Book excerpt: This document summarizes published and grey literature on conceptual framework on the link between child nutrition and economic growth, determinants of child undernutrition, types of investments to enhance maternal and child nutrition, and linkages between urbanization and child nutrition. Several in-sights emerge from the review. First, and despite progresses over the last several decades, maternal and child malnutrition is still prevalent in developing countries and the progress has been uneven. While the percentage of chronically malnourished (stunted) children declined across the developing world, the number of stunted children in Africa increased due to slower reduction in stunting prevalence and population growth. Many developing countries are experiencing the coexistence of different forms of malnutrition including undernutrition, micronutrient deficiency, and overnutrition. Second, child undernutrition, especially stunting during the first 1,000 days of life, has several short- and long-term effects on individuals and economies that include impaired cognitive and non-cognitive development, poor educational performance, low productivity and earnings, and higher healthcare costs. Third, the determinants of child undernutrition are broadly classified as the immediate determinants including dietary intake and diseases; the underlying determinants that include household food security, quality of care and household living environment, and access to healthcare; and the basic determinants that include access to productive resources, stock of capital, as well as socioeconomic, political and cultural factors. Investments to enhance child nutrition can target either the immediate determinants (known as nutrition-specific investments) or the underlying determinants (known as nutrition-sensitive investments). Fourth, the effect of urbanization on child nutrition is mostly determined by the extent to which urban settlements offer their residents with better economic opportunities (e.g., better paying jobs and markets for nutritious food) and services (e.g., healthier living environments). Fifth, given the multilayer causes of child undernutrition, a multi-sectoral approach is needed to address the various determinants of undernutrition to improve maternal nutrition, promote optimal infant and young child feeding practices, enhance household food security, as well as improve healthy living environment and access to quality health care.

This work focused on finding out the bio-demographic, socio-economic and cultural factors influencing child malnutrition in Kenya. Malnutrition was considered to represent child nutritional status. Selected variables drawn from the 1998 Kenya Demographic and Health Survey(KDHS) data were analyzed both at the bi-variate and multivariate levels. The findings reveal the extent to which the selected variables influence child nutritional status in Kenya. Further, they provide a clear insight into the problem of child malnutrition. Such information is useful in developing appropriate strategies meant to promote child development and survival not only in Kenya, but also in many other countries. This work is useful for students, researchers, policy makers and implementers, and all stakeholders in the public health sector.

Malnourished children are more vulnerable to infection and if not treated on time and may fall into the vicious cycle of Malnutritional-infection-Malnutrition. The primary objective of this research is to determine whether economic growth affects nutritional status or not and to understand the effect of other socio-economic factors on Malnutrition. The study has been conducted for three states namely Gujarat, Sikkim, and Bihar. Sikkim had the highest per capita Net State Domestic Product (NSDP) while Bihar had the Lowest per capita NSDP. Researchers have compared Gujarat's nutritional situation with Sikkim and Bihar using data from NFHS-4 & 5 reports. Malnutrition Index, Household Condition Index, and Women's Condition Index have been prepared and efforts have been made to measure the socio-economic factors that influence the level of Malnutrition. The results show that despite having the lowest per capita NSDP, Bihar's nutritional status is far better than Gujarat's Nutritional status. Gujarat's nutritional status is worsened in NFHS -5 as compared to NFHS-4 despite a stable Household Condition Index. This may be because of deprivation in the Women's Condition Index. While, a different situation in Sikkim's case is observed because despite decreasing the Household Condition and Women Condition Index, it has improved its nutritional status in NFHS- 5 compared to NFHS-4.

