Book excerpt: The primary objectives of the DOE Program at the University of Tennessee Biomedical Imaging Center are the development of new boron-neutron-capture agents as well as the technology to detect boron compounds in-vivo. The detection technology focuses on the development of effective magnetic resonance imaging (MRI) and spectroscopy (MRS) techniques for verifying and measuring BNCT agents in-vivo. A significant portion of the effort is directed toward the design of boron-containing neutron-capture-therapy agents. The UT -- DOE program is unique in that it has access to two state-of-the-art multinuclear magnetic resonance imaging units housed in the Biomedical Imaging Center at the University of Tennessee Medical Center at Knoxville. In addition the UT -- DOE researchers actively collaborate with colleagues at other DOE facilities (Brookhaven National Laboratory, Oak Ridge National Laboratory, Los Alamos National Laboratory and Oak Ridge Associated Universities). An important goal of the DOE program at UT is to provide training for students (predoctoral and postdoctoral). The University of Tennessee is one of the very few institutions in the world where students have hands-on'' access to both modern scientific equipment and medical imaging modalities such as the clinical MRI units. The academic nature of the program facilitates collaborative interactions with other DOE programs and helps to insure the continued availability of skilled scientists dedicated to the advancement of diagnostic medical procedures. 14 refs., 3 figs.

Neutron capture therapy (NCT) is the binary system that has been the subject of the Fifth International Symposium on Neutron Capture Therapy, which was held September13-17, 1992, in Columbus, Ohio, undertheauspicesoftheInternational Society for Neutron Capture Therapy. Its objective was to bring together researchers from throughout the world and to provide a forum at which they could present the latest advances in the development of Neutron capture therapy. Neutron capture therapy has largely, but not exclusively, focused on the use of boron-10 as the target nuclide. Boron neutron capture therapy (BNCT) is based on the nuclear reaction that occurs when the stable isotope, boron-10, absorbs low-energy non ionizing thermal neutrons to yield alphaparticles and recoiling lithium-7 nuclei. The size and energy of these high linear energy transfer (LET) particles result in their being confined largely to the cells in which the capture reaction occurs. For BNCT to be successful, a sufficient numberof I~atoms mustbe localized within neoplastic cells, and enough thermal neutrons must be delivered and absorbed by the I~ to produce a lethal 1~(n,QVLi reaction. Two major problems must be surmounted.

Methods to confirm the kinds and grades of glioma have taken a path from classical macro- to microscopic pathohystological confirmation of tumors, through morphological-histological, molecular, and genetic diagnosis. Surgically, progress was made possible with the development and use of technological aids, for example neuronavigation, cortical mapping, electrocorticography, neuromonitoring, functional and intraoperative MRI, magnetoencephalography, etc. Great hope was placed on the extension of tumor resection and popular supratotal resection. Significant progress has been made generally in glioma treatment with the use of modern radiotherapy and new chemotherapeutics. What do we want to see for the future? By way of stem cells, a specific medicine will be produced, individualized for the particular patient, and by using a microcapsule it will be implanted into the brain zone affected by the tumor by way of robot surgery and injection needle. This is not at all an unrealistic expectation in the next decade or two.
