During the course of orthodontic treatment, enamel decalcification is a common problem. Demineralized enamel, the precursor to caries formation, can be attributed to fixed orthodontic appliances and prolonged exposure to bacterial plaque. Progression to clinically detectable white spot lesions may occur as early as one month following the placement of orthodontic appliances. Such problems have influenced clinicians to search for a solution. Because fluoride treatment immediately upon debonding is not advocated, clinicians have proposed fluoride treatment and fluoride-releasing materials at the commencement of the therapy. The two new anti-cavity toothpastes, ClinproTM 5000 with 1.1% Sodium Fluoride and ClinproTM Tooth Créme with 0.21% Sodium Fluoride have been shown to be useful in reduction of white spot lesions. Both the Clinpro TM products (3M ESPE) contain fluoride as well as tri-calcium phosphate, which are components naturally found in saliva. The aim of this study is to determine if ClinproTM 5000, ClinproTM Tooth Créme, or MI-Paste Plus has an effect on the formation and resolution of white spot lesions for patients undergoing orthodontic treatment. Methods: Three prospective groups of 40 patients undertaking routine orthodontic treatment were evaluated (total recruitment of 120 subjects). Patients must have their permanent dentition, be 12 years or older, and have not used extensive fluoride regimes before. Patients were chosen if they appear that they would be compliant as judged by the investigator. The selected product was brushed on the teeth by the patient for two minutes, twice daily for a total of 4 months. After brushing, the patients were instructed to expectorate only, not rinse, eat or drink anything for at least 30 minutes. The subjects were observed every 4 weeks. At each visit, 3 intraoral photos were taken (frontal and buccal views) and measurements based on the Enamel Decalcification Index (EDI) will be recorded. The Enamel Decalcification Index was used to determine the number of white spot lesions and caries risk detected in these photographic records. Also, a brushing diary was reviewed. A trained member of the 3M ESPE Clinical Research group did monitor the study by means of visits to the clinic to evaluate patient charts, study data, and study photographs. Results: Altogether, the results provided strong support with regard for ClinproTM 5000 providing superior enamel protection against decalcification when compared to ClinproTM Créme, and MI Paste. Conclusions: The use of ClinproTM 5000, ClinproTM Créme, and MI paste demonstrated less formation of white spot lesions when compared to data in the literature. ClinproTM 5000 had a marginally better affect compared to the other two products.

Book excerpt: Introduction: Although orthodontics can correct a variety of dental and skeletal malocclusions, research has shown an increased risk of enamel demineralization leading to white spot lesions (WSLs). The purpose of this study was to compare the effectiveness of Pro Seal and Opal Seal, two fluoridated bonding agents, to Assure, a nonfluoridated bonding agent, in preventing demineralization during orthodontic treatment. Methods: 26 patients beginning comprehensive orthodontic treatment were divided into 3 groups. 9 patients were bonded with Assure (control), 9 patients were bonded with Pro Seal, and 8 patients were bonded with Opal Seal. Pretreatment and six-month progress photographs were used to calculate demineralization rates, and statistical analysis was performed to determine if any bonding agent is better at preventing WSLs. Results: There was no statistically significant difference in demineralization rates between the three bonding agents; however, descriptive statistics suggest that Pro Seal and Opal Seal may prevent demineralization more effectively than Assure.

Book excerpt: Introduction: The purpose of this study was to evaluate the effect of 1.23% acidulated phosphate fluoride (APF) gel on white spot lesion (WSL) formation utilizing a typical orthodontic treatment interval for the applications. Methods: In this double-blind, placebo controlled trial, 55 patients starting orthodontic treatment, either at Texas A&M University, Baylor College of Dentistry or at a private practice, were randomly divided into two groups (a treatment and placebo). Every eight weeks, the treatment group received an application of Nupro 1.23% APF gel (Dentsply®) for one minute, while the placebo group received a gel treatment for one minute that was exactly the same composition, but lacked fluoride. Patients were followed for 6 or 8 months depending on when their second set of brackets was placed. Pre and post treatment photographs and Fluorecam® readings were used to quantify the development of WSLs. Results: There was no statistically significant difference in the percentage of patients who developed WSLs, the number of new WSLs that developed on each tooth during treatment, or in the number of new WSLs developed per patient when using photographic analysis or Fluorecam® readings. Based on photographic assessments, 37.5% of patients in the fluoride group and 32.3% of patients in the placebo group developed new WSLs (p=0.685), while with Fluorecam assessments 37.5% of patients in the fluoride group and 58.1% of patients in the placebo group developed a new WSL (p=.130). Five patients had pre-existing WSLs (1 patient in the treatment group and 4 patients in the placebo group). Only patients in the placebo group demonstrated worsening of white spot lesions (p=.375). When using the Fluorecam® to quantify WSL development by measuring the size, intensity, and impact of each lesion, the only measurement which demonstrated a significant difference between the fluoride and placebo group was the intensity of the lower left first premolar (P
