Book excerpt: Le but de cette étude in vitro est de comparer l'incidence de 3 méthodes de préparation canalaire sur la pénétration des irrigants. 36 dents saines, fraîchement extraites pour raisons orthodontiques, parodontales ou prothétiques, sont réparties de manière aléatoire en 3 groupes. 2 systèmes de préparation canalaires utilisant des instruments Ni-Ti en rotation continue, le HeroShaper® et le Protaper® sont comparés à une technique manuelle et mécanisée, forets de Gates et limes manuelles Flexofile®. L'hypochlorite de sodium a été remplacé par un produit de contraste, le Radiosélectan® 30 %. La pénétration de l'irrigant est mesurée aux 4 stades essentiels de la mise en forme canalaire sur des clichés radiographiques numériques. L'analyse des résultats, à l'aide de tests statistiques non paramétriques, nous permet de réaliser des comparaisons. - Les comparaisons intergroupes faites pour chaque étade de mesure ont montré : des différences statistiquement significatives après évasement entre les groupes "Flexofile®" et "Protaper®", ainsi qu'entre les groupes "Flexofile®" et "HeroShaper®". ; aucune différence statistiquement significative n'a été mise en évidence pour la mesure effectuée après l'irrigation de la cavité d'accès, le travail des premier et dernier instruments à la longueur de travail. - Les comparaisons intragroupes ont également révélé des différences statistiquement significatives entre des étapes consécutives de mesures : pour le groupe "Flexofile®" entre évasement et premier instrument à la LT ; pour le groupe "HeroShaper®" entre cavité d'accès et évasement, ainsi qu'entre évasement et premier instrumet à la LT et, pour le groupe "Protaper®", entre toutes les étapes de mesure consécutives. Dans ces conditions expérimentales, l'instrumentation Ni-Ti améliore globalement la pénétration de l'irrigant lors de la mise en forme canalaire.
