Suite à cette "révolution esthétique" et à l'émergence de nouvelles techniques de mise en oeuvre, le tout céramique s'est fortement développé. Différents types de céramiques sont apparus, pour le bien de la discipline, créant peut-être la confusion dans les esprits. Ainsi, si les matériaux à la disposition des praticiens sont nombreux en matière de reconstitution de l'organe dentaire, le choix de l'un ou l'autre se fait plus selon l'expérience ou les habitudes du praticien que par un choix éclairé. Pour palier à ce défaut, les études sur les propriétés des matériaux se multiplient, visant ainsi à permettre une sélection éclairée et judicieuse de la part des chirurgiens-dentistes. La propension plus ou moins forte des alliages métalliques à relarguer leurs composantes sous forme d'ions dans le milieu ambiant a conduit à la création d'un nouveau type d'armature, donnant ainsi naissance à des restaurations entièrement en céramique : armature et couverture. Il faut donc maintenant vérifier le caractère, supposé inerte, des céramiques dentaires dans le milieu buccal. Cette thèse est consacrée plus particulièrement au "comportement in vitro des céramiques dentaires dans le milieu salivaire". Dans un premier temps, les céramiques dentaires sont décrites. Dans une deuxième partie, la salive est abordée. Et le troisième et dernier temps concerne l'étude expérimentale. En s'appuyant sur les travaux précédemment menés par F. Ricard et le Dr Archien (1997) sur l'attaque salivaire des céramiques dentaires, nous développons le protocole, le matériel, la méthode et le but de l'expérimentation. Les résultats et leur discussion clôturent cette étude, en envisageant les conséquences du comportement in vitro des céramiques dans le milieu salivaire sur la gestuelle et les décisions thérapeutiques en odontologie prothétique et implantaire.

Telle qu'elle est définie classiquement, la facette est « un artifice prothétique de fine épaisseur, collé à l'émail et destiné à corriger la teinte, la forme et la position des dents », elle semble donc trouver sa place au sein d'une dentisterie conservatrice et esthétique même si elle ne correspond pas aux premières étapes du gradient et que souvent cette thérapeutique n'est pas utilisée en première intention. Les facettes font partie de la dentisterie conservatrice, la préparation dentaire suit le principe d'économie tissulaire (préparation de la dent a minima, respect de la vitalité pulpaire). Les facettes constituent ainsi un traitement de choix pour les dents antérieures présentant des altérations coronaires ou des malpositions qui peuvent être inesthétiques. Elles concernent des situations cliniques spécifiques : délabrement dentaire peu important, rapports occlusaux non traumatiques, hygiène orale satisfaisante et absence de pathologie parodontale. Les céramiques dentaires sont les biomatériaux présentant le meilleur potentiel biomimétique, et elles possèdent d'excellentes caractéristiques mécaniques une fois collées à l'émail. Cependant, elles ne sont pas exemptes d'inconvénients : leur coût de mise en oeuvre est élevé, et le moindre défaut comme un simple chipping aboutie à la dépose de la pièce et à son remplacement. Des études cliniques à moyen et long terme ont démontré l'excellent maintien du résultat esthétique, la satisfaction totale des patients et l'absence de pathologies gingivales associées. En effet, il a été constaté que le taux de succès des facettes céramiques à 10 ans était de 95% (contre 7 ans pour une couronne). Ces résultats témoignent donc de la pérennité des facettes dans le temps, à condition de poser le bon diagnostic, suivre les bonnes indications et respecter le protocole élaboré. Pour conclure, les facettes céramiques répondent parfaitement à la demande esthétique de plus en plus forte des patients, tout en respectant au maximum les tissus dentaires sains et en apportant un mimétisme parfait avec les dents naturelles, pour un temps de réalisation assez court et une longévité importante."
