Unfortunately, these reviews and supplements appear sporadically, and the reader can never be sure when a topic of special interest will be covered. Cancer Treatment and Research is a series of authoritative volumes which aim to meet this need . It is an attempt to establish a critical mass of oncology literature covering virtually all oncology topics, revised frequently to keep the coverage up to date, easily available on a single library shelf or by a single personal sub scription. We have approached the problem in the following fashion. First, by dividing the oncology literature into specific subdivisions such as lung cancer, genitourin ary cancer , pediatric oncology, etc. Second, by asking eminent authorities in each of these areas to edit a volume on the specific topic on an annual or biannual basis. The first chapters will review the interplay between pathogens and especially H. pylori, inflammation, genomic instability, and tumorigenesis. Secondly, an exhaustive overview of the diagnosis of gastric cancer from a clinical/endoscopic and pathologic perspectives will be illustrated. Two specific chapters will deal with tissue and serum biomarkers with an important role in diagnosis as well as prognostic and predictive markers of response to specific therapy. With the advent of next-generation sequencing and new preclinical model technologies, the genomic landscape of gastric cancer has been delineated and molecular characterizations finalized to novel therapeutic targets of each molecular subtype have been identified. These advances are making it feasible to integrate clinical and phenotype-based diagnostic and therapeutic methods and are described in a dedicated section. The text will conclude with chapters focusing on the future direction of gastric cancer research in the precision medicine era: an in-depth analysis of non-coding RNAs and their role in gastric cancer, the impact of immunomodulation/immunotherapy and the perspectives of nano-medicine will be finally proposed to the audience. A brief review of the existing literature addressing the above mentioned topics will be present in each chapter. Until operative resection pioneered by Bilroth and others became available, no effective treatment was feasible and death from cancer was virtually inevitable. Even with resection by total gastrectomy, the chances of tumor eradication remained small. Over recent years, however, the situation has been changing. Some changes have resulted from better understanding of the disease, early detec tion, and better management techniques with applied clinical research, but the reasons for other changes are poorly understood. For example, the incidence of gastric cancer is decreasing, especially in westernized societies, where it has fallen from one of the most common cancers to no longer being in the top five causes of cancer death. Still it remains the number one killer of adult males in Japan and Korea. Whether the reduced incidence in western societies is a result of dietary changes or methods of food preservation, or some other reason, is as yet uncertain. Improvements in outcome have been reported from mass screening and early detection; more refined techniques of establishing early diagnosis, tumor type, and tumor extent; more radical surgical resection; and resection at earlier stages of disease.

Time trends in cancer mortality vary in the individual cancers. Mortality from stomach cancer shows a decreasing tendency in most countries, although Japan still has the highest incidence in the world. Intestinal other than rectum and pancreatic cancers have shown an increase in most countries, while mortality due to liver cell and gallbladder cancer vary greatly by locality. Since most cancers are considered to be related to en vironmental and lifestyle exposures, such as diet, smoking or excessive drinking, there is hope that action on these factors may serve to substantially reduce occurrence of the disease. Recent progress in early diagnosis has made it feasible to detect small and minute cancers, and these have proven possible to cure with relatively favorable results. The most important advancement has come from a multidisciplinary approach to cancer treatment, utilizing a balanced application of surgery, radiation therapy and chemo therapy. A considerable increase in the five-year survival rate has been realized in stomach cancer. While progress is being made in the practical treatment of this disease, it remains far better to prevent than to cure. For the first time immunization offers a unique op portunity to prevent liver cell cancer.
