Un rapport de l'HAS datant de 2009 fait le point sur la place réelle de cette technologie en imagerie dento-maxillo-faciale notamment dans les domaines de la chirurgie endodontique, de l'implantologie, de l'orthodontie et de la traumatologie dentaire. Les traumatismes dentaires et/ou osseux constituent un motif de consultation fréquent au cabonet dentaire. Le massif maxillo-facial est souvent exposé aux chocs (chutes, agressions...) et les conséquences sont parfois très douloureuses pour le patient. Leur diagnostic s'avère parfois difficile. Ainsi, les signes cliniques sont variés et dépendent de nombreux facteurs. Les différents types de traumatismes dentaires et osseux (variabilité du type de fractures, topographie variée des lésions osseuses etc...) nécessitent une connaissance des signes cliniques, mis en évidence par un examen clinique minutieux ainsi qu'une reconnaissance des différentes lésions existantes au moyen de l'imagerie notamment. En effet, l'imagerie fait partie intégrante de l'établissement du diagnostic mais intervient aussi lors des différentes phases du traitement et enfin du contrôle et suivi. Concernant les clichés 2D conventionnels, ils sont souvent utilisés en 1ère intention cependant ils peuvent se révéler insuffisamment précis ou pire peuvent laisser dans l'ignorance d'un diagnostic de traumatisme. L'évoluion de la technique d'imagerie volumique à faisceau conique dans l'imagerie dentaire et maxillo-faciale donne accès dans de bonnes conditions et avec une irradiotion moindre à l'établissement d'un diagnostic précis et ainsi à une amélioration de la prise en charge des patients.

Ainsi afin de pallier au manque de précision relatif aux superpositions de l'imagerie conventionnelle, se sont développées des technologies sectionnelles dont le scanner en est l'exemple type. Ce dernier est cependant peu à peu supplanté par la technique Cone beam (CBCT) ou tomographie volumique numérisée à faisceau conique. Ce travail a donc passé en revue la littérature scientifique sur l'imagerie dento-maxillo-faciale par CBCT depuis 2008. Près de 200 documents ont été recueillis mais seule la moitié était cliniquement pertinente. L'analyse de ces documents nous a permis de réaliser une actualisation des données acquises sur le CBCT :notamment concernant l'irradiation, allant vers une évolution en faveur du principe ALARA ; puis sur les artéfacts où une amélioration est notée mais toujours sous condition d'une utilisation maitrisée; et enfin en matière d'indications cliniques qui se sont multipliées, depuis la reconnaissance du CBCT par la HAS en 2009 et son entrée à la CCAM en 2012, dépassant le domaine de l'implantologie. Effectivement, même si les logiciels d'exploitation (Simplant®, NobelGuide® etc.) sont orientés implantologie, le CBCT peut également être employé en endodontie pour les traitements endodontiques compliqués, en parodontie pour le bilan parodontal, en chirurgie maxillo-faciale pour les dents incluses et les anomalies osseuses, en ODF et chirurgie orthognatique, dans la visualisation des défauts osseux des ATM ainsi qu'en traumatologie dento-maxillo-facial essentiellement chez les enfants. Toutefois si le CBCT connait une nette croissance, il est essentiel que les principes de justifications et d'optimisation soient respectés. De ce fait des formations spécifiques à l'imagerie CBCT du chirurgien dentiste se mettent progressivement en place permettant de répondre à ces objectifs de santé publique et de donner un avenir au CBCT en exercice libéral.
