Après avoir exposé les étiologies de l'absence de celle dent sur l'arcade ainsi que les moyens de diagnostic, nous envisagerons les différentes possibilités thérapeutiques qui peuvent s'offrir à ces jeunes patients pour lesquels le maintien de l'os alvéolaire est indispensable. Elles incluent : D'une part, lorsque la dent est incluse et récupérable, sa mise en place orthodontico-chirurgicale, améliorée par l'acquisition des techniques de chirurgie muco-gingivale. D'autre part dans les cas où l'incisive est irrécupérable ou absente, son remplacement par différents procédés les prothèses amovibles, les prothèses collées, la fermeture orthodontique de l'espace, dont nous évoquerons les conséquences et enfin les techniques de transplantations dont l'amélioration du pronostic montre l'intérêt d'inclure cette option dans nos plans de traitement orthodontiques si l'on dispose d'un site donneur. Six cas cliniques permettent d'illustrer ce travail. La réhabilitation d'un espace antérieur édenté par l'absence d'une incisive centrale maxillaire constitue un problème délicat à résoudre. Ainsi, aucune solution n'est idéale, toutes ont des conséquences esthétiques et fonctionnelles qu'il convient d'évaluer pour mieux les prévenir.

L'apparition de complications post-traumatiques reste fréquente. Des gestes inadéquats, un traitement tardif ou une absence de suivi entrainent une perte de chance pour les dents traumatisées avec des conséquences variables pouvant aboutir à plus ou moins long terme à l'extraction. La perte prématurée d'une dent antérieure chez un patient jeune occasionne des problèmes esthétiques, fonctionnels et psychologiques importants. Le praticien doit relever un challenge : gérer le présent, préserver l'avenir et réduire les complications liées à la perte d'une incisive centrale maxillaire. L'objectif est de rétablir les fonctions orales, d'éviter les parafonctions et les migrations dentaires, de permettre une évolution optimale des structures maxillo-faciales, de maintenir dans les meilleures conditions le volume osseux et la dimension verticale. Avec les progrès des technologies adhésives et de l'implantologie, de nombreuses options thérapeutiques s'offrent à nous avec leurs avantages et leurs limites. La décision reste complexe, la prise en charge est pluridisciplinaire et individualisée en fonction de chaque patient.
