Drawing together the findings, an accident model has been proposed, illustrating the hierarchy of influences in construction accidents. The model describes how accidents arise from a failure in the interaction between the work team, workplace, equipment and materials. These immediate accident circumstances are affected by shaping factors, whereby the actions, behaviour, capabilities and communication of the work team are affected by their attitudes, motivations, knowledge, skills, supervision, health and fatigue. The workplace is affected by site constraints, work scheduling and housekeeping. The suitability, usability, condition and, therefore, safety of materials and equipment depend on their design, specification and supply/availability. permanent works design, project management, construction processes, safety culture, risk management, client requirements, economic climate and education provision. Achieving a sustained improvement in safety in the industry will require concerted efforts directed at all levels in the influence hierarchy.

Book excerpt: The United Kingdom (UK) construction industry is one of the worst industries in the UK in terms of health and safety (H&S) performance. Numerous injuries, deaths, dangerous occurrences and work related illnesses are reported annually in the industry, and these are associated with huge economic and social costs which make the need for H&S improvement inevitable. The pursuit of improvement has triggered studies into construction accident causation which have emphasised the need to pay attention to underlying accident causal factors which emanate from the pre-construction stage in order to have sustained improvement in H&S. Construction project features (CPFs), such as nature of project, method of construction, site restriction, procurement method, project duration, level of construction, design complexity, and subcontracting, which are organisational, physical, and operational characteristics of projects emanating from pre-construction decisions fall in this category of underlying causal factors. However, despite the significance of underlying causal factors to H&S, not much attention by way of research has been given to CPFs. As a result, insight into how CPFs influence accident occurrence, the degree of their inherent potential to influence accident occurrence (i.e. their potential to cause accident) and their associated degree of H&S risk (i.e. the likelihood of accident occurrence due to CPFs) remain elusive in the extant construction H&S literature. This research was thus undertaken to empirically investigate the mechanism by which CPFs influence accident occurrence and assess their degree of potential to influence accident occurrence and their associated H&S risk. Adopting a mixed method approach, the accident causal influence of CPFs was investigated. Following a conceptualisation of how CPFs influence accident occurrence based on systems models of accident causation, a qualitative inquiry involving semi-structured interviews with experienced construction professionals was undertaken to provide empirical verification of the conceptualised view. Subsequent to the qualitative inquiry, a questionnaire survey was undertaken to elicit relevant data from experienced professionals in construction management roles to enable the assessment of the degree of potential of CPFs to influence accident occurrence and their associated H&S risk. From the analysis of data, it was found that CPFs, emanating from pre-construction decisions, influence accident occurrence by their inherent introduction of certain associated H&S issues (which can be termed as proximal accident factors) into the construction phase of projects to give rise to accidents. There are also causal interactions between CPFs and the proximal factors which can reduce or increase the presence of proximal factors. CPFs have varying degrees of potential to influence accident occurrence which can generally be high or moderate and is influenced by: the extent to which their proximal factor(s) is common (in other words prevalent) within them; and the degree of potential of the proximal factor(s) to influence accident occurrence. Where CPFs apply on a project, they are generally associated with medium risk or high risk. Whereas with medium-risk CPFs some risk control measures would suffice in mitigating risk, with high-risk CPFs substantial measures are required. As a consolidation of the research findings, a toolkit, called CRiMT, has been developed. CRiMT provides H&S risk information regarding CPFs and it has the potential of assisting pre-construction project participants in managing the accident causal influence of CPFs from the early stage of project procurement. In view of the findings, the accident causal influence of CPFs should thus not be ignored or underestimated in construction project delivery. Pre-construction project participants, especially those whose decisions determine CPFs, ought to take into consideration the H&S effects of CPFs when making decisions which determine CPFs. Also, pre-construction project participants ought to plan and implement commensurate risk control measures in the early stage of projects to eliminate or mitigate the H&S risk posed by CPFs.

