Download or read book Comprehensive Diabetes Management Reinforcing Self management Education to Improve the Diabetes Outcomes written by Elizabeth Parackal and published by . This book was released on 2018 with total page 136 pages. Available in PDF, EPUB and Kindle. Book excerpt: Diabetes remains a significant health problem in the United States, and related complications are on the rise as evident by the growing prevalence due to care fragmentation and inadequate care coordination. This quantitative quality improvement project evaluated the effect of diabetes self management education (DSME) reinforcement intervention as a primary care solution for prevention and control of diabetes and prediabetes in a small convenient care clinic in Southern California. The purpose of this project was to design and implement a diabetes self-management educational reinforcement program to enhance patients’ knowledge on diabetes self management. Diabetes knowledge improvement was measured by using a reliable and valid pretest and posttest DSMQ questionnaire. The pretest HbA1c mean was 6.51 and the post-test mean was 6.91. There was a paired differences mean of -0.40. Results indicates a significant rise in post-project HbA1c value among the control group (p=

Book excerpt: The management of type 2 diabetes in the United States is a population health priority. Approximately 1 in 10 adults in the United States have diabetes and a significant majority have inadequately controlled blood glucose, blood pressure, and cholesterol. Previous research shows that many adults with diabetes lack the knowledge, skills, behaviors, and support to properly manage their condition. Diabetes self-management education has become an important part of diabetes care guidelines as a way to improve the self-management of adults with diabetes in order to lead to improve patient health and reduce inappropriate health care use and medical costs. Despite this, it is unclear who is actually receiving diabetes self-management education in the US, overall and by mode of delivery and diabetes educator type, and whether education is associated with improved population health. Therefore, the objective of this dissertation research is to investigate the distribution of diabetes self-management education, patient sociodemographic factors that influence the receipt of that education, and its contribution toward diabetes outcomes, medical expenditures, and healthcare use in US adults. We achieved this research objective by completing three specific research aims. We used nationally representative data from the Medical Expenditure Panel Survey and the National Health and Nutrition Examination Survey. We found that although a substantial majority of individuals received some form of diabetes self-management education, receiving diabetes education was associated with race/ethnicity and income (Aim 1), and varied by mode of education delivery. Individuals who received diabetes self-management education were less likely to have glycemic control but were more likely to receive antiglycemic treatment (Aim 2). Receiving diabetes self-management education was associated with greater diabetes-related ambulatory and prescription use, depending on the mode of education delivery (Aim 3). Our findings highlight some strengths of the current state of diabetes management occurring in usual care, including the widespread delivery of some form of diabetes education and its association with greater ambulatory and prescription use. However, we also found evidence of the need for more effective diabetes education delivery systems in usual care in order to improve patient self-care behaviors and reduce inpatient and emergency room use.

Book excerpt: "Diabetes is a serious health problem in the United States. Scholarly literature review demonstrates that self-management education is the cornerstone of an effective treatment plan. The private practice of a local endocrinologist lacks a formal diabetes education program. Diabetes self-management education training (DSME/T) is necessary in the early stages of the disease and as needed throughout the individual's life to ensure optimal glycemic control. The purpose of this evidence-based practice (EBP) change project was to develop and provide a comprehensive diabetes education program to patients with Type 1 or Type 2 diabetes. The intended outcome of the patient educational program was improved self-management strategies and increased knowledge regarding diabetes. A secondary purpose was to develop a consistent, patient-focused education program for individuals with diabetes in southern New Jersey. This was accomplished by implementing a comprehensive diabetes education program utilizing the 2012 National Standards for Diabetes Self-Management Education. A series of three educational classes were conducted over a three-week period. Statistical analysis of pre-intervention, conclusion, and four weeks after intervention scores on the Summary of Diabetes Self Care Activities questionnaire was utilized to validate the effectiveness of the evidence-based practice change project. Implementing a comprehensive diabetes education program has demonstrated to be an effective means to achieve national guidelines for the treatment of diabetes. Nursing implications are multiple but the most important is the development of a comprehensive diabetes education program. The EBP change project has contributed to increased self-management strategies, improved blood sugar control, and improved quality of patient care." -- Abstract.

