Book excerpt: For more than 100 years, congenital dislocation of the hip has been an area of concern in orthopedics. This publications on the subject are al most too numerous to count. Yet our knowledge of the basic principles of congenital hip dislocation and its management is constantly being expanded by new research. In Germanspeaking countries, Kaiser pub lished the last comprehensive textbook on congenital hip dislocation in 1958, and Schlegel followed with a comprehensive handbook in 1961. In the Angloamerican world, Coleman's monograph was pub lished in 1978, Somerville's in 1982, and Wilkinson's in 1985. In 1982 Tachdjian compiled a volume on congenital hip dislocation that con tained contributions from 44 authorities. The purpose of the present book is to provide an overview of our pre sent state of knowledge of congenital hip dislocation, covering basic principles, diagnosis, methods of closed and open treatment, and indi cations. In the process, an attempt is made to trace progress in the field from its beginnings to the present time. Many authors describe the diagnosis and treatment of congenital hip dysplasia and dislocation in terms of specific age groups. We believe it is more prudent to take an individualized approach based on arthro graphic findings and the degree of severity of pathologic changes. Less emphasis is placed here on the management of patients by age group.

Book excerpt: In the 1950s the direct observation of dislocations became possible, stimulat ing the interest of many research workers in the dynamics of dislocations. This led to major contributions to the understanding of the plasticity of various crys talline materials. During this time the study of metals and alloys of fcc and hcp structures developed remarkably. In particular, the discovery of the so-called in ertial effect caused by the electron and phonon frictional forces greatly influenced the quantitative understanding of the strength of these metallic materials. Statis tical studies of dislocations moving through random arrays of point obstacles played an important role in the above advances. These topics are described in Chaps. 2-4. Metals and alloys with bcc structure have large Peierls forces compared to those with fcc structure. The reasons for the delay in studying substances with bcc structure were mostly difficulties connected with the purification techniques and with microscopic studies of the dislocation core. In the 1970s, these difficulties were largely overcome by developments in experimental techniques and computer physics. Studies of dislocations in ionic and covalent bonding materials with large Peierls forces provided infonnation about the core structures of dislocations and their electronic interactions with charged particles. These are the main subjects in Chaps. 5-7.

In spite of many articles in scientific journals, an outstanding monograph, and several excellent textbook chapters, the patella is still an enigma in many respects. The etiology of patellar pain is controversial, and there is no completely satisfying explanation for its cause or its relationship to chondromalacia. Curiously, neither the widespread use of arthroscopy nor the advent of newer diagnostic tests such as CT scanning and magnetic resonance imaging have cast much light. Without a better understanding of why patellar disorders occur it is not surprising that there is no consensus on how to fix them. Arthros copy has contributed little except to the patient's psyche. The currently most popular surgical treatment for recurrent dislocation of the patella was first described 50 years ago. One concrete advance, albeit a small one, is a better understanding of the role of anatomical abnormalities and patellofemoral dysplasia in patellar instabilities. The techniques to be developed are particularly appropriate when cracks are relatively short, and are growing in the neighbourhood of some stress raising feature, causing a relatively steep stress gradient. It is therefore practicable to represent the geometry in an idealised way, so that a precise solution may be obtained. This contrasts with, say, the finite element method in which the geometry is modelled exactly, but the subsequent solution is approximate, and computationally more taxing. The family of techniques presented in this book, based loosely on the pioneering work of Eshelby in the late 1950's, and developed by Erdogan, Keer, Mura and many others cited in the text, present an attractive alternative. The basic idea is to use the superposition of the stress field present in the unfiawed body, together with an unknown distribution of 'strain nuclei' (in this book, the strain nucleus employed is the dislocation), chosen so that the crack faces become traction-free. The solution used for the stress field for the nucleus is chosen so that other boundary conditions are satisfied. The technique is therefore efficient, and may be used to model the evolution of a developing crack in two or three dimensions. Solution techniques are described in some detail, and the book should be readily accessible to most engineers, whilst preserving the rigour demanded by the researcher who wishes to develop the method itself.
