A timely compendium of the modern day practice of LDK transplantation from a group of outstanding international experts, this text explores a number of controversial aspects of this innovative new technique. Discussing in detail the current situation, the authors also focus on the responsibility of the medical community to the live kidney donor as a patient, and the potential for complacency regarding donor risk. Emphasizing the ethical principles that must dictate medical practice in LDK transplantation for the foreseeable future - voluntarism, informed consent and medical follow-up - this book comprehensively records the best practices currently available.
Download or read book Pediatric Renal Transplantation written by Leonard Curtis Hymes and published by Nova Science Publishers. This book was released on 2014-12 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: The role of immunosuppression is essential to maintain the integrity of a renal transplant by preventing acute rejection. Among adult patients with solid organ transplants, standard immunosuppression does not vary among the major transplant centres in the USA and Europe, and consists of corticosteroids, calcineurin inhibitors and mycophenolic acid. However, children with renal transplants are more susceptible than adults to suffer adverse events related to this standard protocol, including short stature, nephrotoxicity, lymph proliferative disease and infections. For this reason, paediatric nephrologists have explored alternatives to standard immunosuppression while still maintaining acceptable graft outcomes. This book will explore this theme, which will include several chapters that present alternative immunosuppression protocol, in addition to other chapters that address challenging and controversial issues in paediatric renal transplantation. The first three chapters will describe immunosuppression protocols which may have specific benefits in children with renal allografts compared to standard regimens in adults. This includes the use of steroid-avoidance, which obviates many of the problems associated with corticosteroid use in children especially short stature and obesity. Secondly, the mTOR inhibitor Sirolimus is thoroughly discussed, including the potential benefits of this agent because of its lack of nephrotoxicity and possible anti-neoplastic properties. Lastly, induction therapy with Alemtuzumab is described, including recent studies that suggest that this agent may have definite benefits in children compared to standard induction with anti-thymocyte globulin.

Download or read book Controversies in Acute Kidney Injury written by J.A. Kellum and published by Karger Medical and Scientific Publishers. This book was released on 2011-09-15 with total page 267 pages. Available in PDF, EPUB and Kindle. Book excerpt: Acute kidney injury (AKI) is a serious and as yet incompletely understood disorder in which sudden impairment of kidney function occurs secondary to one or more of a variety of underlying conditions. This disorder is very common in (elderly) ICU patients and is associated with very high mortality. Many of those who survive suffer from permanent kidney failure and other long-term morbidities, which may include cardiovascular disease and immune dysfunction. Epidemiologic evidence suggests that AKI is not a single disease, but a syndrome comprised of multiple, often coexisting, etiologies. Being usually part of multiorgan failure syndrome, it calls for multiple organ support therapy. The publication at hand contains sections on prerenal azotemia syndromes, dying ‘of’ or ‘with’ AKI, pathophysiology of sepsis-induced acute kidney injury, developments in prevention / treatment / rehabilitation, and renal support. Reporting the latest recommendations from experts, it provides valuable information for those that are interested in understanding the disorder and its treatment options.
Download or read book Kidney to Share written by Martha Gershun and published by Cornell University Press. This book was released on 2021-05-15 with total page 223 pages. Available in PDF, EPUB and Kindle. Book excerpt: In Kidney to Share, Martha Gershun tells the story of her decision to donate a kidney to a stranger. She takes readers through the complex process by which such donors are vetted to ensure that they are physically and psychologically fit to take the risk of a major operation. John D. Lantos, a physician and bioethicist, places Gershun's story in the larger context of the history of kidney transplantation and the ethical controversies that surround living donors. Together, they help readers understand the discoveries that made transplantation relatively safe and effective as well as the legal, ethical, and economic policies that make it feasible. Gershun and Lantos explore the steps involved in recovering and allocating organs. They analyze the differences that arise depending on whether the organ comes from a living donor or one who has died. They observe the expertise—and the shortcomings—of doctors, nurses, and other professionals and describe the burdens that we place on people who are willing to donate. In this raw and vivid book, Gershun and Lantos ask us to consider just how far society should go in using one person's healthy body parts in order to save another person. Kidney to Share provides an account of organ donation that is both personal and analytical. The combination of perspectives leads to a profound and compelling exploration of a largely opaque practice. Gershun and Lantos pull back the curtain to offer readers a more transparent view of the fascinating world of organ donation.
Download or read book Organ Donation written by Institute of Medicine and published by National Academies Press. This book was released on 2006-09-24 with total page 359 pages. Available in PDF, EPUB and Kindle. Book excerpt: Rates of organ donation lag far behind the increasing need. At the start of 2006, more than 90,000 people were waiting to receive a solid organ (kidney, liver, lung, pancreas, heart, or intestine). Organ Donation examines a wide range of proposals to increase organ donation, including policies that presume consent for donation as well as the use of financial incentives such as direct payments, coverage of funeral expenses, and charitable contributions. This book urges federal agencies, nonprofit groups, and others to boost opportunities for people to record their decisions to donate, strengthen efforts to educate the public about the benefits of organ donation, and continue to improve donation systems. Organ Donation also supports initiatives to increase donations from people whose deaths are the result of irreversible cardiac failure. This book emphasizes that all members of society have a stake in an adequate supply of organs for patients in need, because each individual is a potential recipient as well as a potential donor.

Download or read book ABO incompatible Kidney Transplantation written by Kōta Takahashi and published by . This book was released on 2001 with total page 180 pages. Available in PDF, EPUB and Kindle. Book excerpt: ABO incompatible kidney transplantation is indicated for patients for whom no ABO-identical or minor mismatch donor is available. Since the author and his colleagues performed the first ABO-incompatible kidney transplantation in Japan in 1989, 400 such transplantations have been performed in 41 hospitals in Japan and this practice has contributed to a number of new developments. One is a clearer and more suitable model for conceptualising the mechanism of humoral immune response which enables identification of antigens and antibodies and a therapeutic strategy against rejection. The rejection mechanisms are discussed not only from the perspective of immunology but also viewed from different angles, including anatomy, microscopic and macroscopic pathology, molecular biology and haematology. Immunosuppressive therapy is discussed, divided in four categories: extracorporeal immunomodulation with removal of humoral antibodies; drug therapy to suppress cellular immunity; splenectomy; and anticoagulation therapy. Surgical procedures for kidney transplantation and splenectomy are treated, including discussion of the best timing for the latter. The book gives an overview of the current status with statistics and results of questionnaires and ends with discussions of 17 case histories.
