in healthcare is promising.

Patients experience anxiety related to the uncertainty of any surgical procedure. Increased levels of pain and anxiety can lead to negative effects both physiologically and psychologically. Narcotics and sedatives are most commonly administered to alleviate both of the conditions. When given in large amounts and coupled with anesthetic drugs, adverse effects such as over-sedation can occur. This is an adverse effect that can be problematic and costly for not only the patient, but the nurse and facility as well. Respiratory depression and over sedation go hand-in-hand. When over sedation occurs, a patients respiratory effort can be greatly impacted. Over-sedation and other adverse effects can prolong not only the recovery period, but ultimately delay patient discharge. By adding a non-pharmacologic intervention such as music therapy to the treatment plan, the nurse will be able to reduce pain and anxiety, thus reducing the amount of narcotics and sedatives required. This can be used as a nursing driven intervention to be implemented in the pre-operative phase and follow the patient through the post-anesthesia phase. With the appropriate implementation plan, stakeholder acceptance, and resources, music therapy can be a beneficial influence on physiological parameters and psychological state. Music therapy can provide relaxation, lessen muscle tension, and provide a healing influence for patients. Music therapy can have a great impact on not only patient satisfaction, but anesthesia outcomes as well. Literature has shown that music therapy used as a complementary therapy has been proven as an inexpensive way to reduce perioperative pain and anxiety.

Book excerpt: Patients coming into a facility for invasive procedures often times have a high level of anxiety about the procedure and the outcome, (Ghetti, 2012). Based on research studies, the theory of music therapy intervention can reduce the patient's anxiety before and during an invasive procedure, (Bradt, Dileo and Shim, 2013). Music is a welcome distraction proven to help the patient relax, lower blood pressure, heart rate, respiratory rate and cover up environmental noise, (Comeaux and Steele-Moses, 2013). Data reveals some patients need less sedation and pain medication to lessen base-line vital signs and anxiety levels if music therapy is used, (Matsota, Christodoulopoulou, Smyrnioti, Pandazi, Kanellopoulos, Koursoumi, E., and others, 2013). By implementing music therapy in phases through out the facility, education can be audited through the PDCA model, Plan Do Check Act for process improvement. Starting in the preoperative preparation area for patients having heart catheterizations will allow ample control over the study with less staff to train in the start of the project. This plan allows for the implementer to oversee the study with the assistance of the research committee. Patients will be educated on the trial when checking in for the procedure and given an option to participate. A VAS-A, Visual Analog Scale for Anxiety will be used before and after the procedure, (Abend, Dan, Maoz, Raz, and Bar-Haim, 2014). Patient satisfaction scores will be analyzed before and after the trial. The ultimate goal is to improve patient satisfaction scores by reducing level of anxiety, with the added benefit of possibly lowering cost per case. As evidence of less sedation involved allowing the patient to be dismissed sooner, (Bernatzky, Presch, Anderson and Panksepp, 2011).

Book excerpt: The purpose of this pilot study was to assess the effects of music therapy on comfort in acute mechanically ventilated patients in the Intensive Care Unit. Mechanical ventilation leads to decreased comfort. Pain and anxiety may increase during this treatment modality, and the literature suggests this may directly affect patient comfort levels. Music therapy as a nursing intervention within the context of comfort, pain, and anxiety of mechanically ventilated patients was investigated. This quasi-experiemental study used a pre-test and post-test design with subjects serving as their own control. The convenience sample consisted of 2 men and 3 women who were mechanically ventilated and fit the selection criteria. Dependent variables measured included comfort, pain, and anxiety. Physiologic dependent measures included heart rate, respiratory rate, and systolic and diastolic blood pressure collected at timed intervals. Comfort was measured using a modified version of the Hospice Comfort Questionnaire. Pain was measured using the Numerical Graphic Rating Pain Scale. Anxiety was measured using the Faces Anxiety Scale. Important preliminary quantitative results are provided by this pilot study. The mean, standard deviation, significance, and paired t-tests were compared for each tool to determine changes in scoring before and after the intervention and control. This was also completed for physiological data including systolic and diastolic blood pressure, respiratory rate, and heart rate. Paired t-tests showed no significant mean differences between two points of measurement on systolic, diastolic, heart rate, and respiratory rate in both the intervention and control groups. Comfort, anxiety, and pain scores before and after the intervention and control also did not demonstrate significance. The small sample size makes generalization of these findings impossible to the entire population of acute mechanically ventilated patients in Billings, Montana.

This literature review identifies research which applies passive music listening to distressed neonatal and pediatric acute care patients. Databases searched to find relevant studies include CINAHL plus with full text, MEDLINE, Alt HealthWatch, APA PsycArticles, and APA PsycInfo from EBSCOhost. Six studies were identified to meet search criteria. The studies that supported music listening with the neonatal and pediatric populations had a positive effect in reducing distress levels. Additional research is warranted to further validate these findings. Music listening with neonatal and pediatric patients is a simple, cost-effective intervention that nurses can implement at the bedside.
