Book excerpt: These studies confirm that people with uncomplicated type 2 diabetes have a blunted stroke volume response during exercise, and this response is caused by attenuated left ventricular end diastolic volume at higher heart rates. HIIT increases stroke volume by increasing left ventricular end diastolic volume and augmenting the reduction in left ventricular end systolic volume during exercise independent of changes in total blood volume. This suggests that changes in the diabetic heart can be affected by HIIT.

Book excerpt: In patients with type 2 diabetes (DM2) arterial stiffness, heart rate variability (HRV), high sensitivity C-reactive protein (hs-CRP) and hyperaemic calf blood flow were measured before and after 12 weeks of combined aerobic and resistance exercise training. Arterial stiffness and HRV were also measured after 20 minutes of moderate, aerobic exercise pre and post exercise training. A single acute bout of aerobic exercise results in significantly elevated central arterial stiffness both before and after 12 weeks of training whereas AAI is reduced prior to training only. After 12 weeks of training, significant improvements were found in: C-F PWV (p=0.026); augmentation index (p= 0.005); peak oxygen uptake (p=.002) fasting plasma glucose (p=0.041), abdominal circumference (p=0.001), BMI (p=0.004) and body fat (p=0.032). Improvements in arterial stiffness were not associated with any change in anthropometric measurements, aerobic fitness, hs-CRP or indices of heart rate variability.

Book excerpt: Cardiometabolic diseases such as diabetes and cardiovascular disease are a growing threat to the quality of life of the population. Exercise is a frontline approach to treat and prevent cardiometabolic disease and its associated risk factors. The majority of individuals, however, are physically inactive and fail to meet weekly physical activity guidelines primarily due to time-constraints. High-intensity interval training (HIIT) is a time-efficient method of exercise for improving physical fitness and reducing cardiometabolic risk factors compared to moderate-intensity continuous training (MICT). Recent research indicates that the number of repetitions and duration of high-intensity intervals can be reduced without attenuation of health benefits. This study recruited nine physically inactive but otherwise healthy participants (6 female, 3 male) which engaged in a 6 week reduced-exertion HIIT protocol. Participants were assessed before and after a 2 week run-in period, and again upon completion of the exercise protocol to assess the effect on predicted aerobic capacity (VO2max), resting heart rate (HR), resting blood pressure, heart rate variability (HRV), fasting blood glucose, peak power, mean power, body mass, and body fat %. Participants improved predicted VO2max F(2,16)=6.33, p=.009, peak power F(2,16)=10.84, p=.001, and mean power F(2,16)=20.87, p=.00006, but no changes were observed in body mass, body fat %, resting HR, resting blood pressure, resting HRV, and fasting blood glucose. In conclusion, a reduced-exertion HIIT protocol with minimal time-commitment improved predicted VO2max, peak power, and mean power and is a time-efficient alternative or adjunct method of exercise for eliciting health benefits in physically inactive individuals.

Book excerpt: Achieving good control over glucose concentrations is the fundamental therapeutic goal for individuals with type 2 diabetes (T2D). The effects of exercise on individuals with T2D have been well received and contributed extensively to recent evidence-based exercise guidelines in several countries. However, knowledge regarding the therapeutic benefits of high intensity interval exercise (HIIE) in individuals with T2D was limited. While accumulating evidence suggested the potential benefits of HIIE, it was unknown if it confers additional benefits over traditionally recommended moderate intensity continuous exercise (MICE) in improving the glycemia of T2D. In addition, it was unknown if timing to perform such exercise impacts glycemic responses. Accordingly, a series of studies were performed to: 1) examine the feasibility and long-term efficacy of HIIE; 2) to compare the glycemic responses to HIIE and MICE during exercise; and 3) to compare the acute glycemic responses to HIIE and MICE. Results suggested that HIIE is as feasible as MICE, and, compared to MICE, lowers glucose concentrations to a greater extent during exercise, and induces greater reductions in nocturnal and fasting glucose concentrations on the day subsequent to exercise. While performing HIIE in the fasted-state attenuated the reduction of glucose concentration during exercise, it improved most aspects of dysglycemia measured over hours following the exercise bouts. Postprandial HIIE, on the other hand, resulted in the greatest reduction in glycemia during exercise; however, no glucose profiles were improved hours after exercise as compared to a sedentary day. These results suggest that HIIE has some potential advantage over MICE in improving specific aspects of glycemia, and performing HIIE during a fasted-state may be more beneficial in lowering additional measures of glycemic profiles. In conclusions, HIIE is well tolerated by individuals with relatively well-controlled T2D and effectively improves various aspects of glycemia. The effects of HIIE may be magnified by performing it under fasted-state.

On one hand it includes mitochondrial biogenesis and therefore an enhanced capacity to oxidize fuels such as glucose and fats. This implies that a combination of anaerobic and aerobic energy systems rely on muscle glycogen and/or blood glucose as an important substrate for energy production. On the other hand high intensity intermittent modalities improves aerobic respiratory control sensitivity resulting from increased mitochondrial density. High intensity and reduced volume of the workout utilizes type 2 muscle fibres. This muscle fiber has an anaerobic glycolytic metabolism and produces lactate by using glycogen. This results in the production of the growth hormones testosterone and somatropin. This research question plays an important role for people who are affected by type 2 diabetes or obesity which increases morbidity and mortality. Stroke, hypertension and heart disease are some of the most common disorders associated with these conditions.
