Book excerpt: "Background: Oral health care is amongst one of the highest unmet needs of people living with HIV/AIDS (PLHIV); this may be due to barriers they face accessing care, such as stigmatization and fear of discrimination. PLHIV have indeed reported negative experiences at dental offices and with dental staff. However, there is a lack of recent and in-depth studies that capture the current perspectives of PLHIV regarding accessing dental services. In order to respond to the oral health needs of PLHIV, it is thus important to better understand how they access and experience dental care in Canada. Objectives: Our objective was to better understand the lived experiences of PLHIV with respect to accessing dental care. In particular, we sought to better understand the difficulties and the stigmatization they faced trying to fulfil these needs, and finally to make recommendations for alleviating these difficulties. Methods: We adopted a participatory approach and an interpretive phenomenological research design. We invited community associations for PLHIV and organizations fighting HIV to collaborate with us at all stages of the research. In order to gain an in-depth understanding of the perspectives and experiences of PLHIV, we used a qualitative approach, namely interpretive phenomenology, which is particularly appropriate for understanding and describing complex and sensitive experiences. We conducted in-depth interviews with eight people living with HIV in Montreal. The interviews were audio-recorded, transcribed verbatim, and interpretively analysed.Findings: Living with HIV significantly shapes the experiences of people regarding their oral health and accessing dental care. Our participants struggled with an imperfect oral health and a limited access to dental care. Thus they experienced anxiety over managing a fragile oral health for years to come. Although they were generally satisfied with their dentists, they reported isolated negative encounters with the dental staff. Because of these negative experiences, in addition to that of other PLHIV, some participants anticipated being stigmatized in dental settings. To avoid such potential discrimination, they either chose not to disclose their HIV status to the dentists, or to visit a trusted dentist known for accepting PLHIV.Conclusion: Dental professionals should be aware of and sensitive to the complexities of PLHIV's life experiences and try to accommodate their specific needs. Dentists alongside other members of the society should also tackle HIV stigma in dental settings and society at large." --

Book excerpt: HIV/AIDS is a major problem in South Africa with more than 25 percent of the adult population infected with HIV. Oral lesions and various opportunistic infections characterize the progression of HIV making it imperative for people living with HIV to have access to good quality oral care. There is a need to examine accessibility and use of dental services in South Africans living with HIV as very little research in this regard, has been undertaken. Aim of the research is to investigate the barriers to oral health care for people living with HIV in the KZN and the WC. A cross-sectional study utilising a self-administered questionnaire and semi-structured interviews has been employed. Participants were people living with HIV older than 18 years attending HIV clinics located throughout Kwa-Zulu Natal and WC. Binary logistic regression was performed to determine the variables associated with not obtaining care. The sample (n=435) comprised mainly of black females in the age group 20-29 years. There were statistically significant differences in terms of socio-demographic factors between the two provincial cohorts. Of the total number of participants, 347 (79.8%) had an oral health problem and 83% (n=288) received care. Of those that received care, more than half of the participants (57.6%) reported that the staff were aware of their HIV status. Almost a third of the participants who received care reported a negative experience. Of those that did not receive care, there were disparate reasons between the two provincial cohorts. In KZN, the reasons for not receiving care related to the service being unavailable, too far away or lack of money to pay for transport and for the service, while in WC, participants had no money to pay for transport and for the service, but were mostly afraid of discrimination, stigmatisation and lack of confidentiality.

Book excerpt: HIV is one of the ten significant public health issues in the United States, with more than 1 million Americans living with HIV. People living with HIV (PLWH) are expected to go through the HIV care continuum to manage their HIV. HIV social workers are the primary workforce to deliver HIV care continuum services. HIV care continuum research has three limitations. First, it overwhelmingly collects data on PLWH and HIV social workers who live in metropolitan cities rather than rural areas. Second, it examines one step in the continuum process rather than all the steps. Third, it collects data on either PLWH or HIV social workers rather than comparing these groups' experiences. Given these gaps and in collaboration with an AIDS service organization community partner, the purpose of this dissertation was to explore the lived experiences of the rural HIV care continuum among gay and bisexual men (GBM) living with HIV and HIV social workers who live in South Central and Southern Indiana. Data presented in this dissertation reflects three manuscripts. Manuscript I is an interpretative phenomenological analysis (IPA) study of 15 rural HIV service providers' lived experiences working in the rural HIV care continuum. Manuscript II is an IPA study of the lived experiences of going through the rural HIV care continuum among 15 rural GBM living with HIV. Manuscript III is a multi-perspectival IPA study comparing the rural HIV care continuum experiences of HIV service providers and GBM living with HIV. Manuscripts provide future directions in research and programming for rural HIV care continuum.

Sixty-six HIV-infected adults were interviewed to assess determinants of dental service use. Andersen's Behavioral Model provided the theoretical framework with the primary outcome measured as subjects' self-report of a dental visit within the previous year. Each model domain independently contributed a significant predictor; white race, regular dental visits since seroconversion, more positive oral health attitudes and higher oral health quality of life were significantly correlated with a dental visit within the previous year. Subjects reported significantly fewer restorations and more periodontal treatments after seroconversion. While no factors retained significance in multivariate modeling, results suggest that there may be utility in using Andersen's model to predict use of dental services. Future research should employ a qualitative or mixed-methods approach due to complexities in sampling and characterizing behavior in HIV-infected adult urban cohorts.

Book excerpt: This book consists of four interlinked studies that examine both costs and consequences of a dedicated oral and dental programme for HIV-infected patients at both of the hospital outpatient clinics, Kalafong and George Mukhari Hospitals, and the orphanages. The programme was compared to the status quo for patients who are dependent on the public sector for oral and dental care. The viewpoints of both people living with HIV/AIDS and of Gauteng Health Department were adopted. Beneficiaries' demographic characteristics; types of oral mucosal lesions; dental caries status and trends are described. An appropriate selection of costs were identified, measured and valued. These included: costs of organizing and operating the programme; costs of seeking care, and cost per patient and mucosal lesion/carious tooth appropriately treated. In total, 1027 patients accepted the programme between 2005 and 2006. Their ages ranged between 1 and 70 years. Antiretroviral use was high at 69% in adults and 60% in children. Candidiasis was the most common oral lesion recorded in both adults (81.1%) and children (61.5%). Caries prevalence was significantly higher (p = 0.002) in children receiving HAART.

Book excerpt: Objectives: This report evaluates current organizational and care coordination practices within a sample of community health centers in Philadelphia in order to determine methods that increase service utilization and to provide recommendations that can be used to enhance dental case management and care coordination procedures. Methods: Key informant interviews were conducted with service providers from community health centers that are members of the Health Federation of Philadelphia's CareLink network. CareLink's continuous quality improvement (CQI) data was analyzed to determine demographic information and oral health performance measures. Results: Across the four sites being studied, the rates of clients who self-reported an oral exam by a dentist during the measurement year ranged from 5 to 39 percent. All but one site offers onsite dental care, and though each site offers HIV case management, care coordination, and outreach, the implementation approach varied at each site. Providers list cost and fear as the primary reasons for low dental care utilization and many believe that increased dental care coordination may improve their ability to raise service utilization rates. The only site meeting national average for oral health performance measures is the only site that provides coordination specifically around dental care. Conclusions: Onsite dental facilities appear to be related to increased rates of dental care utilization. Sites with designated dental care coordination report higher rates of self reported oral exams by a dentist. It appears that organizational culture and service integration play a significant role in the ability to engage clients in dental care.
