Book excerpt: Introduction: The majority of therapeutic research in multiple sclerosis (MS) has focused on the development of new treatments and diagnostic techniques with little investigation regarding the possibility of discontinuation of disease modifying therapies (DMTs). Objective: This study aims to understand both the general opinion of patients concerning stopping DMTs and the factors that influence the decision making process. Methods: A mixed-method approach was employed. Three focus groups were conducted, providing patient perspectives. Open coding of the focus group transcripts identified key themes that were used to develop a survey. This survey was administered to 1,000 North American Research Committee on Multiple Sclerosis (NARCOMS) participants who met inclusion criteria (=45 years; on most recent DMT for =5) according to their last annual survey. Descriptive analysis of patient's perspectives on stopping DMT and which factors influenced the decision were analyzed. Subsequently, exploratory and confirmation factor analyses were conducted utilizing structural equation modeling. Results: Of 1,000 invited participants, 491 responded and met inclusion criteria, with 436 completed responses. However, only 377 had been on current medication >1 year and were included in this analysis. Participants were 80.1% female with median age 56.4 and 82.0 % with a relapsing remitting course. The current DMT distribution was 53.6% first-line injectable, 24.7% oral DMT, 12.8% infusions, and 2.7% other. Only 11.9% of patients reported that if their disease was considered stable they would be very likely/likely to come off medications. Key concepts of anxiety and health literacy did not impact the likelihood of coming off medication. A higher degree of internal locus of control significantly increased the likelihood of considering discontinuation of DMT, whereas a high level of external locus of control in powerful others such as physicians significantly decreased the likelihood of considering discontinuation. Conclusion: People with MS report being unlikely to consider stopping their MS therapy. Where patients place their locus of control for decision making has a significant impact on likelihood to consider discontinuing medications. As the results of more studies concerning DMT discontinuation are obtained, the dissemination of information by providers remains an important part of patient decision-making process.