Book Diabetes Self management Education

Download or read book Diabetes Self management Education written by Kimberly Wilkins and published by . This book was released on 2014 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Many patients with diabetes are not able to manage their disease effectively because they do not have a full understanding of their diagnosis and how their lifestyle choices and medication decisions impact their risk for developing complications (Bodenheimer, Lorig and Holman, et al, 2010). Ineffective self-management leads to the development of complications including neuropathy, nephropathy, hypertension, gastroparesis, and glaucoma, and increases the risk for infection (American Diabetes Association, 2014). Research demonstrates that diabetes self management education is an effective method of improving disease management and leads to better patient outcomes (Kent et al, 2013). Implementing a diabetes self-management education program in the primary-care setting is an effective method of empowering patients to manage their disease effectively. A team-based approach with the patient as the central figure in the decision-making process has been shown to improve patient outcomes. Both knowledge and understanding are key, particularly for newly diagnosed patients. Implementation of the diabetes self-management program at a community health center involves a team-based approach to healthcare. The patient, physicians, nurses and other healthcare professionals work together to develop an individualized plan care to meet the needs of the patient. Patient involvement is key and self-management education is critical for improving patient outcomes. Patient self-efficacy is also an important factor in determining changes in behavior that will impact health (Riverside Community Health Foundation, 2002). Comprehensive and ongoing patient self-management education with a multidisciplinary team will effectively meet the needs of patients and will improve patient self-management of diabetes. Teamwork, collaboration and improving patient self-efficacy are integral aspects of the program.

Book Diabetes Self Management Education

Download or read book Diabetes Self Management Education written by Tracy Riddle and published by . This book was released on 2015 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: "Purpose. The purpose of this evidence-based project was to provide diabetes self-management education (DSME) to participants to increase participant knowledge of diabetes self-management behaviors and decrease fasting blood glucose levels. This project was based on published evidence surrounding DSME and the positive outcomes associated with educating individuals with diabetes about self-management behaviors. Methods. Participants were recruited at a rural health care clinic to include patients with insulin and non-insulin dependent diabetes over the age of 18 years that can read and speak English proficiently. Using a pretest/posttest design, participant's knowledge of self-management behaviors were measured before and after the educational intervention session using a self-developed tool, the Diabetes Self-Management Questionnaire (DMSQ). Fasting blood glucose levels were also documented prior to the intervention and post intervention by patient report via telephone. Results. Forty-two participants were included in the project. Mean pretest scores were 60% and posttest mean scores were 80%; posttest scores illustrated a 500% increase in participant's scoring {601} 80%. Overall fasting glucose decreased by only 3 points, indicating deficient applicability of the information outside the clinic setting. Conclusions. Findings suggest DSME is effective in teaching patients in the clinic setting, but more emphasis is needed to assist patients in applying this information outside the clinic setting. Key words: blood glucose; diabetes; diabetes self-management education (DSME); Diabetes Self-Management Questionnaire (DSMQ); evidence-based practice (EBP) " -- Abstract.

Book The Importance of Diabetes Self Management Education

Download or read book The Importance of Diabetes Self Management Education written by Lisa Davidson and published by . This book was released on 2014 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: There are gaps that are present between providing diabetes self management education and actually executing diabetes self management education. Implementing standardized screening tools and education on self care management for nurse to use along with a team approach that includes podiatrist, diabetes educator and nutritionist can help to close that gap and reduces the incidence of diabetes complications. Diabetes is a chronic and complex disease that requires ongoing self management care to prevent complications that includes amputations, disability and even death. The longer the disease is left unmanaged the higher the risk of disabling or life threatening complications. Diabetes education has been proven to help people fully understand their disease process and self management through training and support so that they do not feel isolated. Diabetes education is important to producing good patient outcomes. Diabetes education focuses on eating healthy, staying active, monitoring of blood sugar levels, medication administration and risk reduction. The goal of diabetes education is to create and individualized plan and the tools and support to motivate patient to continue their treatment regimen. Diabetes self management education is the basis for empowering patient to effectively manage their disease and make the important decisions about their care daily. A team approach is beneficial for patients in that the education can be taught from different professionals knowledgeable on diabetes care and management. The information can be practiced and reinforced throughout their hospital stay on a daily basis in order to ensure complications. Doing so will enhance compliance and accountability for self management.

