Book excerpt: Moyamoya disease (MMD) is a chronic cerebrovascular disease characterized by progressive stenosis of the arteries of the circle of Willis, and formation of a collateral vascular network at the base of the brain. Moyamoya syndrome is caused by numerous conditions, which lead to arterial occlusion of the circle of Willis and resultant collaterals reminiscent of moyamoya disease. Due to the diverse angioarchitecture and unknown influence factors, the hemodynamic disturbances of MMD/Moyamoya syndrome are complicated, and result in multimodal clinical manifestations including headache, cognitive impairment, seizures, TIA, infarction, and hemorrhage. Surgical revascularization has been proven as effective in decreasing incidence of long-term risk of stroke. However, occurrence of perioperative stroke and complications such as seizures and cerebral hyperperfusion syndrome are still unpredictable. In addition, many patients with cognitive impairment cannot benefit from surgical revascularization and the mechanism remains unclear. Thus, a series of studies focusing on the objective evaluation of MMD/Moyamoya syndrome are needed to help understand this complicated cerebrovascular disease and improve our treatment strategy. The goal of this Research Topic is to better understand the pathophysiological nature of MMD/Moyamoya syndrome through objective neuroimaging. We also aim to generate a reasonable and objective evaluation system for MMD/ Moyamoya syndrome and would like to understand the link between cognitive impairment and brain damage of MMD/Moyamoya syndrome though neuroimaging. Through these findings we also aim to improve the treatment strategy by understanding the pathophysiological link with relevant complications. Finally, we also aim to improve our understanding of the significance of perioperative management for patients with severe complications from the perspective of objective evaluation.

Outcomes can be poor, with high mortality - possibly related to co-morbidities and the complexities of hospital care. Risk factors for in-hospital stroke include specific operations and procedures (such as cardiac surgery), previous medical disorders (especially a history of stroke or transient ischemic attack), and certain physiological characteristics (including fever and dehydration). More rapid diagnosis and evaluation for interventional therapies is needed. This major new book examines the causes of treatment-related stroke in most hospital-based situations. Therapeutic approaches – including interventional therapies and intra-arterial thrombolysis – are highlighted, including experimental agents and interventions where other treatment possibilities are limited. Increasing the awareness of such interventions is an important factor in reducing delays in the assessment of patients who have strokes while in hospital, thus decreasing morbidity and reducing costs.

The brain is our connection to the world around us and by governing nervous system and higher function, any disturbance induces severe neurological and psychiatric disorders that can have a devastating effect on quality of life. Our understanding of the physiology and biochemistry of the brain has improved dramatically in the last two decades. In particular, the critical role of cations, including magnesium, has become evident, even if incompletely understood at a mechanistic level. The exact role and regulation of magnesium, in particular, remains elusive, largely because intracellular levels are so difficult to routinely quantify. Nonetheless, the importance of magnesium to normal central nervous system activity is self-evident given the complicated homeostatic mechanisms that maintain the concentration of this cation within strict limits essential for normal physiology and metabolism. There is also considerable accumulating evidence to suggest alterations to some brain functions in both normal and pathological conditions may be linked to alterations in local magnesium concentration.
