The specific aims of this study were: (1) determine the magnitude and scope of agricultural injury and asthma among adolescents in 9th-12th grades in rural Minnesota; (2) describe the change in work hours between 9th and 12th grades in terms of total work hours, and the shift in work hours between agribusiness, traditional family farm work, and non-farm work; (3) evaluate the reliability of adolescent self-reported information about agricultural and non-agricultural work hours and injury experiences; and (4) use a cohort analysis to calculate rate ratios for risk factors for injury and to facilitate planning for future prevention and intervention activities.

This study aimed to evaluate the effect of reaction time, its derived variables IQ, postural sway (steadiness), safety courses, and age with respect to childhood agricultural injury (CAT) resulting from the chore of feeding animals by hand. We assume that shorter reaction time is associated with lower CAI risk, controlling for potential covariates. Methods. The target population of this study was 4-H youth in the Central Ohio Area between 9 and 18 years old who were exposed to agricultural hazards. This thesis examined the subsets that were exposed to the possibility of injury while feeding an animal by hand (except pets). The larger cohort study was designed to be longitudinal. Data were collected during 1999-2001 in three stages: pre-baseline demographic information, baseline testing, and the daily diary ("Daily Record Books," or DRB) which contains the amount of time spent on each agriculture-related chores over a period of 13 weeks. A model of the reaction time (Krieg, Chrislip, & Russo, 1996) was used to compute the following variables: the mean reaction time (MRT), the initial performance level (L1), the performance limit (L), the learning rate (l), and the fatigue rate (f). Gender difference in factors such as age and derived variables from the reaction time data were examined using t-test and chi-square test. Longitudinal analysis by using residual pseudo-likelihood was used to estimate the expected injury rate and risk factors. Results. The results suggest that there are several differences in injury rate between boys and girls. The general safety course helped to prevent injuries in boys. The expected injury rate of those who had not taken the general safety course was 1.5 times higher than that of those who had taken the course in boys. Age and the fatigue rate of girls were an important factor in the expected injury rate. The expected injury rate of boys increased as the standard deviation of the reaction times decreased and as the week passed. For girls, the expected injury rates of the first two age groups were significantly different from that of the oldest group (p = 0.0003) and the second age group had the highest expected injury rate. The expected injury rate of girls increased as the fatigue rate increased and decreased as MRT increased. Conclusion. We found that gender difference in injury rates and risk factors including age, safety class, and psychomotor skills are associated with farming related injuries in children and adolescents. Interestingly, we found that a lower MRT coincided with higher expected injury rate of girls, and the lower standard deviation of the reaction time coincided with higher expected injury rate for boys. More studies are warranted to determine whether high risk work environment is associated with lower reaction time.

Through this report, the World Health Organization, the United Nations Children's Fund and many partners have set out to elevate child injury to a priority for the global public health and development communities. It should be seen as a complement to the UN Secretary-General's study on violence against children released in late 2006 (that report addressed violence-related or intentional injuries). Both reports suggest that child injury and violence prevention programs need to be integrated into child survival and other broad strategies focused on improving the lives of children. Evidence demonstrates the dramatic successes in child injury prevention in countries which have made a concerted effort. These results make a case for increasing investments in human resources and institutional capacities. Implementing proven interventions could save more than a thousand children's lives a day.--p. vii.

Youth living on farms are not only exposed to hazards while working, but also by merely residing on the farm. An estimated 1.03 million children and adolescents younger than 20 years of age resided on farms in 2010, with 15,011 youth injured on farms. On average, 113 youth die annually from farm-related injuries (1995-2002). Of the leading sources of fatal injuries to youth on U.S. farms, 23% involved machinery (includes tractors), 19% involved motor vehicles (includes all-terrain vehicles [ATVs]), and 16% were due to drowning. At the request of farm parents seeking guidance in assigning appropriate work to youth, the National Children's Center for Rural and Agricultural Health and Safety created the North American Guidelines for Children's Agricultural Tasks (NAGCAT). NAGCAT was based on child development principles and matched a child's physical, mental, and psychosocial abilities with the requirements of specific farm work. The NAGCAT initiative is a model for effective public-private partnership. It was funded primarily by the National Institute for Occupational Safety and Health (NIOSH); however, continued funding, resources, and partnership with the private sector has allowed for greater awareness and dissemination of NAGCAT. Impact: A randomized control trial was conducted on farms in central New York State to determine NAGCAT's efficacy in reducing childhood agricultural injuries. Active dissemination of NAGCAT to farm families over a one year period created several changes on the farm. Intervention farms made more safety-related changes than did control farms. In the 0 to 19 year age group, the amount of time that lapsed before occurrence of a NAGCAT-preventable injury was significantly longer for the intervention than the control group. Incidence of NAGCAT-preventable injuries decreased by half among seven to 19 year-olds on intervention farms where active dissemination of NAGCAT occurred versus control farms. Lastly, NAGCAT appeared to affect intermediate outcomes, such as setting limits on the amount of time a child could perform work between breaks, delaying initiation of ATV-use, and making more safety-related changes on the farm. Between 1998 and 2009, work-related injury rates among youth living on farms have declined. Several factors may have contributed to this decrease including: National Children's Center initiatives to establish guidelines, most notably NAGCAT and Creating Safe Play Areas on Farms; Congressional approval and funding of the National Action Plan for Childhood Agricultural Injury Prevention; leadership and funding by NIOSH; high-quality research that provided evidence of strengths and limitations of various interventions and policies; private donations to organizations such as Farm Safety 4 Just Kids and Progressive Agriculture Foundation for community-based safety programs; campaigns supported by media coverage; and a willingness of farm owners and parents to bury unsafe traditions and adopt new practices involving youth workers. Development, dissemination, and implementation of NAGCAT has demonstrated its effectiveness in reducing child agricultural injuries. However, the number of youth injuries and fatalities on farms continues to be a public health concern. A multi-faceted prevention strategy that includes use of NAGCAT is recommended as the next step in further reducing childhood agricultural injuries." - NIOSHTIC-2

This National Research Council book reviews the NIOSH Agriculture, Forestry, and Fishing Program to evaluate the 1) relevance of its work to improvements in occupational safety and health and 2) the impact of research in reducing workplace illnesses and injuries. The assessment reveals that the program has made meaningful contributions to improving worker safety and health in these fields. To enhance the relevance and impact of its work and fulfill its mission, the NIOSH Agriculture, Forestry, and Fishing Program should provide national leadership, coordination of research, and activities to transfer findings, technologies, and information into practice. The program will also benefit from establishing strategic goals and implementing a comprehensive surveillance system in order to better identify and track worker populations at risk.
