Although life expectancy and survival rates in the United States have improved dramatically over the past century, Americans live shorter lives and experience more injuries and illnesses than people in other high-income countries. The U.S. health disadvantage cannot be attributed solely to the adverse health status of racial or ethnic minorities or poor people: even highly advantaged Americans are in worse health than their counterparts in other, "peer" countries. In light of the new and growing evidence about the U.S. health disadvantage, the National Institutes of Health asked the National Research Council (NRC) and the Institute of Medicine (IOM) to convene a panel of experts to study the issue. The Panel on Understanding Cross-National Health Differences Among High-Income Countries examined whether the U.S. health disadvantage exists across the life span, considered potential explanations, and assessed the larger implications of the findings. U.S. Health in International Perspective presents detailed evidence on the issue, explores the possible explanations for the shorter and less healthy lives of Americans than those of people in comparable countries, and recommends actions by both government and nongovernment agencies and organizations to address the U.S. health disadvantage.

These health factors include health behaviors (30%), it includes tobacco use, diet and exercise, alcohol use, unsafe sex; clinical care (20%), it includes access to care, quality of care; social and economic factors (40%), it includes education, employment, income, family and social support, community safety; physical environmental factor (10%), includes natural environmental quality, built environmental quality. Then these factors can cause this health outcomes, such as morality ( length of life):50% and morbidity (quality of life):50%. SO, it implies that physical environmental factor can influence human's length of life. So, on our social environmental problems result is from a complex interplay of a number of forces. An individual's health -related behaviors, particularly diet, exercise and smoking, surrounding physical environment and health care ( both access and quality) all contribute significantly to how long and how well human love. However, none of these factors is as important to population health as are the social and economic environments in which human live, learn, work and play. We refer to these factors can be as the social determinants of health to influence our quality of life. How do social determinants affect our quality of life? In the late 19th and early 20th centuries, public health concentrated particularly on the physical environment. Improvements in, for example, clean water supplies, healthier housing, sanitation, workplace safety and safe food lead to sharp increases in average life expectancy . Also our quality of life needed to be concentrated on expanded access to medical care, resulting in further expansion. So, the poverty tap is now characterized by those elements, such as low levels of: (i) environmental quality, (ii) life expectancy and (iii) human capital.In fact, environmental degradation can have a significant impact on human health. De Hollander et. al (1999) & Melse & De Hollander (2001) showed that estimates of the share of environment, related human health loss are as high 5% for high income countries, 8% for middle income countries and 13% for low income countries. Air pollution and exposure to hazardous chemicals are important causes of the related burden of disease in countries. The transport and energy sectors are major contributors to air pollution, when important sources of chemical pollution are agriculture industry and waste disposal.

Are the preferences of the poor towards a cleaner environment really different from those of the rich? Environmental economists have traditionally focused on efficiency issues. In their analyses the quality of the environment is usually related to aggregate or average variables, like per capita income; policy recommendations are usually formulated considering efficiency with no regard for equity and also the predicted effects of policies are evaluated in aggregate terms. The essays collected in this volume go into the problem of the relationship between environmental quality and income distribution. The book’s opening essay shows how different theories of economic growth and environmental quality seem to suggest that the higher the level of income the higher is the value of environmental protection. The essays that follow, a mix of already published papers and of papers solicited for this book, analyse the relationship between environmental quality and income distribution from different perspectives (both micro and macro) and on the basis of more than one methodology. This book highlights that the preferences of the poor towards a cleaner environment may differ from those of the rich, but income is also very likely to represent only one factor affecting them. The essays consider other relevant factors affecting preferences for environmental quality. What clearly emerges is that the distribution of costs and benefits of environmental policies is the key for their successful implementation, and that further research is needed to both address the distributional effects themselves and the strategies to mitigate them.

This book contributes to an improved understanding of the economic dimensions of environmental and natural-resource management and poverty alleviation. The ten chapters of the book offer an overview of the current knowledge concerning the relation between poverty, environment and natural-resource use. Three sides of the debate receive particular attention. First, the relation between resource use and poverty is discussed from a theoretical point of view. Second, it is questioned whether payments for environmental services or considering values of resources can be an effective tool for stimulating both sustainable resource use and poverty alleviation. Mainstream neoclassical economics may be able to contribute to a more sustainable society but it has also played a dominant role in a period where problems have been aggravated. A pluralist and democratic view of economics is therefore very much warranted. This book presents a multidimensional and ideologically more open view of economics: understanding economics in multidimensional terms is in accordance with the 17 sustainable development goals recognized by nations at the UN-level in 2015. Accordingly, approaches to decision making and accounting at the national- and business levels have to be reconsidered. Neoclassical Cost-Benefit Analysis (CBA) with focus on the monetary dimension and an assumed consensus about a specific market ideology to be applied is not compatible with democratic societies where citizen and actors in other roles normally differ with respect to ideological orientation. Environmental Impact Statements and Multi-Criteria methods are used to some extent to broaden approaches to decision-making. In this book, Positional Analysis is advocated as a multidimensional and ideologically open approach. Positional Analysis is based on a political economic conceptual framework (as part of ecological economics) that differs from neoclassical ideas of individuals, firms and markets. And since approaches to decision-making and to accounting are closely connected, a new theoretical perspective in economics similarly raises issues of how national and business accounting can be opened up to meet present demands among various actors in society. This perspective raises also numerous ethical questions at the science and policy interface that need to be properly addressed for sustainability decision making.
