Book excerpt: Liver cancer is one of the most frequent human cancers and a major cause of cancerrelated death worldwide. Hepatocellular carcinoma (HCC) is generally diagnosed at an advanced stage of tumor progression, and more than 80% of the cases prove fatal, underscoring the need for a better understanding of the molecular mechanisms underlying hepatocarcinogenesis and identification of critical gene targets that could be exploited for early detection and therapeutic intervention. Epidemiological data indicate that the major risk factors for developing HCC are infection by hepatitis B virus (HBV) and hepatitis C virus (HCV) and heavy chronic alcohol intake. However, the molecular mechanisms by which these factors promote the development and progression of liver cancer are still unclear. It is believed that epigenetic mechanisms represent an interface between the genome and environmental exposures, suggesting that different etiologies may induce specific epigenetic changes at early disease stages that may promote further hepatocarcinogenesis. Specifically, DNA methylation has already been shown to be deregulated in HCC. However, the detailed profiling of DNA methylation in HCC and its correlation with risk factor exposures remains missing. The aim of this work has been to characterize the DNA methylation profiles in HCC and non-cancerous associated tissues of specific candidate genes to determine their association with tumor status and risk factors exposure using pyrosequencing technology. This study has allowed me to identify specific hypermethylated genes in HCC tumors, including non reported genes. Another important finding of my work is that aberrant DNA methylation of key cellular genes in HCC is associated with major risk factors, including HBV infection and alcohol intake. Given this interesting data, this analysis has been further extended using bead array technology in order to study the DNA methylation profile of more than 800 cancer-related genes. A strong signature distinguishing HCC from surrounding tissue and from other tumor types, independent of risk factors has been established. In addition, aberrant DNA methylation of an independent subset of promoters was associated with tumor progression and etiological risk factors (HBV or HCV infection, and ethanol consumption). Interestingly, distinct methylation of an independent panel of gene promoters was strongly correlated with survival after cancer therapy. This work provides important information concerning the targeted genes affected by DNA methylation in HCC and their association with risk factor exposure. Finally, our study revealed deregulation of a large panel of imprinted genes, suggesting that imprinting mechanisms may be targeted during HCC development. Because of the molecular and clinical distinction of subsets of HCC cancers, our findings could be exploited in molecular epidemiology and clinics for diagnostic and prognostic purposes, and provide the basis for the development of epigenetics-based strategies for risk assessment.

An obvious question was whether DNA methylation was a bearer of additional information to the genetic information and what was the nature of this information? It was tempting to speculate that DNA me thylation applied some form of control over programming of the genome s expression profile. Once techniques to probe the methylation profile of whole genomes as well as specific genes became available, it became clear that DNA methylation patterns are gene and tissue specific and that patterns of gene expression correlate with patterns of methylation. DNA methylation pat terns emerged as the only component of the chemical structure of DNA that exhibited tissue and cell specificity. This data seemingly provided an attrac tively simple explanation for the longstanding dilemma of how could one identical genome manifest itself in so many different forms in multicellular organisms? The DNA methylation pattern has thus become the only known factor to confer upon DNA a unique cellular identity.

Given the prevalence of HCC in global populations (currently >800,000 new cases per year, and rising), there is a requirement to identify molecular targets in HCC and develop new therapeutic strategies to manage and treat HCCs. New technologies and techniques, including gene sequencing, bioinformatics, and liquid biopsy, have helped identify molecular and genetic players involved in the development of HCC. As such, a comprehensive genetic/immune signature of HCC would be invaluable in the development of new therapeutics and treatment strategies, as well as early identification and management of HCC. There are currently various trials that implement a combination of already approved drugs to better the prognosis of patients suffering from HCC. There is, however, still room for improvement, and as our understanding of the various factors involved in HCC improves, so will the prognosis of patients suffering from HCC globally. This Research Topic aims to highlight the molecular and genetic targets that lead to the progression of Hepatocellular Carcinoma. We welcome Original Research, Reviews, and other article types.

Other prevalent diseases that have been associated with age-related epigenetic changes include cancer and diabetes. Paternal age and epigenetic changes appear to have an effect on schizophrenia and epigenetic silencing has been associated with several of the progeroid syndromes of premature aging. Moreover, the impact of dietary or drug intervention into epigenetic processes as they affect normal aging or age-related diseases is becoming increasingly feasible.
