Book excerpt: Introduction : la planification numérique pré-opératoire des prothèses totales de hanche (PTH) est l'outil qui permet d'anticiper les difficultés de pose des implants prothétiques et d'augmenter la survie des implants. L'objectif de ce travail était d'établir la courbe d'apprentissage nécessaire pour produire un calque pré opératoire fiable et reproductible pour les PTHs de première intention chez les chirurgiens juniors (CJ). L'hypothèse était qu'il existait une courbe d'apprentissage pour la réalisation d'un calque pré-opératoire en considérant que la compétence de planification est acquise quand le pourcentage d'erreur de planification du CJ atteins celui du Chirurgien senior (CS). Matériel et Méthode : cette étude prospective a inclus tous les patients pour lesquels une indication de PTH de première intention était posée par le CS responsable du patient du 02/11/2020 à 01/11/2022. Dix CJ ont participé à l'étude et avaient reçu une formation dédiée pour l'utilisation du logiciel et les principes de réalisation de la planification. La modélisation était faite par les CJ quelques jours avant la chirurgie en aveugle et en parallèle par le CS responsable sur une radiographie de bassin de face standardisée avec une bille de 25mm sur le logiciel MediCAD 2D Classic Hospital (version 6, MediCAD Hectec GmbH, Allemagne, Altdorf bei Landshut). Les analyses statistiques pour établir la courbe d'apprentissage étaient faites à l'aide du LC-CUSUM permettant de comparer la planification du CJ à celle du CS et des implants réellement posés afin de connaitre le nombre de procédures nécessaire à une planification équivalente à celle du CS. Résultats : l'ensemble des 10 CJ n'avait pas atteint la compétence pour une planification complète. Nous constatons des disparités dans l'acquisition de la compétence en fonction de l'implant étudié. Comparativement au CS, la compétence est acquise par 60% des CJ après 31,5± 12,9 [14 - 54] planifications pour l'implant cotyloïdien et pour 80% des CJ après 30,3 ± 8,3 [17 - 40] planifications pour la taille de l'implant fémoral. La taille du col fémoral est une compétence acquise par les 10 CJ après 23,1 planifications ± 6,8 [17 - 38]. En revanche la restitution de l'offset n'est correctement réalisée sur la planifications par seulement 30% des CJ au bout de 33,5 calques ± 11,6 [18 - 46]. Conclusion : il existe une courbe d'apprentissage pour la réalisation de la planification 2D des PTHs non cimentées. Il semble que les différents items de la planification présentent des courbes d'apprentissages différentes notamment pour la restitution de l'offset fémoral. La réalisation de 75 planifications n'est pas suffisante dans la globalité mais a pu démontrer la compétence des CJ pour la planification de l'implant acétabulaire, la longueur du col et la taille de la tige fémorale.
