This trail-blazing work in the history of bioethics will be of enduring s- nificance. I am deeply in their debt for having shouldered this far from easy task. Bioethics is the product of very particular socio-historical developments. Most prominent among them have been (1) the secularization of the dominant culture of North America, Western Europe, and now Central and South America as well, (2) a deflation of the status and authority of physicians as moral authorities able to guide their own profession, and (3) the salience of a post-traditional animus that gives c- tral place to persons as isolated atomic sources of moral authority. Bioethics initially took shape in North America as a post-Christian, post-professional, post-traditional social movement. This bioethics sought to establish a moral discourse for the public forum, a moral practice able to give practical guidance in hospitals and other insti- tions, and a body of undergirding and justifying theoretical reflections.

Book excerpt: Dorothy Wertz and John Fletcher pioneered the first international study of ethical and social issues in genetics in 18 nations. This book reports and discusses their second and more representative study in 36 nations. The survey focused on actual situations that occur in the practice of medical genetics, presented as case vignettes that can also be used in teaching and policy discussion. Among the issues discussed are privacy, prenatal diagnosis, patient autonomy, directiveness in counseling, sex selection, forensic DNA banking, "genetic discrimination," and "eugenics". This is Dorothy Wertz's final book, as she died in April, 2003. It is a one of a kind cross-cultural study of complex ethical issues in the uses of genetic information. No one else has attempted to look at the international aspects of medical genetics on such a broad scale. The results provide a resource for discussion both within and among nations. Much bioethical and policy discussion now occurs in an information vacuum. The survey showed that what people would do, and their reasons for doing it, differed considerably from what ethicists think they "should" do. Many will be surprised at the results, especially in nations where bioethical discussion is just beginning. Genetics and Ethics in Global Perspective is of interest to medical geneticists, genetic counselors, social scientists and anthropologists who study cross-cultural issues, bioethicists and bioethics centers and health policy makers.

If one peruses the index of documents discussed in Volume Four, certain topics are more often the focus of legislation and official concern than others: withholding and withdrawing treatment, access to health care, consent to treatment and experimentation, and issues posed by HIV testing and AIDS. Such a common focus should not be exaggerated, for the discussion of topics is wide-ranging. But that commonality, when in evidence, is also not surprising. It suggests that key issues and concerns in bioethics may be widely shared among modern cultures and societies, for all the distinctiveness of a particular nation's or region's response to them. Issues of informed consent, after all, implicate more fundamental matters of respect for persons and the rights of individuals in the contexts of therapy and research. Issues of access to medical care concretize deeper questions about the nature and scope of a society's welfare obligations to its citizens.