Book Comprehensive Diabetes Guide

Download or read book Comprehensive Diabetes Guide written by Grace Ampofoh and published by Xlibris Corporation. This book was released on 2022-05-22 with total page 56 pages. Available in PDF, EPUB and Kindle. Book excerpt: Author Grace Atea Ampofoh was diagnosed with Type-2 diabetes at the age of twenty- eight. During her long period of nursing career, the author took care of many elderly diabetic patients, including family members. “Comprehensive Diabetes Guide” is a book written not only for her legacy, but also for her contribution to worldwide campaigns for diabetes research and awareness of the disease, as well as diabetes self- care management. Updates of current reports and information as related to diabetes have been highlighted in this health resource guide. To achieve and maintain a healthy lifestyle as a diabetic, Atea Ampofoh added that self- management of diabetes is imperative. She said, “Just because you’ve been diagnosed with diabetes does not mean your life cannot be a wonderful and exciting journey.” The author had long experience in Gerontology, and she worked hard daily as a Registered Nurse, and as a diabetic patient herself. So, she understood and learned to be an advocate, a champion, who has been working wholeheartedly to raise awareness of diabetes. The author challenges her audience, diabetics and health-care providers to work closely with each other for successful outcome. Regarding management of all types of diabetes, medical supervision by routine doctors’ visits is necessary. Based on the laboratory tests’ results, the doctor can evaluate how the patient is handling the disease throughout the journey to recovery. The doctor can change medications or adjust medication doses, to control high blood glucose levels to normal range. A diabetic patient can be referred to diabetes management classes with various health-care personnel, including dietician for diet control, fitness center for exercise and weight control. Government programs such as Medicare and Medicaid are utilized for financial assistance. Everyone plays a role in the society to support diabetes management globally. Diabetes is a condition that you must just live with longer and take good care of managing your disease, and your health in general. Although this might be hard, time consuming, and requires the help of loved- ones, health- care team and the society in general. It is hard, but it is a choice you have to make for your survival. Comparatively, it is not easy either, for example, an epileptic individual to live with this illness in life, and that he or she has to suffer from the negative impact, emotionally and physically in some cultures. In fact, Diabetes must not slowly get worse or control your life, because if you have diabetes, you have to be in charge of living a healthy lifestyle. You can control prediabetes from slowly reaching Type-2 diabetes, and Type-2 from getting to complications of diabetes, including heart disease, stroke, nerve damage, eye disease, and kidney disease. Today, there is hope and more opportunity for diabetics than a hundred years ago. More research workers have tried to develop new plans for diabetes management, such as home management education, diabetics with complication of kidney disease can receive kidney transplant treatment or dialysis, and even both. Wow! Same individuals living with diabetes can live longer because of these interventions.

Book Self care Management Education for Diabetic Patients

Download or read book Self care Management Education for Diabetic Patients written by Shirly George and published by . This book was released on 2015 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Diabetes self-management education (DSME) is an essential element of care for all people with diabetes. Patients have to be open and willing to learn of what has to be done to correctly manage diabetes in their daily lives. Healthcare professionals can help educate the patients with diabetes with regards to their self-care needs. By providing them with ample support, knowledge, and information regarding their proper diet and nutrition, exercise regimen, glucose monitoring and medication administration, patients will be able to better manage their care whether at home or in the community. By being able to participate in their own care provides them with self-confidence that they can handle and in turn cope with their condition gives them the ability to manage their own lives more effectively. This paper determines the importance and effectiveness of a DSME to a diabetic patient. To provide DSME to the target individuals, one must first set goals of what one wants to achieve, create a plan of how to achieve the desired goals, implement actions, collect data and evaluate the results. If the results do not meet the desired goals then appropriate changes have to be made in order to correct them so that the desired goals will be achieved. Participants are needed to complete questionnaires regarding their self-care practices. At completion, results are tallied and submitted to the stakeholders and nursing community for review. Effectiveness of the DSME are then taken into consideration and shared to the community to emphasize its benefits to affected individuals.

