However, conventional techniques used to produce sockets with suitable characteristics are labor intensive and expensive, and depend on the work of skilled prosthetists that are relatively scarce compared to the number of transtibial amputees. Selective Laser Sintering (SLS) is a very promising technique for producing subject-specific sockets for transtibial amputee prostheses due to its inherent ability to create complex three-dimensional objects directly from digital shape information without the need for specific tooling, molds or human labor. This dissertation presents a framework for the design, analysis, manufacture and testing of SLS sockets for transtibial prostheses, including the development of a computer-aided design model of the socket with compliant features to enhance comfort, structural analysis using a Finite Element Method (FEM) model, fabrication of a functional prototype using SLS, and experimental validation of the FEM analysis. The validation involved quantifying the failing conditions of sockets manufactured using the framework during destructive tests. The experimental failure loads for the sockets were within a 3% range of the FEM results and were considered satisfactory. The specific design of orthogonally compliant features for socket was also analyzed. This process included the preliminary evaluation of design alternatives using FEM with validation through experimental measurements, definition of specific design methodology for the best alternatives, incorporation of solution within the socket and refinement of design using auxiliary features obtained through topology optimization. Finally, to investigate the structural response of the SLS socket during gait, a FEM model (acquired from Computed Tomography data of a transtibial amputee) composed of a socket, liner and residual limb under quasi-static loading derived from typical ground reaction forces was employed. Three different compliant designs were evaluated to assess their ability to locally relieve local pressure during the stance phase of gait, as well as their structural integrity to ensure safety. The design with a compliant feature consisting of spiral slots within the socket wall was determined to produce a local average relief of 65.8% in the interface pressure, reducing the peak pressure from 172 kPa to 66.4 kPa.

Book excerpt: A prosthetic socket worn by an amputee must serve a wide variety of functions, from stationary support to the transfer of forces necessary to move. Fit and comfort are important factors in determining the therapeutic effectiveness of a socket. A socket that does not fit the subject well will cause movement problems and potentially long-term health issues. Because a subject's residual limb changes volume throughout the day, it is desirable that the socket adapt to accommodate volume changes to maintain fit and comfort. This thesis presents research to manufacture adaptive sockets using selective laser sintering (SLS). This additive manufacturing process allows freedom to design a socket that has both compliant areas that can adapt to changes to the residual limb, as well as rigid regions to provide necessary support for the limb. A variety of concepts are discussed that are intended for manufacture by SLS, and that feature flexible inner membranes in various configurations. For each concept the membrane will be inflated or deflated to match the limb's change in volume and the thesis also presents a study to determine SLS machine parameters for optimal build results. A series of experiments was created to understand the ability of SLS manufactured plastics to be inflated and the possible performance.

Socket stress distribution is performed on three types of socket, polypropylene (5mm), polypropylene (3mm) and a standard laminate (3mm) sockets to determine the stresses path through the prosthetic socket during the gait cycle. It is found that heel strike phase is the critical phase. The stresses increase at the socket base while the maximum deflection is maximum at the patellar region of the socket. This work is achieved by finite element program, ANSYS. The development of design specifications of a below the knee prosthetic socket is presented in this study for successful ambulation and comfort. Seven different design models are created by changing the socket wall thickness and the material. Polypropylene socket gives the best results with regard to the allowable deflection for the regions of the pressure relief areas of the stump. The effect of varying the modulus of elasticity on the polypropylene socket is considered in this study. Three models are analyzed with different tensile creep moduli during different periods. It is found that the polypropylene socket will enlarge in size with time. An experimental study is conducted to compare the strength of five prosthetic sockets made of different materials. Compression, three-point flexural and tensile tests are implemented by the Testometric machine. The laminate sockets have larger compressive stiffness than polypropylene while polypropylene has larger flexural stiffness than the tested laminates except for socket No.5.

Book excerpt: A lower limb prosthesis provides assistance to its user in both ambulation and stationary support. The lower limb prosthesis consists of a socket, which interfaces with the residual limb, a pylon, attachment hardware to secure the pylon to the socket, and a prosthetic foot. For the prosthesis to be effective, the socket must be comfortable, functional and aesthetically appealing, usually in that order. Lack of comfort and fit can cause movement problems and health issues. The residual limb of the amputee changes its volume throughout the day and in order to maintain comfort a socket must be able to adapt to these volume changes. Previous research has resulted in the development of concepts for inflatable prosthetic sockets capable of addressing this need. The concepts rely on laser sintering (LS) to manufacture the parts. This research focuses on the development of a finite element analysis (FEA) method to assist in the design of adaptive sockets. The FEA can be used to predict the pressure-deflection curve of a given socket design. The FEA method was verified by experiments using LS manufactured test specimens.
