Book excerpt: Abstract: "Diabetes mellitus (diabetes), an endocrine disorder, is in epidemic proportions globally, threatening the well being of people affected and challenging healthcare systems. This debilitating chronic condition warrants overall adjustments to lifestyle and therapeutic interventions to self-manage the condition and this can be achieved successfully by empowering the patient. Patient empowerment has emerged as a new paradigm recently, and can help improve medical outcomes while lowering costs of treatment. This paradigm can lead to better self-management, resulting in better clinical outcomes, particularly in managing chronic diseases, such as diabetes. This thesis investigates how information and communication technologies can play an important role in empowering the patients to self manage the disease, and attempts to make two important contributions. The first contribution, involves the development of a novel multimodal diabetes model. The proposed diabetes model can allow better self-management of the disease and lead to therapy success, as it takes into account multiple determinants, including the constructs of self-efficacy and locus of control drawn from socio-cognitive theory, and factors corresponding to personal attributes, demographics, disease traits, socio-environmental context (family and culture), and health context. The second contribution involves the development of a smart Web 2.0 portal technology for implementing the proposed multimodal diabetes model. The implementation of the diabetes model with this portal technology allows an integrated holistic platform for self-managing the disease, with seamless integration, collaboration and communication features provided by the portal."

Book excerpt: An Interactive Health Technology for Type-2 Diabetes Self-maintenance James Milewski University of California, Irvine, 2012 Burgeoning accounts of chronic disease conditions and age-related illnesses are placing demands on healthcare systems worldwide. Interactive Health Technologies (IHTs) are being developed to meet the demands. This thesis makes three contributions to developing IHTs for chronic disease management, specifically the activities related to type-2 diabetes self-maintenance. First, we investigate the dominant conceptual model currently guiding the development of IHTs, identify the weakness of the model, and present a set of application requirements that may be used to strengthen IHT support for the activities related to chronic disease management. Second, in chapter 3 we review the applications intended to support chronic disease management. A comparison of the applications' features may be found in the IHT Features supplementary file. The majority of applications focus on collecting quantifiable data and on providing information and recommendations that are based on the protocols of healthcare professionals. The applications do not meet the requirements identified in chapter 1, thus indicating a need for further research and development of IHTs for chronic disease management. Third, chapter 4 describes our iterative design process for creating an IHT; we discuss the challenges that we faced while designing and prototyping the application and provide the community with a set of suggestions to help guide future applications. In the last chapter, we consider the challenges of designing for different user segments and the need to mitigate the work associated with chronic disease management.

A powerful combination of factors is driving this change. These include rapid advances in mobile technologies and applications, a rise in new opportunities for the integration of mobile health into existing eHealth services, and the continued growth in coverage of mobile cellular networks. According to the International Telecommunication Union (ITU), there are now over 5 billion wireless subscribers; over 70% of them reside in low- and middle-income countries. For the first time the World Health Organization's Global Observatory for eHealth (GOe) has sought to determine the status of mHealth in Member States; its 2009 global survey contained a section specifically devoted to mHealth. Completed by 114 Member States, the survey documented for analysis four aspects of mHealth: adoption of initiatives, types of initiatives, status of evaluation, and barriers to implementation. Fourteen categories of mHealth services were surveyed: health call centres, emergency toll-free telephone services, managing emergencies and disasters, mobile telemedicine, appointment reminders, community mobilization and health promotion, treatment compliance, mobile patient records, information access, patient monitoring, health surveys and data collection, surveillance, health awareness raising, and decision support systems.
