From a psychological standpoint, families formed by complex adoption and assisted reproductive technology (ART) are first and foremost just that--families. Yet they also face a unique array of issues and challenges that may be clarified and resolved in the therapeutic setting. This much-needed book provides a deeper understanding of the ways that complex adoption and ART shape the life experience of children and parents, identifying important areas and methods for assessment and treatment. Combining developmental and ecological research with in-depth case material, the book establishes an integrative framework for clinical practice. The authors draw upon knowledge and skills gained from working in a variety of new family contexts. In the area of adoption, many new options have evolved that differ from traditional practices of adoption at birth. Thousands of older children in foster and institutional care in the United States and abroad are awaiting permanent placements. Open adoption, kinship adoption, and transracial adoption are also transforming family life, as is the use of ART, which raises significant issues of family identity and family process. The book explores such key themes as the significance of early experience, the capacity to recover from exposure to trauma, the impact of heredity and the difference that environment can make, and the centrality of primary attachment relationships. Also discussed are the impact of bias and other issues affecting families of difference, including lesbian and gay families. Concluding chapters consider promising future directions for training and research. A complex and understudied system, transnational adoption opens a window onto the relations between nations, the inequalities of the rich and the poor, and the history of race and racialization, Transnational adoption has been marked by the geographies of unequal power, as children move from poorer countries and families to wealthier ones, yet little work has been done to synthesize its complex and sometimes contradictory effects. Rather than focusing only on the United States, as much previous work on the topic does, International Adoption considers the perspectives of a number of sending countries as well as other receiving countries, particularly in Europe. The book also reminds us that the U.S. also sends children into international adoptions—particularly children of color. The book thus complicates the standard scholarly treatment of the subject, which tends to focus on the tensions between those who argue that transnational adoption is an outgrowth of American wealth, power, and military might (as well as a rejection of adoption from domestic foster care) and those who maintain that it is about a desire to help children in need.

Throughout the book these concepts are discussed in the context of what social workers face when trying to find explanatory structures for the ways in which early childhood experiences affect later life. Later chapters turn even more directly toward practice. Using case examples—including adolescent parents and their children, children with a depressed parent, and children of substance abusing parents—Applegate and Shapiro show clinicians how to make use of neurobiological concepts in designing treatment plans and interventions. One chapter contains three extended case examples, with commentary, representing the three most common intervention models taught in schools of social work—psychodynamic, cognitive-behavioral, and systemic. In order to be effective and meet the complex challenges of the twenty-first century, social work professionals must join with their colleagues in other disciplines in coordinated efforts to integrate and apply newly emerging knowledge toward the enhancement of human well-being.
