Book excerpt: During the last meeting of the European Association of Neurosurgical Societies (Barcelona 7-16 September 1987), a Symposium was devoted to the use of calcium antagonists in cerebral vasospasm. As shown by its title "Prevention and treatment of delayed ischaemic dysfunction in patients with subarachnoid haemorrhage: an update", papers presented at the Symposium covered a broad spectrum from some basic data on pathophysiology of subarachnoid haemorrhage (SAH) and delayed ischaemic dysfunction, to the clinical use of nimodipine, which has been largely documented among calcium inhibitors for its cerebrovascular properties. This supplementum of Acta Neurochirurgica is based upon the papers presented. Some articles, however, have been extended to take into account further results in order to present a broad view of the subject. The Symposium started with two presentations concerning general aspects of SAH: in the first one, B. Voldby reviewed the pathophysiological events following SAH: if modifications of intracranial pressure (ICP) and decrease of cerebral blood flow (CBF) are the primary alterations, metabolic changes (particularly reduction in CMR0 ) 2 contribute rapidly to disturbances of brain circulation. A variety of mechanisms may be responsible for the neurological dysfunction: most of them are directly related to the reduction in cerebral perfusion pressure (CPP) which may result in ischaemia. This fall in CPP (either due to elevation of ICP or reduction of mean arterial blood pressure) is observed under different pathophysiological conditions, discussed in the paper by A. D. Mendelow.

Both structurally and functionally, there are many peculiarities that differentiate the brain from all other organs. The brain is our connection to the world around us and by governing nervous system and higher function, any disturbance induces severe neurological and psychiatric disorders that can have a devastating effect on quality of life. Our understanding of the physiology and biochemistry of the brain has improved dramatically in the last two decades. In particular, the critical role of cations, including magnesium, has become evident, even if incompletely understood at a mechanistic level. The exact role and regulation of magnesium, in particular, remains elusive, largely because intracellular levels are so difficult to routinely quantify. Nonetheless, the importance of magnesium to normal central nervous system activity is self-evident given the complicated homeostatic mechanisms that maintain the concentration of this cation within strict limits essential for normal physiology and metabolism. There is also considerable accumulating evidence to suggest alterations to some brain functions in both normal and pathological conditions may be linked to alterations in local magnesium concentration. This book, containing chapters written by some of the foremost experts in the field of magnesium research, brings together the latest in experimental and clinical magnesium research as it relates to the central nervous system. One of the most common complication after aSAH is the delayed cerebral ischemia (DCI), defined as focal or global neurological deterioration, cerebral infarction or both. The cerebral vasospasm has been suggested for a long time as the main reason of DCI, however the contribution of vessel narrowing in DCI remains unclear [1]. In order to study the pathogenesis behind DCI and determine the factors, which lead to cerebral ischemia, we used a multimodal continuous monitoring of multiple variables. METHODSA total of 40 patients with a mean age of 57u00b115 years with diagnosed moderate aSAH (Hu2013H grade 3u00b11) were prospectively enrolled in the study. Continuous recording of the arterial blood pressure (ABP), intracranial pressure (ICP), endu2013tidialCO2 (EtCO2), regional cerebral oxygenation (rSO2), cardiac output (CO), stroke volume variation (SVV) and cerebral blood flow velocity (CBFV) were performed using computeru2013based system Intensive Care Monitor (ICM+), Cambridge Enterprise. Cerebral autoregulation was assessed applying the pressure reactivity index (PRx), the tissue oxygenation index (TOx) and the mean velocity index (Mx).RESULTSIn analysed group of patients after aSAH 30% have DCI and 42% of them have also a vasospasm, which occurs in 5u00b12 day after onset. 75% of patients with DCI has cognitive or functional disorders. The systemic cardiovascular disorders (significant reduction in CO and ABP) during acute phase have been observed. The CO and SVV were stabilized due to apply a monitored therapy. The PRx on 3th day was significantly higher than on 6th day after onset (0.128 vs. -0.054, p=0.047). DISCUSSIONThe monitoredu2013guided agents therapy, combines with a multimodality monitoring of cerebral autoregulation, autonomic nervous system and cardiovascular system, allows to personalized treatment and better outcome in aSAH patients. One-third of survivors are dependent for care and almost half will have cognitive impairment sufficient to affect their quality of life.A multidisciplinary team is involved in patient care and Anaesthetists are involved from the initial stabilization and diagnosis through to providing anaesthesia for definitive treatment and post operative support in critical care. Materials and methods: We conducted a retrospective audit of paper and electronic records looking at 21 patients who underwent coiling for SAH. The data was collected against the guidelines produced by AHA/ASA 2012 directed towards prevention and treatment of re-bleeding, vasospasm, delayed cerebral ischaemia, hydrocephalus and seizures, to assess current practice at University Hospital North Midlands, UK.Results and discussion: 21 patients (19 females and 2 male), age range 25 to 67 years old, mean 54.5 +/- 11.3. One third of aneurysms were multiple and two thirds were single. 15 aneurysms were less than 7mm in size. 52% of patients were transferred from another hospital.75% patients were treated within 24hrs 90% of patients treated within 48 hours. Mean time to treatment was 19 hours. 100% had nimodipine prescribed and 100% received nimodipine 38% of patients missed between 2 and 4 doses of nimodipine 33% of patients received 1st nimodipine dose in hospital >4 hours post admission 1/21 patients received tranexamic acid. 50% of patients had a 0 or 1 Modified ranking Scale score at 6 months follow up.Retrospective collection of data is flawed, but future audit will be improved following introduction of prospectively compiled database.Pre-operatively patients were managed appropriately as evidenced from good outcomes at 6 months. Improvements could be made in administration of prescribed drugs, in particularly nimodipine for which there is strong evidence for prevention of vasospasm.Optimal fluid balance and closer blood pressure control can also be improved to avoid secondary brain injury.Conclusion(s): Good practice was observed at UHNM complying with the recommendations of the AHA/ASA for definitive treatment of aneurysm within 72hrs. We are introducing a check-list style proforma with key interventions to be signed and dated in front of patient notes to improve and standardise care.
