Chez les patients atteints d'amylose cardiaque (AC), la dysfonction ventriculaire gauche et les troubles de conduction sont fréquents mais le potentiel bénéfice de la resynchronisation bi-ventriculaire dans l'amélioration du remodelage cardiaque et de la survie reste incertain dans cette cardiomyopathie.Notre objectif était d'étudier la survenue d'évènements cardiovasculaires (CV) (MACE : hospitalisation pour insuffisance cardiaque/mortalité cardiovasculaire) chez les patients atteints d'AC après l'implantation d'une stimulation bi-ventriculaire et basée sur la réponse échocardiographique (delta absolu de FEVG 10 %) comparé à une cohorte appariée grâce à un score de propension (1:1 pour l'âge, la FEVG à l'implantation et l'indication de la resynchronisation) de patients atteints d'une CMD idiopathique et implantés d'un stimulateur cardiaque bi-ventriculaire.Notre étude rétrospective a été menée sur 47 patients AC et porteurs d'une stimulation bi-ventriculaire entre janvier 2012 et février 2020, dans 9 centres hospitaliers universitaires français. Ce travail a permis de montrer que les patients AC ont des taux de réponse à la resynchronisation plus faibles que les patients du groupe CMD (36 % contre 70 %, p=0,002). Après analyse multivariée, l'amylose cardiaque était indépendamment associée à la survenue de MACE (HR 2,65, 95%CI,1,31-5,36, p=0,007), ainsi que l'absence de réponse échocardiographique à la resynchronisation cardiaque (HR 3,16, 95%CI,1,64-6,11, p=0,001). L'analyse en sous-groupe de la cohorte des patients AC nous a permis de déterminer que l'absence de réponse à la resynchronisation était le seul critère prédicteur de MACE au cours du suivi.Après l'implantation d'une resynchronisation bi-ventriculaire, et comparé à une cohorte appariée de patients atteints d'une cardiomyopathie dilatée d'origine idiopathique, les patients porteurs d'une amylose cardiaque ont des taux de réponse à la resynchronisation cardiaque plus faible et un pronostic cardiovasculaire plus sombre. Cependant, la stimulation bi-ventriculaire pourrait être bénéfique chez les patients porteurs d'une amylose cardiaque étant donné que la réponse à la resynchronisation était associée à une réduction du risque d'événements cardiovasculaires au cours du suivi.

Over the past few years it has become in creasingly clear that they are closely and causally interrelated and that their relationship can have a significant bearing on prognosis. Epidemiological studies have shown that arterial hypertension is one of the most important risk factors for de veloping heart failure. Only one in four patients with hyper tension is adequately managed, and in 50% of cases, the hypertension has not been recognised or treated. Patients with pre-existing hypertension who go on to suffer an acute myocardial infarction have usually not previously had typi cal angina symptoms, the infarct territory is larger, life threatening arrhythmias are commoner and hence in-hospi tal mortality and long-term prognosis are markedly worse. The presence of raised blood pressure in the post-infarct phase doubles the risk of manifest heart failure. The close relationship between hypertension, coronary heart disease and heart failure makes the choice of therapeu tic strategy particularly important. Agents and classes of agents that have prognostic value in all three conditions should be considered first, as synergy might result in addi tional benefits. In such patients, this sort of therapeutic deci sion-making might have further advantages. The use of these agents may prevent complications which are not yet clinically obvious (such as heart failure).

Special attention is paid to using leadless devices. The subcutaneous ICD obviates the need for transvenous leads and leadless pacemakers are entirely implantable into the right ventricle.
