The gluteus medium and minimus, with the tensor vagina-3 femoris, draw the thigh-bone forwards, and roll it inwards. He shewed me -a preparation 'which confirmed this opinion. I have received from Mr. Brindley, surgeon of Wink Hill, an account of a dislocation of the os femoris, which the patient is able to produce and reduce When he chooses; the man is fifty years of age. Mr. Morley, of Uttoxeter, has transmitted to me a case of compound fracture of the' head of the os humeri; the end of the bone was sawn off, the bone reduced, and. The patient did well; the length of the limb differed but little from that of the other. And Mr. White, of the West minster Hospital, has shewn me a case of dislocation of the os femoris from ulceration, in which the head of the femur was sawn off, and the person recovered. The two small volumes of Desault, and the one of Boyer, issued near the close of the last century, and translated into English early in this, may perhaps pro perly enough have been regarded as complete treatises at the time of their publication, but they certainly cannot be so considered now. The several chapters on Diseases and Injuries of the Bones, contained in the Decons Orales of Dupuytren, translated in 1846, and the Trea tise on Fractures in the Vicinity of Joints, and on Certain Forms of Accidental and Congenital Dislocations, by Robert Smith, are invaluable monographs, but neither of them claims to be anything more than a collection of occasional and miscellaneous papers. The writings of Amesbury and of Lonsdale relate only to fractures. Even the justly celebrated quarto of Sir Astley Cooper is no more than what its title plainly declares it to be, A Treatise on Dislocations and on Fractures of the Joints; but since the announcement of the present volume, a trans lation of Malgaigne's great and crowning work on Fractures and Dislocations has been commenced by Dr. Packard, of Philadelphia, and the first volume has been placed in the hands of the American profession. Should the remaining volume be rendered into English, the gap in our literature will be measurably filled. Under these circumstances I might scarcely have thought it worth while to continue my labors, already so near their completion, had it not seemed to me that Malgaigne, whose researches have been truly marvellous, had failed in some measure to give a just representation of the observations and improvements which have been made from time to time by my own countrymen. Book excerpt: Excerpt from On Fractures and Dislocations According to the degree of separation of the fragments at the point of fracture, we speak of a complete and of an incomplete form. To the latter belong the fissure fracture, which traverses the bone without altering its external form, and the greenstick fracture, which is most commonly seen in the bent tibite of rickety children, although it is also observed in other long bones of young adults, and in some of the flat ones. In complete fractures the line of cleavage may have a varying direction, and we distinguish between transverse, oblique, longitudinal, and spiral fractures if small fragments of bone are completely detached at the point of fracture, with or without their periosteal covering, we speak of a splintered fracture. We may have also a multiple fracture of one and the same bone (at its upper and lower ends, and its middle), and finally simultaneous fractures of several bones (for instance, of the parallel bones of forearm and leg), or of bones widely separated from each other. It is of some interest to decide whether a fracture has occurred from direct or from indirect violence, - for instance, if the ulna is broken in parrying a severe blow we have a direct fracture; if a child falls on its hand and sustains a fracture of the clavicle or the lower end of the humerus the fracture is indirect. Since in the former contusion and haemorrhage are present at the site of fracture, so the direct form, as a rule, must be held to be a more severe lesion than the indirect. Much of the clinical and all of the statistical material has been obtained at the Cook County Hospital, Chicago. Although the author has personally performed the labor of writing the text and gathering the illustrations, for all of which he alone is responsible, he has been fortunate in receiving the helpful cooperation of his colleagues in the hospitals with which he is connected and of members of the Chicago Surgical Societyfi He wishes here to express his appreciation of their kindness if at'any place in the text he has failed to make more specific acknowledgment. It has come to be appreciated in all branches of medical teaching and research that a basic knowledge of pathology is of prime impor tance, so it is believed that clear conception of osseous injuries and their repair is essential to an understanding of fractures. For this reason the author has selected examples of different types of usual fracture pathology and endeavored to bring them before the reader's eye by means of line drawings which illus trate the essential points. Every illustration of this character is a careful reproduction of a tracing made from a Roentgenogmm of an actual case. The Artist, Mr. Streedain, and the author have worked for nearly two years preparing these drawings, checking and rechecking each one' until it conveyed the author's conception of the lesion. No one can read all roentgenograms with positive assurance. The author's obligations to Dr. Blaine, Roentgenologist to the Cook County Hos pital, for his friendly cooperation in this part of the work are gratefully recorded. The Artist has attempted to differentiate the shadows of bones more distant from the observer by lighter lines, and where two shadows overlap has indicated the one more remote by dotted lines. Cutaneous landmarks, lines of incisions with skin clips and plaster dressings have been occasionally indicated for obvious reasons. Each tracing represents the combined interpretation of the plate made by skilled observers who were in every instance familiar with the clinical aspects of the case. The writings of many who have contributed their experience to the literature of fractures have been consulted. Those to whom I feel indebted for suggestions are mentioned in the section on Bibliography. References to literature are not made in the text.
