Although there have been enormous advances in immunology over recent years, much of the new knowledge in relation to infectious diseases was scattered over the world's literature and is now brought together in a single volume. Furthermore, while a number of previously unknown infectious diseases have recently been discovered (e.g. Marburg and Lassa virus infections and Legionnaires' disease) to which many new techniques have been applied, there seemed to be no clear statement of the rationale for their use, or for their further exploitation in some of the more common infectious diseases which are discussed in this book.

Which of the new ideas is sound ? Upon which data may we rely? How may we distinguish the real from the unreal, the true from the false, recent advances from recent retreats? These are the anxieties, often either unstated or alternatively expressed in terms of an attitude of total opposition to 'all of this research rubbish' which are widespread in medical circles today. It is for these individuals who are not themselves directly involved in the immunology of rheumatic diseases but who, nevertheless, recognize the importance of this subject to their patients and, in particular, to tomorrow's patients, that this book has been written. There are two points that I believe to be of central concern. Firstly, I think it important to recognize that the intellectual basis of most of the research work being generated today is actually extremely simple.

There should be specificity, so that tumour cells alone are destroyed whilst normal tissues are unaffected. Provided the tumour is uni form and all of the cells have appropriate antigens, every malignant cell should be destroyed and even distant metastases dealt with. So far these speculative advantages are unfulfilled and the initial optimism that surrounded im munotherapy has not been sustained. Acceptance of the precepts of tumour im munology continues but these disappointing observations had led to increasing scrutiny of certain aspects. The purpose of this chapter is to review the prin ciples which underly tumour immunology and immunotherapy, so that the more detailed studies that follow can be considered in perspective. TUMOUR ANTIGENS (Chapter 2) For a tumour to initiate an immunological response, it must possess distinctive antigens. Much of the early work in tumour immunity was confused because it was not appreciated that tumours, like other tissues, exhibit transplan tation antigens. Only when syngeneic tumours are used can tumour antigens alone be studied and it was the introduction of inbred mouse strains which allowed Foley in 1953' to produce the first evidence for specific an tigenicity of experimental tumours. Demonstration of these antigens requires that pretreatment with syngeneic tumour will influence the growth of a sub sequent challenge with the same neoplastic cells.

Book excerpt: As was shown in the first two volumes of this series, great strides have been made in identifying many of the agents or classes of substances responsible for carcinogenesis and in delineating their interactions with the cell. Clearly, the aim of such studies is that, once identified, these agents can be eliminated from the environment. Yet, despite these advances and the elimination of some important carcinogenic agents, one major problem exists. It is a constant monitor of all oncologic study and diminishes the importance of every experiment and of every clinical observation. As we noted earlier, that problem is our inability to define the malignant cell. It is through studies of the fundamental biology of tumors that we seek this definition. A vast amount of information has been gathered which describes what this cell does and-to a lesser extent-how it does it. But the why evades us. We have been unable to define the malignant cell, save in broad terms by comparing it to its normal counterpart. The major problem appears to be that the malignant cell does so much. It is a chimera, mystifyingly composed of normal activities and structures, of phenotypic schizophrenia with embryonic, fetal, and adult charac teristics and, occasionally, a hint of an unclassifiable capacity unique to malignant cells.