Book excerpt: Malnutrition refers to deficiencies, excesses, or imbalances in a person''s intake of energy and/or nutrients. Child malnutrition, in all its forms, includes undernutrition, which includes wasting (low weight-for-height), stunting (low height-for-age) and underweight (low weight-for-age), micronutrient-related malnutrition (a lack of important vitamins and minerals) or micronutrient excess, overweight, obesity, and resulting diet-related non-communicable diseases. Malnutrition is one of the most important factors for improper physical and mental development of children. One in every five children in the developing world is malnourished, and poor nutrition is associated with half of all child deaths worldwide. The problem of low or excessive body weight concerns countries with different levels of socio-economic development. This is a medical, social, and economic issue. Every country in the world is affected by one or more forms of malnutrition. Combating malnutrition in all its forms is one of the greatest global health challenges. Infants, children and adolescents are at particular risk of malnutrition. In children, malnutrition has particularly significant health consequences during both early development and adulthood. Malnutrition endangers children''s survival, health, growth and development, slows national progress towards the developmental goals and thus diminishes the strength and capacity of nation. Malnutrition in terms of undernutrition is substantially higher in rural than in urban areas and children from indigenous populations often have the poorest nutritional status. The developmental, economic, social, and medical impacts of the global burden of malnutrition are serious and lasting for all involved including individuals and their families, communities and countries. Poor nutrition during the early years of life can also have severe consequences for subsequent skeletal and immunological development. Studies have demonstrated that undernutrition is not caused by food insecurity alone. Other factors range from the length of the breastfeeding period and the availability of milk oligosaccharides, pathogen exposure, and enteric dysfunction marked by villus atrophy and loss of gut barrier function. Differences in the succession of microbial establishment and maturity might contribute to family discordances in nutritional status. Childhood obesity is one of the most serious public health challenges of the 21st century. The problem is global and is steadily affecting many low- and middle-income countries, particularly in urban settings. Its prevalence has increased at an alarming rate. Globally, in 2016, the number of overweight children under the age of five is estimated to be over 41 million. Almost half of all overweight children under 5 lived in Asia. Overweight and obesity have been linked to adverse psychological and physical outcomes during childhood and continuing into adolescence and adulthood. The association between overweight and obesity with psychosocial problems (anxiety, depression and negative self-image), health problems (diabetes and cardiovascular events) and impaired social, educational and economic productivity has been well documented. Moreover, the negative impact of being underweight, overweight, or obese on the health and development of children and adolescents can also extend into adulthood, increasing the risk of chronic non-communicable diseases and disability. Overweight and obesity, as well as their related diseases, are largely preventable. Therefore prevention of childhood obesity is a high priority. The mechanism of development of obesity is not fully understood and it is believed to be a disorder with multiple causes. Environmental factors, lifestyle preferences, and cultural environment play pivotal roles in the rising prevalence of obesity worldwide. In general, overweight and obesity are assumed to be the result of an increase in caloric and fat intake. On the other hand, there are supporting evidence that excessive sugar intake by soft drink, increased portion size, and steady decline in physical activity have been playing major roles in the rising rates of obesity all around the world. Nutritional assessmentis the interpretation of anthropometric, biochemical, clinical and dietary data to determine whether a person or groups of people are well nourished or malnourished (over-nourished or under-nourished). Research can be aimed at identifying the various social, cultural, political, and economic factors of nutrition in order to fully understand the underlying causes of malnutrition. The social determinants of malnutrition can be explored through both qualitative and quantitative research methods. Furthermore, exploring the issues of food security, dietary diversity, and infant-feeding practices can provide a comprehensive understanding of a population''s nutritional status.

For each sector, we document the progress to date, assess where Pakistan stands relative to its peers, highlight key challenges, and estimate the additional spending required to make substantial progress. The estimates for the additional spending are derived using the IMF SDG costing methodology. We find that to achieve the SDGs in these sectors would require additional annual spending of about 16 percent of GDP in 2030 from the public and private sectors combined.

Malnutrition manifests itself in many different ways: as poor child growth and development; as individuals who are skin and bone or prone to infection; as those who are carrying too much weight or whose blood contains too much sugar, salt, fat, or cholesterol; or those who are deficient in important vitamins or minerals. Malnutrition and diet are by far the biggest risk factors for the global burden of disease: every country is facing a serious public health challenge from malnutrition. The economic consequences represent losses of 11 percent of gross domestic product (GDP) every year in Africa and Asia, whereas preventing malnutrion delivers $16 in returns on investment for every $1 spent. The world’s countries have agreed on targets for nutrition, but despite some progress in recent years the world is off track to reach those targets. This third stocktaking of the state of the world’s nutrition points to ways to reverse this trend and end all forms of malnutrition by 2030.
