Book excerpt: According to Valentin (1833) and Luschka (1862), the first description of the structure now known as the carotid body must be ascribed to a Swiss physiolo gist - Albrecht von Haller - who, in 1762, called it the ganglion exiguum. This claim, however, may be erroneous, for Tauber (1743) described a struc ture at the bifurcation on the common carotid artery and called it the ganglion minutum. Andersch (1797) reprinted the text of a study made by his father between 1751 and 1755. The original printing of this work had apparently been sold as waste paper! Andersch called the organ the ganglion intercaroticum on account of its location. He also specifically stated that the sympathetic chain, the glossopharyngeal and the vagus nerves sent branches into the organ. For a while the carotid body remained forgotten, to be rediscovered in 1833 by Mayer of Bonn who again remarked upon the branches of the sympathetic, glossopharyngeal and vagus nerves as sources of a nerve plexus which innervated the ganglion intercaroticurtl. . Valentin (1833) clearly regarded the structure as part of the sympathetic nervous system, although he too recognised that the vagus and glossopharyngeal nerves contributed conspicuously to its innervation. Thus it is evident that the anatomists of the eighteenth and early nineteenth centuries regarded the structure in the carotid bifurcation as one of the many ganglia which are interspersed in the course of the sympathetic nervous system.

Originally described as a ganglion, it was subsequently regarded as a gland, chromaffin paraganglion and non-chromaffin paraganglion. In 1928 it was shown to be a chemoreceptor with close associations with the function of baroreception in the adjacent carotid sinus and perhaps within its own substance. These discoveries led physiologists to embark on a series of elegant experimental studies on a number of animal species which have, however, so far failed to identify the transducer for detection of changes in tension of arterial blood gases or the mechanism of chemor eception. Pathologists on the other hand have largely ignored the carotid body, restricting their interest to its tumour, the chemodectoma. A remarkable disparity in knowledge of the organ has resulted, with most information being available on the physiology of chemoreceptor tissue in laboratory animals. In contrast, there has been sparse interest and awareness of the pathology in man of this nodule of tissue lying in the carotid bifurcation whose functional activity is suggested by the high blood flow it receives, and its rich content of biogenic amines and a wide variety of peptides. This book is an attempt to redress this unsatisfactory situation. During the last few years our understanding of the detailed histology and ultrastructure of the human carotid body has improved.

In 1966 a meeting was organized by R. W. Torrance in Oxford and in 1973 by M. J. Purves in Bristol; in 1974 A. S. Paintal organized a satellite symposium of the Physiological Congress in Kashmir. The organizers of these meetings are to be commended for their efforts in publishing both the papers and discussions. At these meetings it has become apparent that the direction of research is becoming more sharpely focused on the cellular mechanisms of chemoreception. During the meeting in Dortmund the papers dealt mostly with the different cell types in the carotid body and their environment, i. e. , local P0 and local flow. These included 2 light and electron microscopic studies of the morphometric and histochemical pro perties of the different cells, microelectrode studies of the glomoid tissue to understand the conversion of a chemical stimulation into nervous activity, as well as strictly bio chemical and physiologic investigations concerning the dependence of the chemo receptive process on O consumption and the turnover of catecholamines. In spite 2 of the variety in methodology, it was apparent that all contributors had a common interest: to understand the mechanisms of chemoreception. Although at the meeting itself there was ample time to fully discuss the various papers, it has become necessary here to shorten the papers and discussions; otherwise the cost of publication would have been prohibitive.

Book excerpt: The carotid body (CB) is in charge of adjusting ventilatory and cardiovascular function during changes in arterial blood gases. Regardless this essential function, the CB has been implicated in the sensing of other physiological signals such as changes in blood flow and glucose levels. More important, malfunction of the CB chemoreceptors has been associated with the progression and deterioration of several disease states such as hypertension, heart failure, renal failure, insulin resistance, diabetes and sleep apnea. Although the mechanisms involved in the alterations of the CB function in pathophysiology are currently under intense research, the development of therapeutic approaches to restore normal CB chemoreflex function remains unsolved. Recent studies showing the effect of CB denervation in pathophysiology have unveiled a key role of these arterial chemoreceptors in the development of autonomic imbalance and respiratory disturbances, and suggest that targeting the CB could represent a novel strategy to improve disease outcome. Unfortunately, classical pharmacotherapy intended to normalize CB function may be hard to establish since several cellular pathways are involved in the CB dysfunction. Augmented levels of angiotensin II, endothelin-1, cytokines and free radicals along with decreases in nitric oxide had all been related to the CB dysfunction. Moreover, changes in expression of angiotensin receptors, nitric oxide synthases and cytokines that take place within the CB tissue in pathological states also contribute to the enhanced CB chemoreflex drive. It has been shown in heart failure, hypertension and obstructive sleep apnea that the CB becomes tonically hyper-reactive. During the progression of the disease this CB chemosensory facilitation process induces central nervous system plasticity. The altered autonomic-respiratory control leads to increased cardiorespiratory distress and the deterioration of the condition. The focus of this e-book will be to cover the role of the CB in pathophysiology and to provide new evidence of the pathways involved in the maladaptive potentiation of the CB chemoreflex function. It describes structural and functional properties of the cerebral circulation that are unique to the brain, an organ with high metabolic demands and the need for tight water and ion homeostasis. Autoregulation is pronounced in the brain, with myogenic, metabolic and neurogenic mechanisms contributing to maintain relatively constant blood flow during both increases and decreases in pressure. In addition, unlike peripheral organs where the majority of vascular resistance resides in small arteries and arterioles, large extracranial and intracranial arteries contribute significantly to vascular resistance in the brain. The prominent role of large arteries in cerebrovascular resistance helps maintain blood flow and protect downstream vessels during changes in perfusion pressure. The cerebral endothelium is also unique in that its barrier properties are in some way more like epithelium than endothelium in the periphery. The cerebral endothelium, known as the blood-brain barrier, has specialized tight junctions that do not allow ions to pass freely and has very low hydraulic conductivity and transcellular transport. This special configuration modifies Starling's forces in the brain microcirculation such that ions retained in the vascular lumen oppose water movement due to hydrostatic pressure. Tight water regulation is necessary in the brain because it has limited capacity for expansion within the skull. Increased intracranial pressure due to vasogenic edema can cause severe neurologic complications and death.
