Divers facteurs anatomiques, prothétiques, chirurgicaux nous amènerons à choisir entre la conservation de la canine dans sa position incluse ou son extraction. La mise en place d’implant trans-corono-radiculaire offre une alternative originale et conservatrice face à cette situation. Par ailleurs, lorsque l’extraction est choisie, les techniques de chirurgie pré-implantaire ont un rôle prépondérant dans le succès de réussite. Au travers d’une revue de la littérature et de cas cliniques, nous nous proposons de mettre en place un algorithme de prise en charge. Matériel et méthodes : Une revue de la littérature a été effectuée sur PubMed et Cochrane entre 1999 et 2020. Résultats : 15 articles ont été retenus pour cette revue de littérature. Parmi ces articles 8 illustraient des cas d’extraction/implantation immédiate de canine maxillaire incluse et 7 articles détaillaient la mise en place d’implant sans avulsion de la canine incluse au préalable. Parmi ces derniers, 4 traitait d’implant trans-corono-radiculaire. Conclusion : Cette revue nous permet d’élaborer un algorithme de prise en charge concernant la prise en charge implantaire en présence d’une canine maxillaire incluse.

Largement développé par les Docteurs Davarpanah et Szmukler-Moncler et se basant sur l'intégration minérale, cette technique remet en cause le postulat implicite de l'implantologie qu'un implant ne peut s'intégrer qu'avec un tissu autre qu'osseux, et ce sans induire de réaction inflammatoire ou délétère. Les différentes études décrivant les nouvelles interfaces entre implant et tissus dentaires, ainsi que les nombreux rapports de cas valident cette technique novatrice et aux nombreuses implications. Ce travail présente les caractéristiques et thérapeutiques de la canine maxillaire incluse, avant de développer la technique de l'implant transcorono-radiculaire. Enfin solution thérapeutique est illustrée par les cas cliniques présents dans la littérature.

Book excerpt: Arthritis is a very common joint disease. Knee and hip arthritis are increasing in humans and as the final treatment joints are replaced by implants. With more younger and active patients suffering from arthritis, a need has arisen to increase longevity by using customized implants. However, to obtain authorization for implantation of such prosthesis extensive trials are required. Arthritis is also very common in canines. One in every four dogs and cats are likely to develop arthritis before the end of their lives. Hip implants are commercially available for canines but due to large variations in sizes, knee (stifle) implants are not available. To conduct the clinical trials for the newly developed custom implant for humans and provide a new treatment for arthritis in canines, a custom design and manufacturing of knee implants for dogs is proposed herewith. The proposed custom design of implant has become possible with the advancement in medical imaging, bio-modeling, mass customization, reverse engineering, rapid prototyping, rapid tooling, and advanced CAD modeling. In this research, CT scan data was converted into CAD model and using advanced CAD modeling functions a patient specific canine knee implant was designed. A new process for manufacturing custom implants is also proposed. With the introduction of Electron Beam Melting (EBM) process, direct fabrication of fully dense components is now possible. The designed custom canine knee implants were manufactured in biocompatible Titanium alloy (Ti6Al4V) using EBM technology. This research introduces a complete process of designing and manufacturing of, firsts of its kind, custom canine knee implants.

This was a great tribute to the experts whose diligent efforts made it all possible. Apparently the book has stood the test of time to judge from the many inquiries and constructive comments made toward urging us on to write a second edition. It was not an easy task to gather another cadre of authorities to update our knowledge of the hip joint. People who have earned respected positions in their field are unavoidably burdened with a busy schedule, so a chapter in this text must be appreciated as coming from someone devoted to giving up some of his precious time for the sake of sharing his knowledge with peers and students.
