Book excerpt: De nombreuses recherches ont souligné l'importance de l'habitus lectural et des habiletés phonologiques dans le développement du langage (Ecalle & Magnan, 2002a). Toutefois, il existe peu d'études qui s'intéressent à l'exposition à l'écrit et à l'émergence des habiletés phonologiques chez les enfants de moins de quatre ans. L'objet de la présente étude est de comprendre comment ces deux composantes s'articulent dès deux-trois ans, afin de déterminer comment les habiletés phonologiques se développent et quels facteurs favorisent ce développement. Les expérimentations ont porté sur une large population composée de 52 enfants, âgés de deux à trois ans. Dans un premier temps, nous avons proposé aux enfants une épreuve lexicale issue d'une batterie récente (EVALO 2-6) et trois épreuves phonologiques que nous avons créées. Dans un second temps, les parents ont répondu à trois questionnaires les interrogeant sur le développement lexical de leur enfant (IFDC), leur connaissance des écrits environnementaux (QEE) et leurs pratiques lecturales. Pour ce dernier aspect, nous avons élaboré un outil spécifique dont les questions sont la synthèse des résultats figurant dans les études récentes. Nos résultats montrent que le niveau de langage oral est le meilleur indice pour expliquer la variance en habiletés phonologiques. Grâce à une analyse de régression hiérarchique, on parvient à prouver que la connaissance des écrits environnementaux des parents est également prédictive du niveau d'habiletés phonologiques de l'enfant.

Book excerpt: La prise en charge néonatale a fait ces dernières années d’importants progrès qui ont conduit à une diminution de la mortalité. Cependant, la naissance prématurée, jusqu’à 36 semaines d’aménorrhée, reste une situation à haut risque de séquelles neuro-développementales. Les troubles du langage sont plus fréquents chez les enfants nés prématurément. L’hypothèse d’une dynamique intégrative du développement du langage dépendant de la qualité des entrées neurosensorielles, plus fragiles chez le prématuré, ainsi que l’utilisation précoce de ses productions verbales pour les stabiliser souligne tout l’intérêt d’un dépistage précoce de ces troubles. Il s’agit d’aider ces enfants à un stade précoce pour bénéficier de la plasticité cérébrale disponible lors de la fenêtre de développement et donc de tenter de prévenir l’installation de dysfonctionnements cognitifs. Les moyens d’évaluation du langage oral de l’enfant de moins de 3 ans sont pour l’instant limités. Il est important de pouvoir s’appuyer sur des outils de dépistage efficaces. Les Inventaires Français du Développement Communicatif (IFDC) semblent être un outil intéressant, utilisés fréquemment en recherche et en pratique cliniques. Ils n’ont cependant pas été validés pour une population d’enfants nés prématurément. L’objectif principal de ce travail était de rechercher si, dans un groupe d’enfants non cérébrolésés, nés avant 33 SA, les résultats obtenus à un questionnaire parental de langage à l’âge de 2 ans (IFDC) étaient corrélés aux résultats obtenus à un bilan de langage à l’âge réalisé entre 3 ans et 3 ans et six mois (BILO petits). L’objectif secondaire était de décrire des facteurs pouvant influencer la trajectoire développementale des enfants (prise en charge médicale, niveaux d’études des parents). Il s’agissait d’une étude rétrospective monocentrique. Cinquante enfants ont été inclus. Nous avons pu mettre en évidence des corrélations statistiquement significatives entre le nombre de mots produits à 2 ans et la répétition de mots (R=0,40 et p=0,003), la production lexicale concernant les noms (R=0,31 et p=0,028) et la production d’énoncés (R=0,31 et p=0,024) à 3 ans-3 ans et demi. Nous n’avons pu établir aucune corrélation statistiquement significative entre l’item longueur moyenne des énoncés de l’IFDC et les différents items de BILO petits. Les enfants de parents d’un niveau d’études supérieures obtenaient des scores statistiquement meilleurs que les autres enfants aux items de production de mots à l’IFDC et de lexique des verbes à BILO. Le questionnaire de l’IFDC à 24 mois utilisé seul, ne nous a pas semblé être un outil suffisant pour apprécier le développement du langage dans notre population d’anciens grands prématurés. Le niveau d’études des parents joue très probablement un rôle important dans la trajectoire développementale de ces enfants. Il est nécessaire de s’intéresser aux étapes sensorimotrices plus précoces dans le développement pour limiter les dys-stimulations, favoriser les intégrations multi-modales et identifier précocement les contraintes.
