Book excerpt: Civil Society Organizations (CSOs) can make a vital contribution to public health and health systems but harnessing their potential is complex in a Europe where government-CSO relations vary so profoundly. This study is intended to outline some of the challenges and assist policy-makers in furthering their understanding of the part CSOs can play in tandem and alongside government. To this end it analyses existing evidence and draws on a set of seven thematic chapters and six mini case studies. They examine experiences from Austria Bosnia-Herzegovina Belgium Cyprus Finland Germany Malta the Netherlands Poland the Russian Federation Slovenia Turkey and the European Union and make use of a single assessment framework to understand the diverse contexts in which CSOs operate. The evidence shows that CSOs are ubiquitous varied and beneficial and the topics covered in this study reflect such diversity of aims and means: anti-tobacco advocacy food banks refugee health HIV/AIDS prevention and cure and social partnership. CSOs make a substantial contribution to public health and health systems with regards to policy development service delivery and governance. This includes evidence provision advocacy mobilization consensus building provision of medical services and of services related to the social determinants of health standard setting self-regulation and fostering social partnership. However in order to engage successfully with CSOs governments do need to make use of adequate tools and create contexts conducive to collaboration. To guide policy-makers working with CSOs through such complications and help avoid some potential pitfalls the book outlines a practical framework for such collaboration. This suggests identifying key CSOs in a given area; clarifying why there should be engagement with civil society; being realistic as to what CSOs can or will achieve; and an understanding of how CSOs can be helped to deliver.

Book excerpt: This book examines the relationship between civil society and human security in the Indian state of Meghalaya, part of the region known as Northeast India. Civil society has been revived over the last two decades and is now one of the key concepts in development, politics, and international aid. The concept has gained particular significance as part of attempts to analyse and instigate grassroots democratisation through widespread political participation. This is seen as enabling a broader range of issues to be politicised and made a part of political agendas at the local, national, and global levels. However there are few studies that examine the constraints on civil society at the local level, even in contexts where civil society may appear to be active and vibrant. Those studies that do exist tend to focus on the constraints coming from the state, overlooking the constraints that come from within civil society itself. During the same period human security has gained prominence as a challenge to state-centric conceptions of security and as an alternative approach to development by focusing on the security and insecurity of groups and individuals. The concept has been taken up by international organisations, development agencies, and bilateral donors as a more effective way to understand the difficulties people face in their everyday lives that go beyond conventional categories and indicators. In order for those experiencing insecurity to identify and contest the causes of insecurity, participation in civil society is necessary.

Book excerpt: This dissertation deals with community participation in health. The main contention of this work is that community participation has been inadequately conceived in health and considered merely as a strategy for health interventions that gives voice to communities. Civic participation, however, is not just a strategy. It is a political project dealing with relations between state and society or governance-related issues. The dissertation consists of three papers addressing this topic from different aspects. The first paper explores literature that situates the discourse on community participation in health in the wider socio-political context. It describes the main models of democracy and explores the theoretical underpinnings of the predominant model of democracy that is representative democracy. Theoretically speaking, this model is elitist and does not allow for mass participation in political decision making. To redress this problem theoretically an alternative 'public sphere' model and operationally a reform strategy are described. This is followed by a description of the challenges of participation. These challenges highlight that even if the idea of citizen participation is conceived adequately, state-civil society relations are such that they present certain challenges. The second paper picks up from this. It describes an empirical study about the recent health reform in Venezuela known as Barrio Adentro which is known for its high level of popular participation. The study explores the challenges of participation from community's perspective in a health reform known for its high civic participation. As indicated in the first paper, initiatives promoting civic participation, whether in health or any other sector, are situated in the wider socio-political context. In fact, it is the context that not only determines the nature of such participatory initiatives but also determines their trajectory. The third paper then focuses on the socio-political context. The paper involves four countries namely India, South Africa, Brazil, and Venezuela and uses comparative politics and health literature on these countries to do a comparative analysis using two contemporary frameworks. The use of frameworks makes the analysis structured and the findings robust. The paper concludes with a list of implications for policy and practice for the entire dissertation.
