Available in PDF, EPUB and Kindle. Book excerpt: On February 21 and 22, 1975, an International Workshop on the "Genetics of Diabetes Mellitus" was held in Göttingen, West-Germany. This workshop had been organized hy the Department of Medicine, University of Göttingen, and was generously sponsored by the Deutsche Forschungsgemeinschaft. Some forty geneticists and clinicians from Europe and N orth America, working in the field of diabetes both in man and laboratory animals, participated. The 25lectures presented at the workshop are now assembled for publication. Some of the animated discussion which followed the presentations has been included in the final papers by the speakers. Some lectures summarize the recent literature, others present data from recent research. Thus, a comprehensive and modem review of the theoretical and practical problems of diabetes are offered by this volume. related to the genetics The confusion about the mode of inheritance of diabetes mellitus during the last two decades can only be resolved by joint discussions between geneticists interested in diabetes and clinical diabetologists interested in genetics. Knowledge of modem genetics and of the heterogeneity of diabetes mellitus are necessary in order to disentangle the complex scene. Optimistically, the publication of this work on the genetics of diabetes will help to achieve this aim and to establish the concept of genetic heterogeneity of diabetes mellitus. Book excerpt: Since insulin became available for the treatment of diabetes in 1922 a number of major advances have been made, which include the modification of insulin to vary its timing of action, its purification, and latterly, the production of human insulin. Human insulin in quantities sufficiently large for therapy has been made available by two techniques developed in parallel during the late 1970s. These involve either (i) formulation in E. coli bacteria suitably encoded by DNA recombinant methods of the A- and B-chains of human insulin followed by a chain combination reaction ('biosynthetic' human insulin) or (ii) enzymatic conversion (transpeptidation) of porcine insulin brought to react with a threonine ester by porcine trypsin in a mixture of water and organic solvents, yielding human insulin ('semi-synthetic' human insulin). This book includes the first clinical-pharmacological studies of each of the highly purified 'semi-synthetic' human insulin preparations: Actrapid ® HM; Monotard® HM; Protaphane® HM; Actraphane® HM; and Ultratard® HM (Novo Industri A/S, Copenhagen). The preliminary studies established their safety and efficacy relative to their porcine and bovine counterparts emphasising the relevance of species and formulation on the pharmacokinetics and biological responses to insulin. Additional investigations with human insulin demonstrated the influence of insulin concentration, site of administration, the addition of aprotinin to insulin and the mixing of 'short-' and 'intermediate-acting' formulations on insulin 'bioavailability'. Examination of the 'within' and 'between' subject day-to-day variation in absorption and the effect of subcutaneous insulin also demonstrates the dominating influence of insulin responsiveness.

Book excerpt: Type 1 diabetes as well as multiple sclerosis are thought to be T cell mediated autoimmune diseases that involve a detrimental action of inflammatory cytokines and autoaggressive T lymphocytes. They still pose many unsolved puzzles, and the precise etiology as well as prevention have remained elusive. It is clear that genetic factors can predispose for developing diabetes, however, based on significant disease discordance found in monozygotic twins, additional environmental factors have to be postulated. Viruses are good candidates because they induce strong cellular and humoral immune responses, but no single etiologic agent has been identified. Several animal models are presented which have been used to study the activation of naive autoreactive lymphocytes. It is shown that regulation of the autoaggressive process occurs prior to clinical diabetes and is mediated by a complete network of cytokines, as well as regulatory circuits/cells. The spreading of autoimmunity to self-antigens not involved in the initial phase of islet destruction is not necessarily detrimental and can carry benefits. Therapeutically, counter-regulation of aggressive responses has been demonstrated via various means in animal models preventing diabetes or rejection of transplanted islets.
