Book excerpt: The purpose of this study is to demonstrate how to improve the measurement of access through changes in the Department of Defense's medical information system, the Composite Health Care System (CHCS). The research question is, "what percent of our beneficiaries who make contact with an appointment specialist are unable to receive an appointment?" Specific attention is given to how many beneficiaries try to get an appointment through the appointment specialist, only to be told there are no appointments available. This study found that in the month of January 1998 eleven percent of the TRICARE Prime enrollee callers could not get an appointment at Military Treatment Facilities (MTFs) located in the Colorado Springs area, also known as the Pikes Peak Region. In that same month, nine percent of the TRICARE Prime enrollee callers could not get an appointment at Evans Army Community Hospital (EACH). Medical Treatment Facilities currently have no method for measuring how many beneficiaries are turned-away other than through trial and error or through Military Health Service (MHS) Performance Report Card survey. This study recommends a method for measuring the number of beneficiaries denied appointments. Those recommendations come in the form of slight modifications to the existing information system, CHCS.

Book excerpt: GAO provided information about the Department of Defense's (DOD) progress in developing and testing the Composite Health Care System (CHCS), which is intended to integrate a wide array of data needed by military physicians and facilities in managing and treating patients. GAO found that: (1) DOD wants to deploy the first phase of CHCS before addressing important capabilities involving the identification, elimination, and prevention of multiple patient records and record archival and retrieval; (2) those capabilities are critical to physician acceptance and use of CHCS and the system's ability to support clinicians' inpatient activities; (3) DOD has delayed the incorporation of inpatient order-entry capability until the second phase deployment scheduled for 1994, but faces a major developmental risk in its design and testing; (4) inadequacies in operational test and evaluation (OT&E) planning and implementation yielded inconclusive test results that may not be representative of the environment in which CHCS is to be deployed; (5) DOD is attempting to address 17 fundamental CHCS weaknesses identified in spite of the OT&E inadequacies; (6) CHCS life-cycle costs exceed the congressionally established $1.6-billion ceiling by more than $400 million; and (7) DOD has had difficulty in estimating and validating CHCS benefits, with five recent benefit studies estimating cost savings ranging between $1.7 billion and $3.8 billion. GAO believes that CHCS is not ready for deployment, since major deficiencies still exist in system capabilities and the scope and quality of testing have been inadequate.

Book excerpt: Pursuant to a legislative requirement, GAO reviewed the Department of Defense's (DOD) Composite Health Care System (CHCS), focusing on: (1) DOD efforts to complete deployment of CHCS to military medical treatment facilities worldwide; (2) DOD efforts to address previously identified problems; and (3) a new CHCS operational issue. GAO found that: (1) DOD completed deployment of CHCS to 526 medical treatment facilities worldwide, which was difficult because of the system's complexity and the number of sites involved; (2) two DOD officials ensured the deployment's success by providing leadership and using fundamental information management practices; (3) DOD expects CHCS benefits to exceed its costs by $1.3 billion over the system's expected life; (4) CHCS should improve scheduling, give greater and quicker access to patient information, and increase the timeliness of medical care; (5) DOD has made progress in addressing its two previously identified problems by developing a prototype clinically oriented graphical user interface to make patient order-entry less cumbersome and strengthening the tools and methodology needed to manage CHCS performance; (6) DOD has updated its CHCS performance management plan and developed performance simulation models for each CHCS computer platform; (7) the lack of an effective plan for rapidly repairing or replacing CHCS equipment damaged by disaster remains a problem; and (8) DOD did not address this problem because of cost concerns and a lack of accurate information, but it is reconsidering its options for providing equipment adequate backup.

Book excerpt: The Composite Health Care System (CHCS) is a state-of-the-art, integrated medical information system the Department of Defense is considering for its 767 medical treatment facilities worldwide. Our objectives were to (1) determine whether Defense will be able to test and evaluate CHCS adequately before its planned procurement/deployment decision, (2) identify Defense's latest cost and funding estimates, and (3) evaluate the reasonableness of Defense's projected benefits. We visited Fort Knox, which serves as Defense's test site for new software development, and 6 of 11 OT & E sites; met with Defense and contractor representatives; and reviewed government and contractor documents. We conducted our evaluation from July 1989 to March 1990, in accordance with generally accepted government auditing standards. We have concerns as to whether Defense will be able to realize the projected CHAMPUS benefits. While CHCS may allow facilities to treat more patients, current CHAMPUS regulations allow beneficiaries to get outpatient care from civilian hospitals and physicians without Defense's approval and regardless of whether the military facility has excess capacity. Additionally, the benefit study did not consider restrictions on the number of patients specialists may treat during a given period. (KT).
