Book excerpt: At first sight it may appear strange that a volume of Progress in Pediatric Surgery should be devoted to the history of our specialty. One assumes that progress is concerned primarily with recent developments whilst history deals with matters of the past. However, in the past there has also been considerable progress in the development of our understand ing of paediatric surgical problems, otherwise we would not have pro gressed to our present achievements. The editors, therefore, do not apologize for compiling this volume but, on the contrary, feel that the publication of this volume is most timely. Modern paediatric surgery has now been practised for three genera tions. The handful of pioneers who were the founders of our specialty worked mainly before the last world war. A few dozen of the inter mediary generation started work immediately after the war, while the new generation who are now dominating our specialty must be counted in thousands. Two factors have radically altered paediatric surgery as practised by the intermediary and the present generation of surgeons.

Additional vagueness is introduced as models become more complex but not necessarily more meaningful by the added details. During the last two decades one has become more and more aware of the fact that not all this uncertainty is of stochastic (random) cha racter and that, therefore, it can not be modelled appropriately by probability theory. This becomes the more obvious the more we want to represent formally human knowledge. As far as uncertain data are concerned, we have neither instru ments nor reasoning at our disposal as well defined and unquestionable as those used in the probability theory. This almost infallible do main is the result of a tremendous work by the whole scientific world. But when measures are dubious, bad or no longer possible and when we really have to make use of the richness of human reasoning in its variety, then the theories dealing with the treatment of uncertainty, some quite new and other ones older, provide the required complement, and fill in the gap left in the field of knowledge representation. Nowadays, various theories are widely used: fuzzy sets, belief function, the convenient associations between probability and fuzzines~ etc ••• We are more and more in need of a wide range of instruments and theories to build models that are more and more adapted to the most complex systems.

The objective of the conference is not only to present the most innovative and promising research work but also to highlight research trends and to foster dialogues and debates among participants. This event was decided after a preliminary successful symposium organized in Stanford in March 1994 by E. Grimson (MIT), T. Kanade (CMU), R. Kikinis and W. Wells (Chair) (both at Harvard Medical School and Brigham and Women's Hospital), and myself (INRIA). We received 92 submitted full papers, and each one was evaluated by at least three members of the Program Committee, with the help of auxiliary reviewers. Based on these evaluations, a representative subset of the Program Committee met to select 19 long papers, 29 regular papers, and 27 posters. The geographical repartition of the contributions is the following: 24 from European countries (other than France), 23 contributions from France, 20 from Northern America (USA and Canada), and 8 from Asia (Japan and Singapore).

Book excerpt: Cancer is a common disease with a devastating impact on the physical and psychological well being of patients. The diagnosis of cancer brings upon many clinical challenges and questions for which clear and simple answers are not always provided by modern medicine. To date, only limited therapeutic options are available for patients with advanced cancer. The recent shift toward targeted therapies has improved substantially patient’s survival, however, relapses are frequent and cure remains rare. This led patients and many health care managers to shift attention to the holistic approach of traditional medicine particularly preparations from herbal products to manage and alleviate the disease. Typically, herbal preparations contain single or multiple plant ingredients, including a number of potential active components. Yet, they remain classified as food supplements and thus are exempt from regulations on quality control and proof of efficacy that govern standard pharmaceuticals. Clinical evidence for many preparations is often based on non-documented or anecdotal evidence. In consequence, several preparations with unproven efficacy are circulating in the market with the fear of interference with standard cancer therapies and/or severe toxicity that some can generate, in addition to the unjustified economical burden to patients. Despite inconsistent and conflicting clinical results single molecules have been isolated from herbal preparations and many are exploited to develop potential novel agents. This has fostered the need to organize a set of timely, in-depth and up-to-date review covering the latest developments in alternative cancer management from a scientific and clinical perspective dedicated to the medical community and health care providers, as well as to patients and their families. This book brings the latest comprehensive cancer information and practical recommendations on the best documented practice of alternative therapies for cancer management put together by recognized experts in the fields of medical oncology, traditional medicine, and cancer pharmacology.