Book The Impact of Diabetes Self management Education on Management of Type 2 Diabetes

Download or read book The Impact of Diabetes Self management Education on Management of Type 2 Diabetes written by Chekwube M. Okoli and published by . This book was released on 2015 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Different studies have shown the impact of Diabetes Self Management Education (DSME) in preventing diabetes exacerbation and possible complications of type 2 diabetes mellitus. Type 2 diabetes mellitus is a major global problem which continues to affect millions of people and has been ranked number seven in the causes of death within the United States ( Centers for Disease Control and Prevention, 2013). Exacerbation and complications associated with type 2 diabetes has tremendous negative effect on individuals , families and the general public. There are huge expenses in the treatment of diabetic complication than it is to prevent or control it. The proposed solution for this problem is individualized, self-centered diabetic education and follow-up care. This is a crucial component in the management and treatment of the type 2 diabetes mellitus. Proper, efficient and individualized Diabetes Self Management Education (DSME) will definitely improve glucose control, prevent hospitalization, avoid short and long term complications, improve quality of life, and save cost ( American Diabetes Association, 2015 ). Studies shows that there is significant knowledge and skill deficit in 50% to 80% of diabetic patients on diabetic management. This study suggests that patients with low baseline knowledge should receive special attention in Diabetic Self Management Education ( DSME) programs, as knowledge gain in this group can significantly improve glycemic control (Ogrizio, Otero, L.M., Zanetti, M.L . 2007) . Studies has shown that diabetic counseling and education enhances great improvements in glycemic levels control and ultimately helps in the management of diabetes (Dalal, M. R., Robinson, S.B., and Sullivan, S.D. 2014).

Book Diabetes Self management Education a Powerful Tool for Diabetes Control in India

Download or read book Diabetes Self management Education a Powerful Tool for Diabetes Control in India written by Majumder Anirban and published by . This book was released on 2017 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Diabetes self-management education: a powerful tool for diabetes control in IndiaDiabetes self-management education (DSME) is a critical element of care for diabetes management and is a powerful tool for reducing glycosylated haemoglobin (HbA1c), risk for complications, hospital admissions and cost of care.1 The purpose of this study was to determine the impact of DSME in improving the outcomes of diabetes care as measured by glycaemic (FPG, PPG & HbA1C), lipid (total cholesterol[TC], triglycerides[TG], low density lipoproteins[LDL] and high density lipoproteins [HDL]) and anthropometric parameters (weight, BMI & waist circumference) in individuals with type 2 diabetes mellitus (T2DM).A retrospective analysis was performed for 160 adult T2DM patients who received DSME training in 2013u20132014 from our center. DSME provided to the study participants was consistent with the requirements from the National Standards for Diabetes Self-Management Education.1 Participants received one time 6 hours intensive group training. All education was provided by a team of diabetologists, certified diabetes educators who in addition are dietician and exercise physiologist. Patients received instruction in: diet/exercise education; self-monitoring of glucose levels; medication management specifically for insulin users; diabetes related problem solving, and lifestyle changes to help reduce the risks and complications of diabetes. Subjects were judged for their acquired knowledge about the disease by using a set of questionnaire and were awarded a score based on their performance. Their socioeconomic status was also ascertained by using Kuppuswammy scoring (KS score)2 which looked at their education, monthly income and occupational status. The subjectsu2019 glycaemic parameters, lipid parameters and anthropometric measurements were checked before the DSME programme and at the end of 6 months and 12 months respectively. The DSME scores were divided into tertiles and lower tertile patients were compared to intermediate and higher tertile patients, to assess, whether differences in acquired knowledge about diabetes had impact on glycaemic, lipid and anthropometric parameters. It was revealed that waist circumference increased in all the three tertiles over the entire year following DSME without any statistically significant change in the weight and BMI over the year. Subjects with DSME score in the lowest tertile had an initial improvement in the FPG values at 6 month but a rebound increase at the end of the year made it lose statistical significance. There was however no statistically significant change in the HbA1C and PPG over the year. The lipid parameters (TC and LDL) showed statistically significant progressive deterioration which perhaps emphasises the need for an intermittent reinforcement of education. Subjects with DSME score in the intermediate tertile had an improvement in the glycaemic parameters (FPG and HbA1C) throughout the year but there was no improvement in the lipid parameters . Subjects with DSME score in the upper most tertile were already having better controls of their glycaemic parameters at the baseline and did not show significant improvements over the year but their lipid parameters (TC and LDL) did show a significant improvements over the year, perhaps secondary to their better understanding of the contribution of lipid control towards prevention of macro vascular complications. Further analysis of the data also revealed that DSME scores were independent of socioeconomic status even when analysed separately for income levels and educational qualification. In India where the large number of diabetic population has a considerable economic implication on the state as well as on the individual, more widespread use of DSME can be a cost-effective option in fighting against the growing menace of the diabetes epidemic. In conclusion, comprehensive diabetes outcomes were correlated with adequacy of baseline diabetes education. Subjects with even minimal diabetes education (lowest tertile) had transient improvement in outcomes whereas those with optimal education had sustained multifactorial benefits. Moreover DSME benefits were independent of socio-economic and educational status indicating its widespread applicability even in resource limited settings. In India where the large number of diabetic population has a considerable economic implication on the state as well as on the individual, more widespread use of DSME can be a cost-effective option in fighting against the growing menace of the diabetes epidemic.References :1. Book Improving Self Management for Type 2 Diabetes

