Book excerpt: Contexte : la période des mille premiers jours de vie est fondamentale pour la santé ultérieure de l'enfant cependant les recommandations n'ont cessé d'évoluer. Objectif : l'objectif principal de l'étude était d'évaluer les connaissances des parents concernant l'alimentation de leur enfant âgé de 0 à 24 mois et d'évaluer le rôle de conseil du médecin généraliste (MG). Méthode : étude descriptive, transversale auprès de parents d'enfants de moins de 24 mois. Le recueil de données a été réalisé par la diffusion d'un auto-questionnaire sur les réseaux sociaux et dans 20 cabinets de MG. Deux types de scores ont été calculés. Le premier (score de connaissances) était basé sur l'ensemble des réponses au questionnaire. Le deuxième (score diversification) était spécifique aux questions autour de la diversification alimentaire. Résultats : au total, 874 réponses ont été analysées. Parmi les parents ayant répondu une grande majorité était des mères (95,9 %). La moyenne des scores de connaissances était de 48,7 % ± 13,6. Les méconnaissances concernaient l'âge d'introduction des fruits rouges et exotiques, de la viande et du poisson, de l'œuf, des produits laitiers, des matières grasses, du gluten et des fruits à coque et de l'arachide. 84 % (n = 727) des parents souhaitaient avoir à disposition des fiches d'information. Discussion : les résultats étaient nuancés. On notait une méconnaissance des recommandations récentes et de la notion de « fenêtre de tolérance immunitaire ». Les mères allaitantes semblaient retarder d'avantage la diversification alimentaire. Une population cible a été mise en évidence et pour laquelle les MG pourraient être plus attentifs : les pères, les parents dont l'enfant est allaité, les parents d'enfants de moins 4 mois, ceux avec plusieurs enfants. Conclusion : les parents affichent un besoin d'accompagnement plus précis. Des informations claires de la part des professionnels de santé avec des fiches d'information actualisées pourraient améliorer les pratiques de la diversification alimentaire et agir en termes de prévention des allergies.

Les recommandations préconisent actuellement une introduction des aliments à partir de 4 mois révolus, pour tout enfant. L'objectif principal était d'identifier les sources d'information des parents, et le rôle du médecin généraliste dans cette information. L'objectif secondaire était d'étudier les connaissances et pratiques des parents dans la diversification alimentaire. Matériel et méthodes: Une étude prospective, descriptive et quantitative a inclus de façon exhaustive les nourrissons de 4 à 8 mois consultant aux urgences pédiatriques ou en pédiatrie au CHSF, pendant 3 mois. Un questionnaire auprès des parents a recueilli les sources d'informations, leurs connaissances et les modalités du début de la diversification alimentaire. Résultats : Les parents s'informent essentiellement auprès du pédiatre, puis de la PMI puis du médecin généraliste. Celui-ci est en troisième position mais régulièrement sollicité par les parents dans la diversification alimentaire, de par sa proximité et son rôle de prévention. Les grandes lignes du début de la diversification sont en majorité connues, mais les parents se fient davantage à leur entourage pendant la mise en pratique. Conclusion : Ce travail valorise le rôle du médecin généraliste et apporte des pistes pour intégrer l'éducation nutritionnelle dans le suivi des nourrissons, informer les jeunes parents sur les recommandations et les stimuler à la réflexion sur les enjeux d'une diversification alimentaire adaptée. On peut imaginer la création d'une fiche à double volet (parents-médecin) récapitulant les idées-clés de la diversification.

Elaborer des fiches conseils pour les parents. METHODE : Il s’agit d’une étude qualitative basée sur un questionnaire semi-dirigé de mères dans un cabinet parisien de médecine générale. RESULTATS : L’information prénatale est encore insuffisante à propos de l’alimentation mais efficace dans la promotion de l’allaitement maternel. Les mères connaissent alors peu les bienfaits de l’allaitement. Ensuite, en post partum, elles consultent en général pour des problèmes mammaires ou pour le choix du lait mais trouvent peu d’aides lorsque l’alimentation devient compliquée. Le conseil reste flou lors de la diversification : elles voudraient être plus guidées car considèrent que la santé se joue dès le plus jeune âge. Selon elles, le médecin généraliste n’accorderait que trop peu de temps au conseil alimentaire et ne l’aborderait pas systématiquement. Les mamans n’osent pas le solliciter, ne sachant pas toujours qu’il est qualifié pour le suivi infantile et consultent souvent en premier lieu la PMI ou le pédiatre. Bien qu’elles s’estiment satisfaites du médecin généraliste, elles avouent utiliser des sources d’informations complémentaires (internet ...). Elles ont peu de traces écrites et le carnet de santé est lu comme une « bonne roue de secours ». Elles souhaiteraient des fiches conseils mensuelles ou des consultations dédiées à l’alimentation. CONCLUSION : Les mamans sont soucieuses de l’alimentation de leur nourrisson et le médecin généraliste un précieux allié. Mais de nombreux progrès doivent avoir lieu dans ce domaine. Quel est l’avis des médecins généralistes?
