Book excerpt: Abstract: Introduction: Self ligating brackets (SLB) manufacturers claim that expansion of arches and alveolar bone building is facilitated by their bracket design. The objective of this study was to determine whether SLB brackets have a different effect on buccal bone changes compared with conventional brackets (NSLB) during orthodontic treatment. Methods: Cone beam computed tomography (CBCT) images were obtained before and after treatment of 45 patients (26 treated with conventional brackets (NSLB) and 19 with SLB). The CBCT images were compared between groups for buccal bone height and thickness at the maxillary first molars, maxillary second and first premolars using imaging software. The intermolar distance and buccolingual angulation were assessed for the maxillary first molars as potentially confounding factors. Differences for each variable measured before and after treatment were compared between the two groups. Results: Initial and final thickness and height of buccal bone were significantly different between the teeth (p 0.001). The buccal bone height (p 0.001) and thickness (p= 0.018) significantly decreased after treatment for both groups. The mean changes of buccal bone height and thickness measured were not significantly different between SLB and NSLB groups (p0.05). The intermolar distance and molar angulation changes were not significantly different between the 2 groups (p0.05). Book excerpt: This study evaluates the anterior-posterior dental control of orthodontic patients treated with the Damon2 passive self-ligating bracket system. These patients were compared to patients treated with a conventional twin .022 (MBT prescription) bracket system. The Damon2 group (n=30) and the MBT group (n=33) consisted of patients who presented with mild to moderate dental crowding (measured by irregularity index). All patients had full complements of teeth, first molar to first molar, in both maxillary and mandibular arches. They were skeletal and dental Class I and all were orthodontically treated non-extraction. The measurements collected were U1-SN angle, Interincisal angle, IMPA and SN-GoGn. Measurements were taken from pre and post treatment lateral cephalometric head films. Measurements were repeated and paired samples t-tests were used to compare consistency of repeated measures. The t-test was used to compare the treatment change for the above measurements and also to compare the post-treatment results for the measurements. The IMPA treatment changes was also tested for high and low MPA patients in the two treatment groups. Data indicates that there was no significant difference between the two bracket systems with regard to treatment changes for incisor position. A comparison of the post-treatment measurements also shows no significant difference. Finally, no difference was found between the bracket systems for treatment of high and low MPA patients.

Book excerpt: Marketing strategies for self-ligating brackets have focused on less need for extractions and faster treatment. Recent meta-analyses have not found these claims to be well supported, but they point out limitations of the current evidence. These meta-analyses also conclude that additional, well-designed studies are indicated to improve our evidence base. The purpose of this retrospective cohort study is to assess the effects and efficiency of treatment with self-ligating brackets compared to conventional brackets. A secondary purpose is to identify any pretreatment malocclusion characteristics associated with choice of either self-ligating or conventional brackets. Patients treated in the private offices of two practitioners who simultaneously used both self-ligating and conventional brackets were selected for this study. The self-ligating subjects were selected first, and for each subject, an age and gender matched subject was chosen from the respective office. The self-ligation subjects were consecutively treated adolescents and young adults (11-25 years-old) who completed comprehensive fixed appliance therapy between January 1, 2011 and April 31, 2012. Patients with craniofacial anomalies, surgical treatment, treatment with arch expansion using expanders, interdisciplinary treatment, and incomplete records were excluded. The outcome measures were changes in the intercanine width, intermolar width, and arch length for both arches, change in mandibular incisor inclination, final PAR score, percent PAR reduction, overall treatment time, total number of visits, number of emergencies, number of broken brackets, and other emergencies such as a wire sliding to one side or the loss of ligation. All measurements were performed on digital models or digital lateral cephalograms in a blinded manner. PAR scores were measured by two calibrated assessors and the average of the two scores were used. Data were analyzed using the independent sample t-test, chi-square or fisher exact test and linear regression analysis. The final sample comprised 74 patients from Clinician 1, and 34 patients from Clinician 2. The two clinicians exhibited significant differences in treatment duration, as well as in treatment strategies, such as the use of the SureSmile technique. Therefore, the decision was made not to pool the data from the two offices. For Clinician 1, no significant differences were observed between self-ligating and conventional brackets for the outcomes described above, other than increased arch length in both arches in the self-ligating group. The self-ligation patients treated by Clinician 2 demonstrated a significant increase in transverse dimension, less percent reduction in PAR score, less treatment time, fewer visits, and more wire sliding emergencies compared to the conventional bracket group. While some significant findings were observed, the small sample and lack of consistent findings between the two clinicians makes it difficult to draw strong conclusions.