This "unacceptable level of fatalities" led to the commissioning of this inquiry, though Rita Donaghy remarks that there is no public sense of shock at the regular toll of fatalities in the industry and there is a need to raise the profile of these tragedies so that a construction fatality becomes socially unacceptable. The construction industry generally is modelled to provide maximum flexibility. Consequently the majority of functions are contracted out and at least 40 per cent of workers are self-employed or in the Construction Industry Scheme. The advantages are obvious in that it reduces overheads and some commentators argue that it improves profitability and productivity. The disadvantages are that it becomes more difficult for a safety culture to flourish, worker engagement is weak, employment security and continuity is minimal and skills training is at best patchy. The findings of the Review's investigation of a number of case studies reveal a combination of factors for each fatal accident of which the most frequently cited are: (a) the incidence of training factors, experience; (b) information and advice deficiencies; (c) risk perception; (d) rescheduling of work without planning; (e) minor / one-off jobs; (f) compliance; (g) equipment operability, space, personal protective equipment (PPE) issues and tools not designed to fit the user / task. 28 recommendations are made.

Book excerpt: ABSTRACT: The construction industry has a high rate of worker fatalities and considerable efforts are expended to find ways increase safety and curb the high death toll in the construction industry. The Occupational Safety and Health Administration (OSHA) is mandated to monitor the causes of fatalities as one way of identifying the areas that pose the greatest risk to construction workers. Whenever fatalities are investigated by OSHA, a determination is made about the cause of the fatality. There are five broad categories of causes of fatalities, including falls from elevation, electrocutions, struck by accidents, caught in/between accidents and other accidents. While most causes have been extensively examined, those fatalities attributed to "other" causes have received minimal attention. This is despite the fact that nearly ten percent of the construction worker fatalities are categorized as other accidents. This paper describes the results of a study of 795 incidents of construction worker fatalities occurring from 1990-2003 and attributed to "other" causes by OSHA. The incident data were analyzed for cause, frequency, and general circumstances surrounding the incidents.

These are rare but often calamitous events that occur in complex technological systems operating in hazardous circumstances. They stand in sharp contrast to ‘individual accidents’ whose damaging consequences are limited to relatively few people or assets. Although they share some common causal factors, they mostly have quite different causal pathways. The frequency of individual accidents - usually lost-time injuries - does not predict the likelihood of an organizational accident. The book also elaborated upon the widely-cited Swiss Cheese Model. Organizational Accidents Revisited extends and develops these ideas using a standardised causal analysis of some 10 organizational accidents that have occurred in a variety of domains in the nearly 20 years that have passed since the original was published. These analyses provide the ‘raw data’ for the process of drilling down into the underlying causal pathways. Many contributing latent conditions recur in a variety of domains. A number of these - organizational issues, design, procedures and so on - are examined in close detail in order to identify likely problems before they combine to penetrate the defences-in-depth. Where the 1997 book focused largely upon the systemic factors underlying organisational accidents, this complementary follow-up goes beyond this to examine what can be done to improve the ‘error wisdom’ and risk awareness of those on the spot; they are often the last line of defence and so have the power to halt the accident trajectory before it can cause damage. The book concludes by advocating that system safety should require the integration of systemic factors (collective mindfulness) with individual mental skills (personal mindfulness).

Book excerpt: In spite of the continuing decrease in accident rates in the US Construction industry, accidents are one of the key factors constraining construction industry in terms of cost and time. Fatality rates in the US are more than other developed countries. Research into accident causation included accident occurrence mechanisms, different levels and types of accident causes as and best practices for accident prevention. These efforts, however, did not focus particularly on fatal accidents and did not prioritize the relationships amongst root causes for accidents. Accordingly, this research's goal is to quantitatively analyze relationships between fatal accident root causes as well how they are tied to direct causes commonly quoted in fatality investigations using social network analysis (SNA). Social network analysis is a set of tools derived from mathematical graph theory and utilized in several knowledge areas including social sciences, natural sciences, engineering, construction management and safety. Accordingly, a three-step methodology is devised. First, 100 case files are analyzed for accident causation data. Second, an SNA model is built utilizing the relationships between root causes and broken down according to direct causes. Third, model is analyzed, and results are interpreted and validated to provide insights into fatal accident causation and contributing underlying factors. The analysis of all social networks yield that accident root causes interact very closely, and that only a few causes contribute most to fatality networks, a relationship which can be described by a power law. The model is capable of successfully identifying the most influential root causes for fatal accidents and their relationships. It identified "lack of job specific training" as the key cause for fatal accidents particularly for the direct causes "struck by" and "caught in between". Moreover, SNA showed that this cause is most influential when combined with "absence of fall arrest system" and "lack of personal protection equipment". Benefits of this research include providing a different approach to causation of fatal accidents that provides deeper more holistic insights into their root causes. Additionally, it provides a quantitative tool for prioritization of root causes which can guide implementation of Safety Management policies and practices.
