Lowu2010molecularu2010weight heparin (LMWH) remains the standard therapy for VTE in cancer patients.MethodsThis is an observational study conducted by the Hellenic Society of Medical Oncology (HeSMO) that aims to record and highlight the current clinical practice and management of VTE in Greek cancer patients.ResultsThe 18 centers reported approximately 4300 cancer patients managed on monthly basis, where the vast majority (80%) in outpatient setting. 340 patients with active cancer enrolled in this study: 53,2% males, mean age 64,3 years, PS 0u20101:62,1%, tumor types lung:22,3%, pancreas:16,3%, colon:13,6%, breast:11%, stomach:8,3%, ovarian:6,5% and other:21,7%. Moreover, 95,3% received treatment while 21,3% were inpatients and 78,6% outpatients. Among 340 patients, 254 had no VTE and 56,8% had PS 0-1. In the cohort of 254 patients, 72,8% were outpatients, 77,5% had metastatic disease and 95,7% were on treatment. Regarding cancer types 66,5% had high thrombotic risk cancers (23,2% lung, 18% pancreatic, 10,4% colon and 8% stomach). Interestingly, the Khorana mean risk in this population was 2 and all patients received prophylactic antithrombotic treatment with LMWH.ConclusionsGreek Oncologists are aware of cancer-associated thrombosis (CAT), especially in high thrombotic risk cancer types and advanced disease stages. Of interest, the majority of patients who received thomboprophylaxis had good PS and the Khorana mean risk score was 2.Key words: Venous ThromboEmbolism (VTE), Cancer-Associated Thrombosis (CAT), Khorana score, Thromboprophylaxis, Lowu2010molecularu2010weight heparin (LMWH).

CAT is common, could delay anti-cancer therapy and increase costs. Oncologists should be aware of CAT and its clinical significance.Methods: Prospective observational study conducted by HeSMO to record CAT clinical management. Patients with active cancer received antithrombotic agents for treatment or thromboprophylaxis were enrolled.Results: 546 patients enrolled. Primary cancers were: lung 24%, pancreas 13.4%, breast 8.8%, colorectal 8.1%, stomach 8.1%, ovarian 6.6% and other 39.1%. 120 received Low Molecular Weight Heparin (LMWH) for Venus ThomboEmbolism (VTE) treatment (Group A), 426 for thromboprophylaxis (Group B).Group A: 35% of 120 VTEs were diagnosed incidentally and treated as symptomatic [mean duration 5.51 months (SD+/-3.42)]. Recurrences occurred in 3 (2.5%) patients. Four (3%) grade 1 bleedings.Group B: 213 (50%) received LMWH at prophylactic doses while the rest received therapeutic doses [mean duration 4.42 months (SD +/- 2.68)]. 126 (30%) patients had Khorana score u22653. Even though, 300 (70%) patients had Khorana score u22642, 68% were metastatic and 58% were receiving high thrombotic chemotherapy agents. 16 (3.8%) patients experienced VTE while 9 (56%) of them were incidental. Notably, lower VTE risk [OR: 0.32 (95% CI 0.10, 1.0) p=0.04] was observed in patients on therapeutic doses LMWH while higher VTE risk [OR: 3,14 (1.01, 9.9)] was observed in patients on prophylactic doses LMWH. Six (1.4%) grade 1 bleedings.Conclusions: Oncologists use LMWH for the CAT management. Incidental VTE is a common and insidious clinical entity. Therapeutic doses of LMWH for thromboprophylaxis are effective and safe. Book excerpt: IntroductionThrombosis risk is estimated to be as high as 18% in cancer patients and a source of morbidity and mortality. The hemostasis study group organized members to develop clinical practice guidelines for cancer-associated thrombosis (CAT). The 2017 MASCC/ISOO workshop hosted by the hemostasis/education study group revealed that further education materials might be indicated to improve patient understanding.ObjectivesTo review the recent literature to determine recommendations on CAT with an initial focus on inpatient practice primary prevention. Of note, information will also be collected to develop patient friendly educational tools. MethodsThe co-chairs met with MASCC members periodically through teleconference to discuss project development. We adapted literature search methods similar to previous clinical practice guidelines published by MASCC/ISOO. The co-chairs conducted the initial literature search using MEDLINE and the Cochrane database and eliminated studies if the abstract revealed exclusion criteria. All other studies were pulled and divided to each participant. Grading and recommendations will follow previous guidelines by MASCC/ISOO.Results1589 studies were reviewed during the literature search. 1336 were eliminated based on the abstract. The following 253 studies have been assigned to each member and the full article will be reviewed for inclusion/exclusion criteria and grading, if applicable.ConclusionEach study participant will review at least 21 studies with the goal of developing guidelines and educational tools for inpatient CAT prophylaxis. The target date is to present to the participants of the Hemostasis study group in Vienna and to the executive leaders of MASCC/ISOO in 2019.

VTE is the second leading cause of death in cancer patients receiving chemotherapy. The incidence of VTE varies among cancers, and it is highest in pancreatic cancer patients. Increased white blood cells and thrombocytosis are risk factors for developing cancer-associated VTE. Some other proteins (tissue factor, podoplanin, P-selectin, and plasminogen activator inhibitor-1) may also play roles in thrombus formation in cancer patients. Certain diets and nutrition (e.g., enough fish, vegetables, and fruits) may reduce the risk of VTE. Certain diets and nutrition also may reduce the risk of cancer, and alcohol drinking and cigarette smoking definitely increase risk of cancer. Some studies suggest that aspirin, a widely used antiplatelet drug, may reduce cancer incidence and mortality, but other studies fail to show the beneficial effects of aspirin.

Book excerpt: Based on the results of randomized clinical trials (RCT) assessing direct oral anticoagulants (DOACs) for the treatment of patients with cancer-associated thrombosis (CAT), DOACs have been proposed as alternative to low molecular weight heparin (LMWH) by several international guidelines. However, the proportion of CAT patients who would have not been eligible for such trials is currently unknown. Aim. The primary aim of our study was to assess the proportion of patients seen in clinical practice for acute CAT and who would not have been eligible for a RCT. Secondary aims were to describe patients outcomes according to this eligibility. Methods. In a multicenter, observational study, all patients consecutively admitted from January 2017 to December 2019 for an acute CAT event were retrospectively analysed. Patients were classified according to the presence or absence of non-inclusion criteria from the CARAVAGGIO study. Death, venous thromboembolism (VTE) recurrence and major bleeding during a 6-month follow-up were analysed as secondary endpoints. Results. Three hundred and two patients with CAT were included (women: 53%, mean age: 67.9 ± 13.2). Among them, 161 (53%) met one or more criteria for non-inclusion (IC95% [47.68, 58.93]). Main exclusion criteria were unusual deep vein thrombosis (DVT) sites (n=56, 18.5%), bleeding risk (n=43, 14.2%), brain tumour (n=33, 10.9%), ECOG PS >2 (n=28, 9.3%), renal failure (n=16, 5.3%). At 6 months, the event-free survival rate was not statistically different between the two groups (55% for the non- eligible group versus 60% for the eligible group, P= 0.24). Conclusion. Among patients with CAT, a significant proportion would have not been able to participate in a trial as CARAVAGGIO. Evaluation of DOACs safety and efficacy in some of these specific setting needs further research.
