Book excerpt: Preface During the past two decades, a significant international research exchange has taken place in order to understand the pathobiology of corneal diseases, and to develop more sophisticated treatments. For this purpose, many Japanese cornea specialists have been abroad to the United States, the United Kingdom, etc., to perform collaborative works in basic and clinical cornea research. As a result, many original concepts and thoughts have been produced from Japan, and some of them are now accepted world wide. Further to promote the international exchange of current knowledge and research on corneas in both health and disease, the First Annual Meeting of the Kyoto Cornea Club was held in Kyoto City on December I and 2, 1995. The goal of this meeting was to provide each speaker's updated knowledge that would be of value to all active corneal researchers who are interested in 'state-of-the-art' information from Japan. To achieve this objective fully, 41 selected Japanese cornea specialists registered as active participants in this meeting. James D. Zieske, PhD, from Schepens Eye Research Institute and the Department of Ophthalmology at Harvard Medical School, Boston, MA, USA, and Tadatsugu Taniguchi, PhD, from the Department of Immunology, Faculty of Medicine, Tokyo University, Japan, participated as special guests at this meeting, and both gave plenary lectures. This volume was created to provide each speaker's current opinions on the subjects they presented at this meeting, and also to provide a scientific reference for cornea research. We hope that the first issue will serve as a useful source of current knowledge and insight on Japanese cornea research. Book excerpt: Our ability to treat common bacterial infections with antibiotics goes back only 65 years. However, the authors of this report make it clear that sustaining a supply of effective and affordable antibiotics cannot be without changes to the incentives facing patients, physicians, hospitals, insurers, and pharmaceutical manufacturers. In fact, increasing resistance to these drugs is already exacting a terrible price. Every day in the United States, approximately 172 men, women, and children die from infections caused by antibiotic-resistant bacteria in hospitals alone. Beyond those deaths, antibiotic resistance is costing billions of dollars through prolonged hospital stays and the need for doctors to resort to ever more costly drugs to use as substitute treatments. Extending the Cure presents the problem of antibiotic resistance as a conflict between individual decision makers and their short-term interest and the interest of society as a whole, in both present and future: The effort that doctors make to please each patient by prescribing a drug when it might not be properly indicated, poor monitoring of discharged patients to ensure that they do not transmit drug-resistant pathogens to other persons, excesses in the marketing of new antibiotics, and the broad overuse of antibiotics all contribute to the development and spread of antibiotic-resistant bacteria. The book explores a range of policy options that would encourage patients, health care providers, and managed care organizations to serve as more responsible stewards of existing antibiotics as well as proposals that would give pharmaceutical firms greater incentives to develop new antibiotics and avoid overselling. If the problem continues unaddressed, antibiotic resistance has the potential to derail the health care system and return us to a world where people of all ages routinely die from simple infections. As a basis for future research and a spur to a critically important dialogue, Extending the Cure is a fundamental first step in addressing this public health crisis. This effort has been motivated by the recognition that the tear film plays an essential role in maintaining corneal and conjunctival integrity, protecting against microbial challenge and preserving visual acuity. In addition, research has been stimulated by the knowledge that alteration or deficiency of the tear film, which occurs in countless individuals throughout the world, may lead to desiccation of the ocular surface, ulceration and perforation of the cornea, an increased incidence of infectious disease, and potentially, pronounced visual disability and blindness. 7 To promote further progress in this field of vision research, the International Conference on the Lacrimal Gland, Tear Film and Dry Eye Syndromes: Basic Science and Clinical Relevance was held in the Southampton Princess Resort in Bermuda from November 14 to 17, 1992. This meeting was designed to assess critically the current knowledge and 'state of the art' research on the structure and function of lacrimal tissue and tears in both health and disease. The goal of this conference was to provide an international exchange of information that would be of value to basic scientists involved in eye research, to physicians in the ophthalmological community, and to pharmaceutical companies with an interest in the treatment of lacrimal gland, tear film or ocular surface disorders (e. g. Sjogren's syndrome).
