This will bring clarity to the high standards expected and quality performance will be measured and published. Clinical and managerial leadership skills at the local level need further development, and all levels of staff will receive support through education and training (chapter 6). Book excerpt: ‘Whiteness’ is a politically constructed category which needs to be understood and dismantled because the system of racism so embedded within our society harms us all. It has profound implications for human psychology, an understanding of which is essential for supporting the movement for change. This book explores these implications from a psychoanalytic and Jungian analytic perspective. The ‘fragility’ of whiteness, the colour-blind approach and the silencing process of disavowal as they develop in the childhood of white liberal families are considered as means of maintaining white privilege and racism. A critique of the colonial roots of psychoanalytic theories of Freud and Jung leads to questioning the de-linking of the individual from society in modern day analytic thinking. The concept of the cultural complex is suggested as a useful means of connecting the individual and the social. Examples from the author’s clinical practice as well as from public life are used to illustrate the argument. Relatively few black people join the psychoanalytic profession and those who do describe training and membership as a difficult and painful process. How racism operates in clinical work, supervision and our institutions is explored, and whilst it can seem an intractable problem, proposals are given for ways forward. This book will be of great importance to psychoanalysts, psychotherapists, social workers and all those with an interest in the role of white privilege on mental health.

Book excerpt: The NHS needs to be an organization in which an open dialogue about care quality is part of the natural culture of the organization, not a duty which only arises in cases of service failure. Robert Francis made 290 recommendations in his report, but in truth they boil down to just one - that the culture of 'doing the system's business' is pervasive in parts of the NHS and has to change. Many who raise their concerns in the NHS at present risk serious consequences for their employment and professional status. But disciplinary procedures, professional conduct hearings and employment tribunals are not the proper place for honestly-held concerns about patient safety and care quality to be aired constructively. The NHS standard contract imposes a duty of candour on all NHS providers. This is an essential principle, but it is not adequately understood or applied. It should mean that all providers create a culture which is routinely open both with their patients and their commissioners. The same principle should apply to commissioners so that they are routinely open and accountable to local communities. The Health Committee recommended this approach in 2011 and repeats that now. It should be a prime role of the CQC to encourage the development of this culture within care providers, and of NHS England to develop the same culture within commissioners. The Health Committee will in future work closely with the Professional Standards Authority to develop the accountability process for professional regulators in healthcare

Book excerpt: The National Programme for IT in the NHS is an ambitious £11.4 billion programme of investment. This report is concerned with a central part of the Programme, where the aim was to create a fully integrated electronic care records system, which is expected to cost around £7 billion in total The Department has failed to demonstrate the benefits achieved for the £2.7 billion spent to date on care records systems and has accepted it is unable to deliver its original vision. It is now relying on individual NHS trusts to develop systems compatible with those in the Programme. Furthermore the Department could not explain how potential inconsistencies would be dealt with or what it will cost local NHS organisations to connect up. The Department has not got the best out of its suppliers, despite having paid them some £1.8 billion so far. One supplier, CSC, has yet to deliver the bulk of the systems it is contracted to supply and has instead implemented a large number of interim systems as a stopgap. The Department has been in negotiations with CSC for over a year, but conceded that it may be more expensive to terminate the contract than to complete it. The Department has also revised its contract with BT reducing the number of systems and increasing the price for each system delivered. This has resulted in BT being paid £9 million to implement systems at each NHS site, even though the same systems have been purchased for under £2 million by NHS organisations outside the Programme. The Committee is further concerned about the problems in getting timely and reliable information from the Department. Information provided has frequently been late, has contained inconsistencies and has contradicted other evidence.
