Book excerpt: There is much debate about the effects of pharmaceutical direct to consumer advertising (DTCA) on health care use. In this paper, we inform this debate by examining the effects of DTCA on office visits, as well as treatment courses resulting from those visits, for five common chronic conditions (hypertension, hyperlipidemia, diabetes, depression, and osteoporosis). In particular, we examine whether office visits result in use of drug therapy and/or continued office visits over time. We test these questions by combining data on pharmaceutical advertising from Nielsen with claims data from 40 large national employers, covering 18 million person-years. We analyze a non-elderly population by exploiting plausibly exogenous variation in advertising exposure across areas due to the implementation of Medicare prescription drug coverage which led to larger increases in advertising in areas with high elderly share of population compared to low elderly share areas. We find that advertising increases the number of office visits for the non-elderly for the advertised condition. We also find substantial spillovers -- a large share of the increased office visits from advertising are associated with use of non-advertised generic drugs or do not result in use of any drugs. Finally, we find that the increase in office visits due to DTCA is associated with continued engagement with a physician through multiple consecutive follow up visits over time.

At the same time, however, the United States is facing the vexing challenge of a seemingly uncontrolled rise in the cost of health care. Total medical expenditures are rapidly approaching 20 percent of the gross domestic product and are crowding out other priorities of national importance. The use of increasingly expensive prescription drugs is a significant part of this problem, making the cost of biopharmaceuticals a serious national concern with broad political implications. Especially with the highly visible and very large price increases for prescription drugs that have occurred in recent years, finding a way to make prescription medicinesâ€"and health care at largeâ€"more affordable for everyone has become a socioeconomic imperative. Affordability is a complex function of factors, including not just the prices of the drugs themselves, but also the details of an individual's insurance coverage and the number of medical conditions that an individual or family confronts. Book excerpt: Pharmaceutical firms currently spend over $4 billion on direct-to-consumer advertising (DTCA) of prescription drugs, a nearly 30-fold increase since 1993 that has led to much debate about its value to patients. We examine how DTCA influences drug utilization along the extensive and intensive margins by exploiting a large and plausibly exogenous shock to DTCA driven by the introduction of Medicare Part D in 2006. Using data on advertising for local media markets from Nielsen, we show that Part D led to large relative increases in DTCA in geographic areas with a high concentration of Medicare beneficiaries compared to areas with a low concentration. We examine the effects of this sudden differential increase in advertising on non-elderly individuals to isolate the effects of advertising on drug utilization from the direct effects of Part D. Using data from pharmacy claims, we find substantial differential increases in drug utilization that mirror the increases in DTCA after Part D. These effects are driven both by increased take-up of treatment and improved drug adherence. Our results imply significant spillovers from Medicare Part D onto the under-65 population and an important role for non-price factors in influencing prescription drug utilization.

Book excerpt: A random sample of 50 diabetes patients from a managed care company was surveyed over the telephone to determine their attitudes and responses to direct to consumer advertisements for prescription drugs (DTCA). Most of the sample were of minority race/ethnicity, older age, lower income, had lower education, and took at least five prescription medications daily. A nine- item questionnaire with a five point Likert scale was used to determine an attitude score. Overall attitudes about DTCA were negative. Respondents were asked if they had ever reacted to an ad with any of three behavioral responses (asking for information about a drug, requesting a prescription for the drug they saw advertised, requesting a change in a current prescription due to an ad). Respondents stated they asked their provider for further information about a drug due to an ad at a higher rate (40 percent) than they asked that the advertised drug be prescribed (16 percent). Respondents who asked for change in a current prescription due to an ad they saw made the request due to side effect information in the ad. Attitudes about DTCA were negatively correlated with these behaviors. The findings show this population's skepticism about ad information, their comfort discussing it with their providers, and their need for further information on risk assessment. The cost of non- formulary, non-generic medications makes patients cautious about asking for a specific advertised prescription. Providers need to be sensitive to patients concerns about DTCA information and be available to discuss risks and benefits.
