Book excerpt: Cesare Romano revisits Dora's clinical case in light of Freud's own seduction theory. His central thesis is that Freud failed to follow through with his initial proposition of confirming his theories on the traumatic aetiology of hysteria. He also suggests a new dating for the duration of Dora's therapy, placing the beginning of the analysis within the context of Freud's concurrent and recent life events. A detailed analysis of Dora's first dream shows that Freud did not go back to Dora's first infantile traumas, but stopped instead at the period of her infantile masturbation. In analysing this dream, Romano's theory begins to take shape around the idea that Dora suffered an early trauma: possibly, a sexual abuse inflicted by her father. Drawing on Ferenczi, the author uses the notion of the 'traumatolytic function of the dream' to show that Dora, through her two dreams, was elaborating her early sexual trauma. Dora's analysis is investigated alongside what was happening in Freud's life at the time of the therapy.

Patient Tales follows the development of psychiatric case histories from their origins at Edinburgh Medical School and the Royal Edinburgh Infirmary in the mid-eighteenth century to the medical records of contemporary American mental health clinics. Spanning two centuries and several disciplines, Berkenkotter's investigation illustrates how discursive changes in this genre mirrored evolving assumptions and epistemological commitments among those who cared for the mentally ill. During the asylum era, case histories were a means by which practitioners organized and disseminated local knowledge through professional societies, affiliations, and journals. The way in which these histories were recorded was subsequently codified, giving rise to a genre. In her thorough reading of Sigmund Freud's Fragment of an Analysis of a Case of Hysteria, Berkenkotter shows how this account of Freud's famous patient "Dora" led to technical innovation in the genre through the incorporation of literary devices. In the volume's final section, Berkenkotter carries the discussion forward to the present in her examination of the turn from psychoanalysis to a research-based and medically oriented classification system now utilized by the American Psychiatric Association. All of Freud's important assumptions and characteristic modes of thought are to be found in this formative period. The placement also brings out more clearly the basis of a number of the unresolved problems of contemporary psycho-analytic theory, such as the place of affect and the instinctual drives, the role of the ego, and the basis of treatment. The core of the evaluation centres on Freud's basic method for gathering data - free association - a method which is not much written about and hardly ever criticised. What is said about it is new and more substantial than the few criticisms that have been made. Although a very critical work, there is probably no other appraisal which allows Freud and his colleagues and followers to speak so directly for themselves.
