Book excerpt: The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) began 40 years ago as a pilot program and has since grown to serve over 8 million pregnant women, and mothers of and their infants and young children. Today the program serves more than a quarter of the pregnant women and half of the infants in the United States, at an annual cost of about $6.2 billion. Through its contribution to the nutritional needs of pregnant, breastfeeding, and post-partum women; infants; and children under 5 years of age; this federally supported nutrition assistance program is integral to meeting national nutrition policy goals for a significant portion of the U.S. population. To assure the continued success of the WIC, Congress mandated that the Food and Nutrition Service of the U.S. Department of Agriculture (USDA) reevaluate the program's food packages every 10 years. In 2014, the USDA asked the Institute of Medicine to undertake this reevaluation to ensure continued alignment with the goals of the Dietary Guidelines for Americans. This, the second report of this series, provides a summary of the work of phase I of the study, and serves as the analytical underpinning for phase II in which the committee will report its final conclusions and recommendations.

Book excerpt: The purpose of this study was to determine the effectiveness of the Breastfeeding Peer Counseling Program at the Chippewa County WIC clinic in Wisconsin. The study examined the relationship of sociodemographic variables to breastfeeding rates as well as factors promoting breastfeeding and barriers to breastfeeding in a sample of WIC participants. Effectiveness was determined by comparing program goals to pre-and post-implementation duration rates, and participant's ratings and recommendations of the peer counselor. Pre-and-post implementation data for all WIC mothers and prenatal WIC mothers were obtained using secondary data. In addition, a survey was administered to 68 women as they entered the WIC clinic for their individual WIC appointments, and data were analyzed for sociodemographic variables related to breastfeeding rates. Subjects were 18 years or older with infants born between May 1, 2010 and January 31, 2011. Most subjects were White ranging in age from 18 to 42. Approximately, half were employed and had completed technical school/some college. Findings showed that breastfeeding goals were significantly higher than breastfeeding duration rates. Significant, positive relationships were found for maternal age and duration rates, and maternal age and exclusive rates. Mothers with highest educational attainment had significantly higher breastfeeding duration rates. No other significant findings were observed in the study. Findings related to program effectiveness showed that all pre-to post-implementation duration rates increased, except for the 6 month rate, and percentage increases exceeded the targeted goals of the Peer Counseling Program; all participants who rated the peer counselor's assistance gave the highest possible rating; and all who used the Program would recommend it to other mothers.
