Book excerpt: While the global maternal mortality ratio has significantly declined since 1990, a large number of women still die in child birth, with most of these deaths occurring in sub-Saharan Africa and South Asia. Evidence suggests that use of maternal health care services during delivery can substantially avert maternal mortality. This paper uses India's 2005-06 National Family Health Survey (NFHS-3) household data to investigate the economic, demographic and socio-cultural factors that determine a woman's use of professional delivery care services in India. Recognizing evidence which suggests that men are the key decision makers in India, this paper tests the impact of a husband's involvement in a woman's pregnancy on her use of these services, as its main variable of interest. A husband's involvement is measured by his presence at a woman's antenatal visit to the health clinic. The multivariate regression analysis undertaken in this paper finds that a woman whose husband is present at her antenatal visit is 17.06 percentage points more likely to deliver in a health facility. Further, the analysis also finds factors such as a mother's education level, place of residence (urban/rural) and household wealth to be significant in determining a woman's use of professional delivery care services. These results are important for designing policies aimed at increasing the use of maternal health care services in India and highlight the importance of involving men (husbands) as an integral part of reproductive health care interventions.

Book excerpt: The evaluation of reproductive, maternal, newborn, and child health (RMNCH) by the Disease Control Priorities, Third Edition (DCP3) focuses on maternal conditions, childhood illness, and malnutrition. Specifically, the chapters address acute illness and undernutrition in children, principally under age 5. It also covers maternal mortality, morbidity, stillbirth, and influences to pregnancy and pre-pregnancy. Volume 3 focuses on developments since the publication of DCP2 and will also include the transition to older childhood, in particular, the overlap and commonality with the child development volume. The DCP3 evaluation of these conditions produced three key findings: 1. There is significant difficulty in measuring the burden of key conditions such as unintended pregnancy, unsafe abortion, nonsexually transmitted infections, infertility, and violence against women. 2. Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.

Book excerpt: Adolescent childbearing is a phenomenon that has significant ramification at personal, societal, and global levels. Present research attempts to explore whether the utilization of Reproductive and Child Health Care Services differs among adolescent and older mother after controlling the potential background socio-economic and demographic characteristics. The study utilizes data from three rounds of National Family Health Survey (NFHS) to understand the level, trend and determinants of teenage childbearing and utilization of basic Reproductive and Child Health Services (RCH) in India. The Reproductive and Child Health Services that are examined in present study includes antenatal care, safe delivery, and child immunization. Results show that prevalence of teenage childbearing in India has remained almost stagnant over the past 13 years. The analysis shows that after controlling the potential socioeconomic and demographic factors, maternal age appeared to have the significant influence on the use of maternal and child health care services. Adolescents are less likely to use any health care services than the older women.

However, the approach was, for a very long time, mainly geared towards achieving demographic targets. During the late 80s and early 90s, global focus shifted away from mere population control to one that emphasizes that individuals should have the ability to reproduce and regulate their fertility, and that women should be free from the fear of pregnancy and disease. India has since then adapted its own programs to reflect this change. With some of the highest maternal mortality, infant mortality, and under-five mortality rates in the world, India has a long way to go. While socio-economic factors have always been offered as the primary reason for low levels of utilization of reproductive and child health services (RCH) in India, this paper seeks to make an important contribution by highlighting that much of the problem is hidden in the quantity and quality of these services, and attempts to identify key inputs on the delivery side that could lead to significant improvement in India's RCH indicators.

The chapters of this book are written with extraordinary intelligence and und- standing, and with attention to practical considerations in the selection and mana- ment of contraceptive options. The authors have reviewed the science behind contraception, including the chemical structure and effects of hormonal contraception, physiology of contraception, efficacy rates, and side effects, as well as the practical considerations that are relevant in helping patients choose between different cont- ceptive options. They do this with a clarity of language and intent that lets the book cover with sufficient detail the full range of questions that any primary care clinician will have regarding any of the traditional or new contraceptive options. Also included in each chapter is a section on “counseling tips,” which explicitly answers many of the questions that clinicians and their patients often have when discussing contraceptive options. For a book so useful and well done, the editors and authors deserve our thanks.
