Book excerpt: Excerpt from Diseases of the Stomach and Intestines In view of the excellent works on diseases of the stomach and intestines that have been placed before the medical profession, the publication of a new book on these subjects might almost seem to be superfluous. From a great accumulation of material, it is often difficult for the general practitioner to select simple and practical methods, and it is the endeavor that this volume should render service in this special direction. Many physicians have neither time nor opportunity to devote to a practical clinical course, and next in value to this for the purpose of instruction is the employment of photography to demonstrate the methods of diagnosis and treatment. Of this I have endeavored to take advantage. Many of the illustrations are from photographs of patients at the Manhattan State Hospital, taken by Mr. Hill, the official photographer, for whose services I am indebted to the courtesy of Dr. Win. Mabon, the Medical Superintendent. Some of the illustrations have been made by my artist from models. As visceral displacements have recently assumed an important position, their symptoms, diagnosis, and treatment, notably by mechanical methods, are specially described. Typhoid fever is included in this volume on account of its intestinal complications and for the purpose of differential diagnosis. A chapter is devoted to Diverticulitis, which has become an important subject. The endeavor has been made to indicate as clearly as possible the conditions which call for surgical procedure. Book excerpt: Excerpt from Diseases of the Intestines and Peritoneum Definition. - Enteralgia is a painful affection of the intestines, of neuralgic character, generally accompanied with constipation and flatus. It may come on gradually in a dull and obtuse manner, but in the great majority of instances its supervention is sudden, and the pain is sharp and violent. It is, correctly speaking, visceral neuralgia, and mostly occurs in neurotic individuals. The common accompaniments of inflammation are absent. The skin is cool, the pulse is not accelerated, and the heart's impulse is rather subdued than augmented. Its attacks are paroxysmal. It shifts its position in the abdomen. It is often a pain reflected by distal disease, but if continuous it may end in inflammation. Synonyms. - Enteralgia, Tormina, Dolor, Colicus, Colicodynia, Spasmus Intestinorum (various authors), Ileus (Sauvages), Spasmus Ventriculi (Wiessner). Some writers have confounded it with Gastrodynia, or Gastralgia. In the vernacular the affection is identical with Pain of the Intestines, Spasm of the Bowels and Belly-ache, Pain in the Belly, Gripes, and Cholick, or Cholick Colic. Causes. - The causes of this complaint are to be regarded as those which are Predisposing or Remote, and those which are Proximate or Exciting. Under the head of the first-named may be mentioned the influence of sex, and it is beyond dispute that females are more prone to this affection than males; their greater sensitiveness, and their susceptibility to moral emotions, favor the development of nervous diseases; and the sympathy of the uterus and its appendages, as familiarly known, in marked manner reacts upon the cerebro-spinal and ganglionic systems. The particular temperament of the patient will confer a proneness to, or tend to give an immunity from, this complaint; those who are nervous and melancholic being more liable to it, and those who are leuco-phlegmatic or lymphatic being less susceptible. The condition of asthenia conduces to the production of enteralgia, and a lowered vitalism is often associated with an exaltation of sensibility. Book excerpt: Excerpt from The Stomach and Abdomen: From the Physician's Standpoint In submitting this work to the medical profession some words of explanation seem to be necessary. The book is offered as a contribution towards the elucidation of the disorders and diseases of the stomach and other abdominal viscera as these have to be dealt with by the family practitioner, and by the hospital and consulting physician. The writer was interested in these matters before the advent of abdominal surgery, but the opening of that epoch gave a motive and an impetus to abdominal diagnosis which had previously been lacking. That diagnosis could be tested on the operating table, instead of the post-mortem-room table only, gave a significance to abdominal diagnosis which had been largely wanting. As a teacher in a great medical school, and as a physician to a great general hospital, where every type of disorder and disease was admitted into the wards, it was a duty and a privilege, not only to treat sick people, but to expound to students the methods and the steps by which diagnosis was reached, and on which treatment was based. The advent of abdominal surgery opened up a new field for investigation, and for teaching internal diagnosis, of which the writer took as full advantage as circumstances allowed. As time passed, and experience was tested and re-tested, the problems became simpler and the points essential to correct diagnosis clearer. This led to simplification in teaching, and this book represents more or less fully the position reached. There is little in it which has not been taught and re-taught by the writer as abdominal cases were admitted to his wards. From that time on the science of gastric diseases remained in a state of quiescence until 1867, when Kussmaul methodically applied the stomach pump in the treatment of dilatation of the stomach. The real progress, however, began a few years later, when Leube made use of the stomach pump for diagnostic purposes. Ewald, Boas, Beichmann, Kiegel, and others then instituted extensive studies of the gastric functions in health and disease. This second epoch in the study of digestive diseases, which had its inception in Germany, soon made itself felt in other countries, notably France, Russia, Austria, England, and America. In our counr try especially it seems that a very fruitful activity is developing in this direction.
