Depuis plusieurs années des marqueurs enzymaîiques de plus en plus sensible et spécifique ont permis la détection précoce de toute souffrance myocardique ; Ceux-ci engendrent de réels progrès dans la prise en charge des patients au sein des unités d'urgences. Dans le cadre d'une expérience de biologie délocalisée, nous avons réalisé à l'hôpital Saint-Antoine pendant un an dans le service des urgences une étude rétrospective pour évaluer l'intérêt du dosage de la troponine I. Cette étude a mis en évidence que seulement 14% des dosages réalisés étaient positifs ce qui implique une demande trop peu justifié (6% des patients venant consulter aux urgences avaient un dosage de troponine I). La valeur seuil étant fixé à 0.15 nanogramme par millilitre, dans notre étude seulement 38% des patients ayant une troponine élevée souffraient d'une insuffisance coronarienne aiguë Toutefois, le problème des surcoûts engendrés par l'expérience de biologie délocalisé est compensé par une prise en charge des patients optimale permettant souvent d'éviter des hospitalisations injustifiées dans les services de réanimation cardiologique. Le dosage se révèle vraiment utile en l'absence de signes cliniques ou éléctrocardiographiques typique permettant d'orienter Furgentiste dans sa démarche de soins.