Methods: A survey was developed to evaluate how SL brackets compare to conventional brackets when perceived by practicing orthodontists (n=430). The initial series of questions focused on individual practitioner characteristics and the clinician0́9s experience with SL brackets, while the second part of the survey allowed the orthodontists to indicate a preference for either SL or conventional brackets in regard to a variety of treatment factors. Results: Most of the responding orthodontists (90%) had experience using SL brackets in clinical practice. This is often accomplished by means of self-ligating fixed appliances. When movements take the teeth outside those confines, without adoption, the muscular forces are unbalanced and may lead to dental relapse. Muscle spindles and associated reflex loops within the tongue provide feedback to arch perimeter changes that may produce postural changes to the new archform. Resting posture has long been accepted as aiding in tooth position. The objective of this study was to assess the oral and pharyngeal postural changes that result from arch development with the Damon system and report the amount of expansion accomplished. Methods: Pre- and post-treatment models and lateral cephalograms were collected on 69 previously treated orthodontic patients from four different private practices. Expansion was measured from the buccal cusp tips of the first and second premolars and first molars. A new cephalometric analysis was implemented to diagnose both variations in malocclusion and variations in posture of the head, neck, pharynx, hyoid bone and tongue. Results: Both tongue height and length increased, 2.9mm (P-value 0.001, SEM=1.06) and 3.76mm (P-value 0.00002, SEM=0.62) respectively, following posterior dental arch expansion using Damon archwires. Hyoid position was not significantly different. Conclusion: As dental arches are expanded the tongue increases in both length and height to fill the space and therefore may aid in stability during the retention phase of treatment. The lack of change in hyoid bone position, as one would expect with a rise in tongue position, might be explained by either slight changes in head position or the need to maintain the airway.

Book excerpt: This study compared the perception of pain experienced by patients undergoing orthodontic treatment with conventional, self-ligating brackets and aligners, and investigated the impact that pain had on their daily lives. 346 consecutive patients were included in the study: 115 patients treated with conventional brackets, 112 Patients treated with self-ligating brackets, and 119 patients treated with aligners. The quantitative aspect of pain was assessed using the Visual Analogue Scale, while the qualitative aspect of pain was evaluated using the Moroccan Short Form of McGILL Pain questionnaire. In all three groups experienced pain after activation tended to decrease in the following week. This pain was greater in patients with conventional braces and less in patients with aligners. Using the M-SF-MPQ to describe the qualitative aspect of the pain revealed that the ,Äúcramping ŸÖÿ≤Ÿäÿ±,,Äù ,Äúaching ÿTMŸäÿ£ŸÑŸÖ,Äâ,Äù aspect was most accentuated in the 3 groups. Medication intake was correlated with the intensity of pain experienced in all 3 systems. As for the impact of pain on daily activities, patients in groups of conventional and self-ligating braces showed more pain than those in the aligners group. Overall, aligners were less painful than conventional and self-ligating appliances. Patients did not suffer from an alteration in their quality of life due to orthodontic treatment.

It has been suggested that with the use of self-ligating brackets, less friction will be applied to teeth and reduced treatment time will be achieved with the potential benefit of less root resorption. Root resorption of teeth in patients undergoing fixed appliance therapy using self-ligating or conventional brackets was compared. Radiographs of twenty-six subjects were examined. Average length of appliance therapy for the MT group was 24.3 months and for the D2 group was 14.0 months. There was a significant difference between percent resorption of the maxillary laterals and mandibular centrals in the MT and D2 groups pre- and post-treatment. The percents of resorption between the two groups in the maxillary centrals and mandibular laterals were not significantly different. The degree of root resorption was less using the self ligating method.