Download or read book Improving Self Management for Type 2 Diabetes written by Burke Harrison and published by . This book was released on 2014 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Diabetes is a health problem that occurs when the pancreas does not create enough insulin to sustain normal glucose levels, leading to hyperglycemia. Type 2 diabetes continues to be one of the most challenging worldwide health care issues for patients and health care providers in today's society. This disease affects millions of people each year and with the increasing occurrence the problem may grow rapidly, leading to increased hospital readmission rates and more complex care for health care workers. Diabetes can lead to severe long-term health problems, such as chronic kidney disease, vascular problems, blindness, or even death. To tackle this issue all health care professionals that provide direct care must learn accurate techniques to educate self management interventions to diabetic patients and patients must be more involved in their treatment plans to manage their disease process. Research indicates that self management for controlling diabetes is a vital component of a patient's treatment plan. Patients must receive education about medications, exercise, healthy food choices, and screening. Based on a variety of research studies in regard to self management education, participants learned ways to self manage diabetes which led to positive outcomes. Participants and patients changed their outlook and behaviors, underwent self management education, and utilized exercise and healthier food choices for self care interventions to gain better outcomes. Some positive outcomes included lowering HbA1C levels, weight loss, and a decrease in the daily glucose tests. The implementation of self care for diabetes is very important and can improve outcomes, quality of life, health care costs, and lead to less complications.

Download or read book Self management Programs that Increase Health Outcomes and Perceived Health Status in Patients with Chronic Disease Specifically Diabetes written by Tiffany Wachowiak and published by . This book was released on 2015 with total page 63 pages. Available in PDF, EPUB and Kindle. Book excerpt: There is increasing evidence to suggest that self-management education programs can complement medical care for patients with chronic conditions, particularly diabetes. Research indicates that self-efficacy is important to improve outcomes and perceived health status for patients with chronic disease including diabetes. This nine article review of research literature was designed to identify self-management programs that positively impacted the health outcomes and perceived health status of those individuals with diabetes in both rural and urban communities. The review included quantitative studies done between 2008 and 2015 in the United States. Findings support the value of disease self-management programs for long-term support of diabetic patients to improve health outcomes and perceived health status of their chronic disease. To facilitate continuous reinforcement to achieve optimal health and perceived health status of these patients, specially trained nurses are needed within the community health setting.
