Book excerpt: Cardiac Dynamics is the name of a relatively young field of study, born from the fruitful interaction between branches of two different disciplines: medicine and physics. "Dynamics" is the branch of physics which deals with the action of forces on bodies or particles in motion or at rest. "Cardiac" relates to the clinical field of cardiology but also to cardiophysiology, both of which are specialized branches of medicine. Narrower than the well established field of Hemodynamics, Cardiac Dynamics is restricted to dynamic phenomena occurring in and around the heart. The mathematical treatment of such phenomena, however, is vastly more complex because of the intricate nature of the mechanisms involved in the cardiac action. Thus, whereas hemodynamics is concerned with predominantly passive (visco-) elastic structures - vessels - containing time-variant flow of viscous flui- blood -, the mechanical study of the heart requires additional con siderations such as: active elastic components representing the contractile mechanism of cardiac muscle, complex geometry and fiber structure in the myocardial wall, autoregulatory mechanisms, and intricate flow patterns associated with valve motion. Viewed in this light it is not surprising that attempts to describe ventricular pump function and to quantify contractile performance have not reached the level of sophistication which is common in e. g. arterial hemodynamics. For the same reason, many of the often simplified approaches to describe ventricular mechanics failed to stand up to more rigorous theoretical, experimental or clinical testing.

Developments in radiological diagnostics have - technologically and medically speaking - focused on morphology. At present, computer-aided tomography (CAT) is at a high point in deve1opment, medical application, and validation. The preconditions for this success were rapid advances in electronics and computer technology - in hardware and in software - and an unexpected cost reduction in these fields; the co operation of various scientific disci plines was also essential. Functional radiological diagnosis has been neglected in part, owing to the emphasis on morphology, but alone the synthesis of morphology and function prornises further advances. Apart from the limited capabilities ofuItrasonic techniques there is no way other than using X-rays to carry out functional studies of organs and their systems through an intact body surface. It is frequently necessary to do further processing and evaluation of image series which have been recorded from the morphological viewpoint. This further picture processing may be of selected points (pixels) in the image, of certain regions of interest (ROI), or of the overall picture. For the measure ment of rapid phenomena, such as the blood flow in the main arteries, high image-frame rates are required, and at the moment these can only be achieved with cinemascopic techniques. For slower processes, other techniques such as videography have some advan tages.

Book excerpt: The Advanced Study Institute on Regression of Atherosclerotic Lesions, sponsored by the North Atlantic Treaty Organization, was held in Maratea, Italy, September 11-13, 1983. The institute was designed to foster discussion of experimental data on the regression of atherosclerosis in animals and evidence of a similar phenomenon in humans. The participating scientists showed with anatomical and biochemical data, that atherosclerosis regresses in cholesterol fed animals after withdrawal of atherogenic stimuli. The existence of regression in humans was inferred from sequential angiographic visualization of arteries, but anatomical and biochemical characteristics of the process have not been defined. Two mechanisms are probably involved in such an angiographic regression. The first, which may occur when hyperlipidemia is partly or fully corrected or during undernutrition or other metabolic abnormalities, may be related to atherosclerosis changes noted in animals undergoing regression. The second type seems independent of changes in plasma lipid levels and may involve resolution of thrombi or other underlying arterial wall factors. The relative frequencies of the two mechanisms are unknown; whether they can occur simultaneously also warrants consideration. It is likely that an exciting milestone will be forthcoming when regression of atherosclerosis is unequivocally demonstrated in humans. Publication of these institute proceedings constitutes a valuable background for future developments.

At the conclusion of the conference, those present decided unanimously that its success warranted having at least one more -- and that it was conceptually a sound idea to plan it on a biennial basis for late spring. Hence, the second Mid-Atlantic Conference on Bio Fluid Mechanics took place at Virginia Tech on May 4-6, 1980. This volume documents the Proceedings of the second conference. It contains full texts of 23 contributed papers, 2 guest lectures and 1 invited seminar. The papers are gr9uped according to subject matter, beginning with 3 in the area of respiration, followed by 1 in kidney dialysis, 1 in reproduction, 1 in joint lubrication, 1 in prosthetic fluidics, 2 in zoology, and ending with 14 in the general field of cardiovascular dynamics. Of the latter, 5 deal with the subject of heart valves, 2 concern themselves with the microcirculation, 6 address vascular system hemodynamics and 1 covers some aspects of blood rheology.
